Midlife Orthostatic Hypotension May Increase Risk of Dementia, Stroke
People with orthostatic hypotension (an excessive drop in blood pressure when standing up that can lead to dizziness and fainting) in midlife may be more likely than those without the condition to develop stroke and dementia later in life, according to astudy inNeurology.“The presence of orthostasis, especially in midlife, might identify individuals in whom more careful monitoring or risk factor management might be warranted,” wrote lead author Andreea Rawlings, Ph.D., of the Johns Hopkins Bloomberg School of Public Health and colleagues. These findings come from an analysis of data from 11,709 participants without a hi...

Effect of Inflammation on the Process of Stroke Rehabilitation and Poststroke Depression
Conclusions Stroke comprises ischemic stroke and ICH. The immuno-inflammatory process is involved in neural plasticity following events such as a hemorrhage or ischemic stroke. After ischemia, astrocytes, microglia, and MDMs play important roles during rehabilitation with the modulation of cytokines or chemokines, such as TNF-α and IL-1. Moreover, MiRNAs are also important posttranscriptional regulators in these glial mitochondrial responses to cerebral ischemia. ICH involves processes similar and different to those seen in ischemia, including neuronal injury, astrocytic and microglial/macrophage activation, and n...

Anxiety in Patients With Acute Ischemic Stroke: Risk Factors and Effects on Functional Status
Conclusion: Depressive symptoms are the major correlates of PSA while more severe PSA is associated with poorer ADL and health-related QOL. Acute lesions involving CHWM may correlate with PSA in ischemic stroke patients with mild-to-moderate neurologic deficits, supporting a lesion-location hypothesis in PSA.IntroductionAnxiety is prevalent after stroke and occurs in about one-quarter of stroke survivors (1, 2). Poststroke anxiety (PSA) may have a negative impact on quality of life (QOL) of stroke survivors, affecting their rehabilitation (3). Furthermore, one prospective study found that severe anxiety symptoms were assoc...

Prevalence of pre-stroke depression and its association with post-stroke depression: a systematic review and meta-analysis.
CONCLUSIONS: One in six stroke patients have had pre-stroke depression. Reported rates may be routinely underestimated due to limitations around assessment. Pre-stroke depression significantly increases odds of post-stroke depression.Protocol identifierPROSPERO identifier: CRD42017065544. PMID: 30107864 [PubMed - as supplied by publisher]

Depression, anxiety, insomnia, stress, and the way of coping emotions as risk factors for ischemic stroke and their influence on stroke severity: A case –control study in Lebanon
ConclusionDespite several limitations, the findings of our study suggest that people who are depressed or stressed are more likely to have an ischemic stroke. Consequently, additional research into the causes and effects of depression and perceived stress may provide new directions for preventive strategies that can help reduce the risk of stroke. Since pre-stroke depression and perceived stress were also found to be strongly correlated with stroke severity, future studies should evaluate the association between pre-stroke depression, perceived stress, and stroke severity to gain a deeper understanding of the complex inter...

Systematic Review and Meta-Analysis of Prevalence in Post-Stroke Neurocognitive Disorders in Hospital-Based Studies.
CONCLUSION: More than half of stroke survivors experience post-stroke NCD, corresponding to mild post-stroke NCD in two-thirds of cases and major post-stroke NCD in one-third of cases. Harmonization of stroke assessment and cognitive score thresholds is urgently needed to allow more accurate estimation of post-stroke NCD prevalence, especially mild post-stroke NCD. Association of diabetes with stroke and post-stroke dementia: A population-based cohort study.
DISCUSSION: Diabetes, but not prediabetes, is associated with an increased risk of ischemic stroke and post-stroke dementia. PMID: 32489021 [PubMed - as supplied by publisher]

Risk of First Onset Stroke in SSRI-Exposed Adult Subjects: Survival Analysis and Examination of Age and Time Effects.
CONCLUSIONS: Use of SSRIs independently increases the risk of stroke across age strata. The risk is higher in younger adult subjects, and the stroke is more likely to be ischemic than hemorrhagic. The underlying mechanisms of stroke may be related to cerebral microbleeding or an overcorrection of hemostasis function. PMID: 28994901 [PubMed - as supplied by publisher]

Does stroke laterality predict major depression and cognitive impairment after stroke? Two-year prospective evaluation in the EMMA study
Publication date: Available online 7 May 2019Source: Progress in Neuro-Psychopharmacology and Biological PsychiatryAuthor(s): Alessandra Baccaro, Yuan-Pang Wang, André Russowsky Brunoni, Miriam Candido, Adriana Bastos Conforto, Claudia da Costa Leite, Paulo A. Lotufo, Isabela M. Benseñor, Alessandra C. GoulartAbstractDepression and cognitive impairment are common conditions following stroke. We aimed to evaluate stroke laterality as predictor of post-stroke depression (PSD) and cognitive impairment (PCI) in a stroke cohort. Major depression (Patient Health Questionnaire-9, score ≥ 10) and cognitive impairment (Modi...

Increased Risk of Stroke Linked to Stress at Home, Work
Experiencing multiple stressful events or chronic stress at home or at work over the course of a year raises the risk of several types of stroke, astudy published today inJAMA Network Open has found. However, the study also suggests that having a greater sense of control over the situations and experiences that affect one ’s life—a greater locus of control—may help counter the impact of stress on stroke risk.Catriona Reddin, M.B., of the University of Galway in Ireland and colleagues analyzed data from 26,812 participants in the INTERSTROKE study, an international case-control study of risk factors for stroke. Among ...

Anti-inflammatory treatment and risk for depression after first-time stroke in a cohort of 147 487 Danish patients.
CONCLUSION: Anti-inflammatory treatment is associated with a lower risk for depression shortly after stroke but a higher risk for late depression. This suggests that inflammation contributes differently to the development of depression after stroke depending on the time of onset. PMID: 28632121 [PubMed - as supplied by publisher]

Longitudinal Impact of Depression on Quality of Life in Stroke Patients.
CONCLUSION: PSD in the acute phase of stroke is an independent predictor of QOL in both the acute and chronic phases of stroke. Our findings underscore the importance of evaluating depression in the acute phase of stroke. PMID: 29475223 [PubMed]

Are Depression and Dementia a Common Problem for Stroke Older Adults? A Review of Chosen Epidemiological Studies
AbstractStroke is the second leading cause of death in the world. It is often associated with various long-term physical and neuropsychological consequences. The aim of this article is to present the results of research on the occurrence of dementia and post-stroke depression. Using the keywords: post-stroke dementia, post-stroke depression, post-stroke cognitive impairment, stroke, Polish and foreign bibliographic databases were searched: PubMed, Science Direct, Wiley, Google Scholar. Documents that met the eligibility criteria were selected for inclusion in this review. After the conducted analyses, 7 Polish articles and...

Predictors of early post ischemic stroke apathy and depression: a cross-sectional study
Conclusions: Three risk factors for post-stroke apathy were identified as a history of cerebrovascular disease, low HbA1c and lower MDRS I/P scores. A low MDRS I/P score was also identified as a risk factor for post-stroke depression. These results may be useful to clinicians in recognizing and treating apathy and depression in patients after a stroke.
