Renal artery occlusion in a young woman - a tale of mysterious thrombosis
CONCLUSION: Renal artery thrombosis in young people is extremely rare and presents a diagnostic and management challenge requiring input from multiple teams including nephrology, rheumatology, paediatrics and vascular surgery. Systemic coagulopathy and vasculitis are differentials against anatomical aetiologies such as fibromuscular dysplasia. Our case adds to the limited literature regarding this in the young population. Renal artery thrombosis with occlusion in young people is very rare and is most often associated with a systemic coagulopathic disorder, such as antiphospholipid syndrome or structural pathology of the re...

Current perspective on infections and mitigation strategies in primary systemic vasculitis
AbstractPurpose of ReviewThe purpose of this review is to summarize and evaluate most recent evidence on the epidemiology of infections and associated risk factors in patients with primary systemic vasculitides (PSV), as well as discuss mitigation strategies including the risk of antibiotic prophylaxis.Recent FindingsInfections remain one of the leading causes of mortality in patients with PSV, with rates of severe infection ranging from 16 to 40% in different cohorts. Older age, frailty, renal and pulmonary involvement, and higher burden of comorbidities have been recognized as important patient-associated risk factors. T...

Hepatic micro ‐abscesses as an unusual initial presentation of systemic lupus erythematosus: A case report
We report the case of a 27-year-old female who presented with persistent high-grade fever, bilateral exudative lymphocytic pleural effusion, hepatic micro-abscesses, anemia, and lymphopenia. Despite extensive investigations and antibiotic therapy, the patient's condition continued to worsen. The diagnosis of hepatic vasculitis, a rare manifestation of SLE, was ultimately made based on clinical suspicion, positive autoimmune markers, and negative septic workup. The patient responded well to high-dose corticosteroid therapy and intravenous immunoglobulin, with resolution of liver lesions and clinical improvement. Hepatic inv...

Towards better anticoagulation monitoring for extracorporeal membrane oxygenation – Taking C-reactive Protein Out of the Equation?
C-reactive protein (CPR) is an acute phase reactant protein, primarily synthesized in the liver.1 It belongs to the Pentraxin family of the innate immune system, serving as a pattern recognition molecule for phosphorylcholine residues abundant in bacterial cell membranes. It also binds to damaged cells and altered self-antigens, making it a valuable marker for inflammation, particularly in conditions such as rheumatoid arthritis and certain types of vasculitis. CRP exhibits a notable affinity for oxidized phosphatidylcholine (lysophosphatidylcholine) expressed on apoptotic cells and in oxidized low-density lipoproteins (LD...

Influence of Post-radiation Ocular Surface Disorder on Ocular Surgery: A Case Report and Review of the Literature
CONCLUSION: The continuous worsening of clinical symptoms of the disorder of the ocular surface after proton beam radiotherapy can be the result of a post-radiation syndrome. Gradual expansion of ischemia, vasculitis, and inflammatory mediators compresses the retinal tissue, leading to recurrent macular edema as well as to secondary glaucoma and corneal decompensation. Band keratopathy is occasionally noted and seems to result from severe post-radiation disorder of the ocular surface. However, PKP would typically be indicated in cases of corneal perforation, uncontrolled infectious keratitis, or for improving vision in the...

Adverse events in biologics for severe asthma
CONCLUSION: Biologics for severe asthma are recent drugs with a favorable safety profile, but with little real-life experience, justifying increased vigilance by prescribing physicians.PMID:38653607 | DOI:10.1016/j.rmr.2024.04.001 (Source: Revue des Maladies Respiratoires)

A case of spontaneous coronary artery dissection that later turned out to be Takayasu arteritis
J Cardiol Cases. 2024 Jan 16;29(4):186-189. doi: 10.1016/j.jccase.2023.12.009. eCollection 2024 Apr.ABSTRACTSpontaneous coronary artery dissection (SCAD) is a rare cause of acute coronary syndrome (ACS), which typically occurs in women at low risk of atherosclerosis. We herein report a case of SCAD in a 57-year-old man who later developed Takayasu arteritis. The patient presented to our hospital complaining of chest pain and was diagnosed with unstable angina. Emergent coronary angiography was performed, and optical coherence tomography revealed that ACS was caused by SCAD. The patient was treated medically without further...
