Vasculitis that did not read the books
A 29-year-old woman presented to our rheumatology clinic with a 6-month history of cough, dysphonia, dyspnoea and weight loss. She also complained of a lump in her throat (globus pharyngeus), left otalgia and hearing loss. One month before evaluation, she started experiencing blood-tinged sputum and dyspnoea. On clinical examination, the patient had nasal discharge, crusting and congestion, laryngeal oedema, stridor, tympanic membrane retraction and external auditory canal ecchymosis. Laboratory findings showed anaemia (haemoglobin: 75.4 g/L, normal: 122.0–181.9), lymphocytosis 4.63x109/L (normal: 0.60x109–3.4x...

Immune checkpoints in autoimmune vasculitis
Best Pract Res Clin Rheumatol. 2024 Apr 9:101943. doi: 10.1016/j.berh.2024.101943. Online ahead of print.ABSTRACTGiant cell arteritis (GCA) is a prototypic autoimmune disease with a highly selective tissue tropism for medium and large arteries. Extravascular GCA manifests with intense systemic inflammation and polymyalgia rheumatica; vascular GCA results in vessel wall damage and stenosis, causing tissue ischemia. Typical granulomatous infiltrates in affected arteries are composed of CD4+ T cells and hyperactivated macrophages, signifying the involvement of the innate and adaptive immune system. Lesional CD4+ T cells under...

Accelerated atherosclerosis in Anti-Neutrophil Cytoplasmic Antibody-Associated Vasculitis: case report
We describe the case of a 60-year-old patient with a history of hypertension, diabetes mellitus, and chronic kidney disease of unknown etiology, who presented two acute coronary syndromes with only a six-month difference. Rapid progression of coronary involvement was evidenced, along with increased markers of inflammatory response, usual interstitial pneumonia on tomography, and positive anti-myeloperoxidase antibodies (anti-MPO), leading to the diagnosis of microscopic polyangiitis (MPA). In these cases, timely diagnostic suspicion is crucial, as early treatment significantly impacts the course and prognosis of the diseas...

An Autopsy Case of Antineutrophil Cytoplasmic Antibody-Associated Vasculitis Induced by Propylthiouracil
Intern Med. 2024 Apr 9. doi: 10.2169/internalmedicine.3397-23. Online ahead of print.ABSTRACTAntineutrophil cytoplasmic antibody (ANCA)-associated vasculitis (AAV) is a complication caused by antithyroid drugs, particularly propylthiouracil (PTU). Most patients experience organ failure due to the affects of the treatment regimen. We herein report the case of an 89-year-old woman whose severe AAV induced by PTU resulted in various instances of organ failure that eventually led to death after 9 years of PTU therapy. During autopsy, we identified five types of organ failure. As AAV is a potentially fatal disease, the developm...

MR Vessel Wall Imaging for Atherosclerosis and Vasculitis
Conventional imaging modalities, such as computed tomography angiography, MR angiography, transcranial Doppler ultrasonography, and digital subtraction angiography, are utilized in evaluating intraluminal or intravascular pathology of the intracranial vessels. Limitations of luminal imaging techniques can lead to inaccurate diagnosis, evaluation, and risk stratification, as many cerebrovascular pathologies contain an extrinsic vessel wall component. Furthermore, vessel wall imaging can provide information regarding extent, treatment response, and biopsy targets for vasculitis cases. Overall, while vessel wall imaging can p...

Proteinase ‑3‑antineutrophil cytoplasmic antibody‑associated vasculitis secondary to subacute infective endocarditis: A case report
Exp Ther Med. 2024 Mar 20;27(5):216. doi: 10.3892/etm.2024.12504. eCollection 2024 May.ABSTRACTA 58-year-old male patient was admitted to Peking University First Hospital (Beijing, China) due to recurrent hematuria, proteinuria and kidney dysfunction. The patient was positive for proteinase-3 (PR3)-antineutrophil cytoplasmic antibody (ANCA). Pathology of the kidney showed focal proliferative necrotizing glomerulonephritis with crescent formation and immune complex-mediated glomerulonephritis. The patient was diagnosed with PR3-ANCA-associated vasculitis (AAV), received intensive immunosuppressive therapy and experienced tw...

Blood Whispers: Exploring Hematologic Indicators for Diagnosing and Predicting Severity of Vogt-Koyanagi-Harada Syndrome
CONCLUSION: CBC-derived inflammatory parameters indicate that VKH is a systemic inflammation. These parameters can be used in the diagnosis and determination of disease severity of VKH.PMID:38592364 | DOI:10.1080/09273948.2024.2334793 (Source: Ocular Immunology and Inflammation)
