Does hospitalist directed care reduce the length of stay for acute ischemic stroke patients and improve adherence to the "Get with the Guidelines" inpatient quality measures? (P5.153)
CONCLUSIONS: There is a significant improvement in adherence to the GWTG-ISPM when an AIS patient is admitted under the care of a hospitalist. Prospective databases are recommended to evaluate if this leads to better long-term outcomes.Disclosure: Dr. Tekle has nothing to disclose. Dr. Hassan has nothing to disclose. Endovascular Treatment of Acute Middle Cerebral Artery Occlusion: Comparison of Percutaneous Transluminal Angioplasty and Mechanical Embolectomy (P4.298)
CONCLUSIONS: Data from this national multicenter registry showed that both PTA and ME represent safe and effective recanalization methods of acute MCAo. Bridging therapy was associated with better clinical outcome in the case of ME+IVT than PTA+IVT combination. Crossed Subcortical Aphasia (P1.048)
CONCLUSIONS: This patient is a rare case in which right-sided subcortical damage results in aphasia. The most comprehensive evidence on language lateralization comes from functional studies of healthy adults that differ in their degree of handedness. It appears that the degree of right-handedness correlates, with the degree of left hemisphere language dominance.Disclosure: Dr. Murray has nothing to disclose. Dr. Kase has nothing to disclose. Are patients with carotid stenosis receiving optimal medical therapy? (P2.265)
Conclusion: Despite the presence of established large vessel atherosclerosis, a sizable proportion of patients reported that they were not receiving statin therapy or aspirin. Intensification of "real world" medical therapy will be important to reduce stroke rates and improve long-term survival.Disclosure: Dr. Koch has nothing to disclose. Impact of stroke misdiagnosis on short-term and discharge use of vascular medications in young adults (P7.125)
CONCLUSIONS: ED stroke misdiagnosis was associated with reduced use of early AT. At discharge, there was no variation in antithrombotic or statin use based on stroke misdiagnosis. Fewer than half of cryptogenic stroke patients received statins. Greater statin use in this population could reduce the rate of major vascular events Study Supported by: None Disclosure: Dr. Chaturvedi has received personal compensation for activities with Genentech, Inc., Abbott Vascular, Boehringer-Ingelheim, WL Gore, and Merck. Factors predictive of hospice care in patients with Ischemic stroke (P1.077)
CONCLUSIONS: In our patient population, factors associated with hospice discharge were female gender, age > 70 years, prior stroke and not receiving thrombolysis and/ or intervention. Severity of deficits (NIHSS) was not a reliable marker. Study Supported by:Disclosure: Dr. Sultan has nothing to disclose. Dr. Lee has nothing to disclose.

Quality Assessment of Vascular Disease Prevention Practices in High Risk Urban Women (P7.139)
CONCLUSIONS: Despite their proven value for secondary prevention, antiplatelet therapy and statins were underutilized in women with prior stroke or TIA. Strategies to intensify vascular disease prevention in high risk urban women and particularly women under 70 are needed. Secondary Stroke Prevention Prescribing in a National Cohort (P1.087)
Conclusions: The majority of participants with incident IS within REGARDS were newly prescribed antithrombotics at discharge but alarmingly few were newly prescribed statins or ACE-I/ARBs. Our findings among incident SSP users differ from previous reports among prevalent users within hospitals participating in a national quality improvement program. More studies of quality of care after IS at unselected hospitals are needed.Disclosure: Dr. Albright has nothing to disclose. Dr. Boehme has nothing to disclose. Dr. Howard has nothing to disclose. Statins and Intracerebral Hemorrhage
To the Editor In an article and accompanying editorial about intracerebral hemorrhage (ICH) and statins, Flint et al and Gonzalez-Castellon and Marshall gave too much credence to the controversy about statins causing ICH. Most of this controversy is probably owing to misinterpretation of the results of the Stroke Prevention by Aggressive Reduction in Cholesterol Levels (SPARCL) trial. Flint et al slightly misquoted my editorial about this topic, suggesting that I attributed the increase in ICH to the high crossover rate in SPARCL. The full quote is below, followed by my comments for clarification.

Statins and Intracerebral Hemorrhage—Reply
In Reply We appreciate the comments Dr Spence made in response to our article and the accompanying editorial. While our group is convinced by the arguments he has made in the quoted editorial piece regarding the SPARCL (Stroke Prevention by Aggressive Reduction in Cholesterol Levels) trial results, it is important to recognize that controversy does persist in the literature when it comes to this issue.

Early statin therapy in patients with acute intracerebral hemorrhage without prior statin use
ConclusionsEarly statin group patients with acute ICH did not have a higher recurrent risk of ICH and might have lower all‐cause mortality during follow‐up. It is concluded that statin therapy might be beneficial for patients with ICH.

Cryptogenic stroke
In about a quarter of ischaemic strokes the cause is undetermined, because the investigation is incomplete or delayed, because there are multiple causes or because the stroke is truly cryptogenic. Cryptogenic stroke can be further classified as non‐embolic or embolic. Embolic stroke of undetermined source can be due to paroxysmal atrial fibrillation, minor emboligenic cardiac conditions, atheroembolism, cancer associated and paradoxical embolism through a patent foramen ovale (PFO) or less often a pulmonary fistula. Currently, risk factor control, statins and antiplatelets are the main therapeutic measures to prevent rec...
