Conclusion Of the estimated 48 000 patients with TMVL or RAO annually in the USA, few are evaluated in the ED, and admission rates are less than for transient ischaemic attack or ischaemic stroke and are decreasing over time.

Ocular motor nerve palsy in patients with diabetes: High-resolution MR imaging of nerve enhancement
CONCLUSION: High-resolution MRI evaluation of diplopia in diabetic patients is used to rule out a diagnosis of acute stroke and contributes to the positive diagnosis of ocular motor nerve impairment, possibly combining the influences of inflammatory and microvascular phenomena. Pole to pole
An otherwise asymptomatic 67-year-old man presented to his ophthalmologist complaining of acute painless “dark area on the right”. Visual acuity was preserved and a single cotton-wool spot was noted in each retina. An inferior right quadrantanopia was evident on automated visual field and computerized tomography of the brain confirmed a left occipital stroke. Acute phase markers were elevated and t emporal artery biopsy was consistent with a diagnosis of giant cell arteritis. Isolated retinal cotton wool spots, even in the absence of systemic signs and symptoms, may be suggestive of giant cell arteritis.

Progressive Central Artery Occlusion, Ophthalmic Artery Occlusion, and Hemispheric Intracranial Thrombosis after COVID-19 mRNA Vaccine Application: Case Report
Thromboembolic events as a result of COVID-19 mRNA vaccination are a rare, though life-threatening complication. In this case report, we describe a 40-year-old female patient who developed central retinal artery and ophthalmic artery occlusion progressing to intracranial thrombosis 3 weeks after vaccination with the Pfizer-BioNTech COVID-19 vaccine. Management of binocular diplopia in the department of ophthalmic emergencies at the Regional University Hospital of Tours
CONCLUSION: Among patients assessed in an ophthalmological emergency department setting, one in ten patients had stroke. It is essential to inform patients of the urgent nature of ophthalmological evaluation in the case of acute binocular diplopia. Urgent neurovascular management is also mandatory and should be based on the clinical description provided by the ophthalmologist.
