In Belgium, between 1 March and 31 December 2022, 25 cases of toxigenic C. diphtheriae infection were confirmed among asylum seekers, mostly among young males from Afghanistan. Multi-locus sequence typing showed that most isolates belonged to sequence types 574 or 377, similar to the majority of cases in other European countries. The investigation and management of the outb...

In Service to Their Patients and Their Country
Caring. Effective. Dutiful. Supportive. Serving. Protective. Leading. Responsible. These words easily describe a hospitalist or a soldier—and sometimes both. In honor of Veteran’s Day, we talked with four SHM members, working hospitalists, who also have served, or currently serve, in the military. While their paths differ, their commitment to their patients and their country is the same. Jon Sweet, MD, FACP (@jonmsweet) Dr. Sweet Chair of medicine and senior vice president, Carilion Clinic, professor, Virginia Tech Carilion School of Medicine in Roanoke, Va., former major, United States Air Force Dr. Sweet began his ...

Intussusception risk following oral monovalent rotavirus vaccination in 3 Asian countries: A self-control case series evaluation
Vaccine. 2023 Oct 24:S0264-410X(23)01225-2. doi: 10.1016/j.vaccine.2023.10.042. Online ahead of print.ABSTRACTRotavirus vaccines have substantially decreased rotavirus hospitalizations in countries where they have been implemented. In some high- and middle-income countries, a low-level of increased risk of intussusception, a type of acute bowel obstruction, has been detected following rotavirus vaccination. However, no increased risk of intussusception was found in India, South Africa, or a network of 7 other African countries. We assessed the association between a 2-dose monovalent rotavirus vaccine (Rotarix) and intussus...

Survey-based Women Empowerment Index for Afghanistan (SWEI-A): An Explanatory and Confirmatory Factor Analyses
This study aimed to develop a country-specific index to measure women empowerment among married women aged 15 –49 years in Afghanistan. The data from the 2015 Afghanistan demographic health survey (ADHS) was used to develop the index. The data on 26 variables across eight hypothesized domains related to women empowerment were used in EFA to probe the underlying domains in the data. CFA examined the struct ural validity of hypothesized factors in EFA. Four indicators were dropped during the analysis either due to the low and significantly different loading on one factor as compared to other indicators or due to overlap wi...

Spatiotemporal multi ‐graph convolutional network‐based provincial‐day‐level terrorism risk prediction
This study proposed a novel spatiotemporal graph convolutional network (STGCN)-based extension method to capture the complex and multidimensional non-Euclidean relationships between different provinces and forecast the daily risks. Specifically, three graph structures were constructed to represent the contagious process between provinces: the distance graph, the province-level root cause similarity graph, and the self-excited graph. The long short-term memory and self-attention layers were extended to STGCN for capturing context-dependent temporal characters. At the same time, the one-dimensional convolutional neural netwo...

High risk of intestinal colonization with ESBL-producing Escherichia coli among soldiers of military contingents in specific geographic regions
AbstractOne-hundred Polish soldiers of a contingent in Afghanistan in 2019 were screened forEnterobacterales resistant to newer-generationβ-lactams at their departure and return. Seventeen percent were colonized in the gut at the departure, whereas 70% acquired carriage in Afghanistan. The commonest organisms were extended-spectrumβ-lactamase (ESBL)-producingEscherichia coli (ESBL-Ec; 96.6%). All isolates were sequenced and were clonally diverse overall, even within the same sequence type, indicating that independent acquisitions mainly. ESBL-Ec were often multi-drug-resistant. Soldiers stationing in certain regions are ...

Two-Stage Endovascular Treatment of  a Traumatic Pseudoaneurysm of Superior Mesenteric Artery Branch in a Soldier After a Military Combat Mission
This article presents a case of a 50-year-old former active duty soldier and veteran of a military combat mission in Afghanistan, during which he sustained a traumatic injury resulting from a mine explosion under a vehicle. After completing the mission and returning home, the patient developed abdominal pain. The diagnosis made in the general surgery department of the district hospital was upper gastrointestinal obstruction and aneurysmal rupture of the superior mesenteric branch with inflammatory infiltration of the pancreatic-intestinal area. The patient underwent emergency gastrointestinal anastomosis and Braun enteroen...
