Should we liberalize potassium intake in CKD? No, we should not
Although a high dietary sodium (Na) intake has dominated our understanding of pathophysiology of hypertension, the moderating effects of a dietary potassium (K) intake on blood pressure (BP) are less appreciated. K depletion in normotensive men can increase BP,1 supplementation of K among normotensive women can modestly lower 24-hour ambulatory BP,2 and in those with untreated hypertension3 or among those with hypertension and hypokalemia provoked by diuretics, K supplementation can lower BP.4 A cluster randomized trial from 600 villages in China in people with a history of previous stroke (72.6%) or a history of hypertens...

Association of acute kidney disease with the prognosis of ischemic stroke in the Third China National Stroke Registry
Acute kidney disease (AKD) evolves a spectrum of acute and subacute kidney disease requiring a global strategy to address. The present study aimed to explore the impact of AKD on the prognosis of ischemic stroke.

Association of Urine Albumin-Creatinine Ratio and Cystatin C-Based Estimated GFR with Outcomes in Patients with Ischemic Stroke
Conclusions: Both decreased eGFRcys and albuminuria are independent risk factors for all-cause death and poststroke disability. Combining the two markers is useful for improving risk stratification even in those without chronic kidney disease.

Association of Urine Albumin-Creatinine Ratio and Cystatin C –Based Estimated GFR with Outcomes in Patients with Ischemic Stroke
Background/Aims: Data about the independent and combined effects of cystatin C based estimated glomerular filtration rate (eGFRcys) and albuminuria on the risk of poor outcome in stroke patients is limited. The aim was to elucidate how these two renal markers affect the clinical outcomes after ischemic stroke separately and jointly. Methods: The study subjects consisted of 10,197 patients with ischemic stroke from the third China National Stroke Registry (CNSR-III). The study outcomes were all-cause mortality, post stroke disability, recurrence of stroke and cardiocerebral vascular disease (CVD) composite events. Cox prop...

Premature Stroke Secondary to Severe Hypertension Results from Liddle Syndrome Caused by a Novel SCNN1B Mutation
Conclusions:We identified a novelSCNN1B mutation (c.1849C#x3e;A) in a family affected by LS. Patients with LS, especially those with severe hypertension, should be alert for the occurrence of premature stroke. Timely diagnosis using genetic testing and tailored treatment with amiloride can help LS patients to avoid severe complications.Kidney Blood Press Res

Kidney dysfunction is associated with risk of cardiovascular events in middle-aged and elderly population with hypertension: A  5-year community-based cohort study in China
.
CONCLUSION: Moderate to severe kidney dysfunction was an independent risk predictor of CVD events. Among subjects with hypertension or poorly controlled blood pressure level, the presence of CKD significantly increased the risks of CVD events and stroke.

Factors Associated with Chronic Kidney Disease and Their Clinical Utility in Primary Care Clinics in a Multi-Ethnic Southeast Asian Population
Conclusions: CKD prevalence in our Southeast Asian population is high and under-documented even in high-risk patients. Our findings highlight factors associated with CKD, and the predictive value of hypertension, diabetes, and advancing age as EHR-based screening targets for CKD. Our results also suggest that complementary educational efforts will be needed to increase physician detection and optimize the management of CKD, especially in high risk and marginalized groups across all clinics in Singapore, and possibly in the region.Nephron

Comparison of Associations of Outcomes After Stroke With Estimated GFR Using Chinese Modifications of the MDRD Study and CKD-EPI Creatinine Equations: Results From the China National Stroke Registry
Background: The CKD-EPI (Chronic Kidney Disease Epidemiology Collaboration) creatinine equation estimates glomerular filtration rate (GFR) more precisely than the MDRD (Modification of Diet in Renal Disease) Study equation. The risk implications of this equation have been compared with the MDRD Study equation in different demographic and clinical characteristics. However, whether a Chinese modification of this equation performs better than a Chinese modification of the MDRD Study equation in risk prediction in Chinese patients with stroke is unknown.Study Design: Prospective cohort study, China National Stroke Registry.Set...
