Thiamine deficiency as a cause of anorexia in terminally ill cancer patients: A case of possible gastrointestinal beriberi
The objective of this report was to describe such a case.METHODS: An 82-year-old woman with advanced cervical cancer was admitted for palliative care because of progressive disease. One month after admission, she developed myoclonus on the left side, difficulty speaking, slurred speech, and drooping of the left corner of the mouth, with no consciousness disturbance. She also developed nausea and complained of decreased dietary intake for more than 2 weeks.RESULTS: The patient's thiamine level, which had been measured 2 weeks earlier, was 27 ng/ml, which was below the normal range, and 100 mg of thiamine was administered in...

Thiamine Deficiency Neuropathy in a Patient with Malnutrition due to Melancholic Depression
CONCLUSION: In our case, the rapid weight loss facilitated a cascade of complications related to nutritional deficiencies. Based on our clinical observations and the literature, thiamine deficiency should be considered in the presence of malnutrition and vulnerability, both on an organic and psychiatric level.PMID:38495842 | PMC:PMC10942818 | DOI:10.1155/2024/1797983 (Source: Case Reports in Psychiatry)

Severe Fatigue in Inflammatory Bowel Disease: Dopaminergic Therapy With Modafinil or Vitamin Therapy With Thiamine
We found Moulton et  al’s1 illustrative case series of 10 patients with inflammatory bowel disease (IBD) and chronic fatigue, all presenting with depression, particularly interesting. 1 Among the patients, 8 previously had undergone treatment with multiple psychotropic medications, and 2 had active IBD as indicated by increased fecal calprotectin levels. Remarkably, all 10 patients responded positively to open-label treatment with modafinil, a central nervous system stimulant that blocks dopamine reuptake transport, which resulted in an impressive improvement in their fatigue symptoms. At baseline, the self-re ported me...

Comprehensive metabolomics analysis reveals novel biomarkers and pathways in falsely suspected glutaric aciduria Type-1 newborns
CONCLUSIONS: A distinct metabolic pattern linked to the transient C5DC elevation in newborns was reported to enhance the prediction of the falsely positive cases, which could help avoiding unnecessary medical treatments and minimizing the financial burdens in the health sector.PMID:38490341 | DOI:10.1016/j.cca.2024.117861 (Source: International Journal of Clinical Chemistry)

Vitamin C for Patients Hospitalized With COVID-19 —Reply
In Reply Dr McKenzie and colleagues raise concerns regarding the safety of bundling thiamine with other vitamins into a product primarily prescribed for thiamine replacement in patients with or at risk of alcohol withdrawal. This formulation consists of 2 ampules (thiamine, riboflavin, and pyridoxine in ampule 1; ascorbic acid, nicotinamide, and glucose in ampule 2) that are intended to be diluted together in normal saline or 5% dextrose and infused. We are aware of a similar bundled injectable vitamin solution in other countries. (Source: JAMA)

Optimization of bacterial cellulose production by Komagataeibacter sucrofermentans in synthetic media and agrifood side streams supplemented with organic acids and vitamins
Bioresour Technol. 2024 Mar 2:130511. doi: 10.1016/j.biortech.2024.130511. Online ahead of print.ABSTRACTThe effect of thiamine (TA), ascorbic acid (AA), citric acid, and gallic acid (GA) on bacterial cellulose (BC) production by Komagataeibacter sucrofermentans, in synthetic (Hestrin and Schramm, HS) and natural substrates (industrial raisins finishing side stream extract, FSSE; orange juice, OJ; green tea extract, GTE), was investigated. The Response Surface Methodology was found reliable for BC yield prediction and optimization. Higher yields were achieved in the FSSE substrates, especially those supplemented with AA, T...
