To compare intraoperative goal directed fluid therapy by trans-oesophageal Doppler vis- & #224;-vis FloTrac & #8482; in patients undergoing living related renal transplantation-a prospective randomised controlled study
Conclusions: Both TED and FloTrac devices can be used effectively to guide GDFT in RT, However, lesser total fluid was required in the FloTrac group, which may lead to a lesser number of fluid-related postoperative complications.

Aortic Valve Replacement in Young and Middle-Aged Adults: Current and Potential Roles of TAVR.
CONCLUSIONS: TAVR is increasingly used for adults <55 years. Given the uniformly excellent results with SAVR, including both mortality and morbidity - particularly regarding stroke, our data favor SAVR in this population, but a prospective trial is needed. Ongoing efforts to harmonize variables and outcomes definitions between the ACSD and CHSD are valuable. PMID: 32768428 [PubMed - as supplied by publisher]

The Effect of Allograft Transplantation of Sertoli Cell on Expression of NF- кB, Bax Proteins, ...
In this study, a remarkable decrease in neurological deficits, infection, blood-brain barrier permeability, and brain edema was observed in the cell transplant recipient group in comparison with the ischemia group. Probably, a reduction in inflammation (NF-кB factor) an d apoptosis (Bax) following injection of Sertoli cells result in amelioration of ischemic damages induced by MCAO surgery.

Natural Y-shaped radial artery graft bypass for a complex middle cerebral artery aneurysm: A case report
We describe a technical method using revascularization with a natural Y-shaped graft that provides a normal variation for a complex MCA aneurysm. A 65-year-old man with a giant thrombosed MCA aneurysm presented with right hemiparesis and aphasia. The patient had a history of clipping surgery for the ipsilateral side of the MCA aneurysm 25 years before, and a de novo aneurysm developed over the previous 18 years.

An altered posterior question-mark incision is associated with a reduced infection rate of cranioplasty after decompressive hemicraniectomy.
CONCLUSIONS: The primary goal of this retrospective cohort analysis was to identify adjustable risk factors to prevent post-CP complications. In this analysis, a posterior question-mark incision proved beneficial regarding infection and CP failure. The authors believe that these findings are caused by the better vascularized skin flap due to preservation of the superficial temporal artery and partial preservation of the occipital artery. In this trial, the posterior question-mark incision was identified as an easily and costless adaptable technique to reduce CP failure rates. PMID: 32330877 [PubMed - as supplied by publisher]

Left ventricular assist systems and strokes: Statins to the rescue?
The development of durable left ventricular assist systems (LVASs) has been critical in providing an option for management of end-stage heart disease and addressing the increasing gap between demand and supply of donor cardiac allografts for management of end-stage heart disease. The technology also provides an alternative to heart transplantation in many patients. Unfortunately, the long-term use of LVASs has been associated with significant morbidities, the most devastating of which may relate to the thrombogenicity of the artificial devices, leading to ischemic stroke or pump thrombosis.

Allograft inflammatory factor-1, a multi-target regulator of atherosclerosis
Atherosclerosis is a chronic inflammatory disease of the arterial wall responsible for ischemic heart disease and stroke, the most frequent causes of death worldwide. Cardiovascular diseases are expected to remain the main cause of death globally within the next 15 years, owing to a rapidly increasing prevalence, mainly due to the rising incidence of obesity and diabetes on a global scale in both developed and developing economies. This forces us to consider new strategies for prediction, prevention, and treatment of cardiovascular disease [1].

Stem cell transplantation for ischemic stroke.
CONCLUSIONS: Overall, in participants with ischemic stroke, stem cell transplantation was associated with a reduced neurological impairment, but not with a better functional outcome. No obvious safety concerns were raised. However, these conclusions came mostly from small RCTs with high risk of bias, and the certainty of the evidence ranged from low to very low. More well-designed trials are needed. PMID: 31055832 [PubMed - as supplied by publisher]

Connecting Metainflammation and Neuroinflammation Through the PTN-MK-RPTP β/ζ Axis: Relevance in Therapeutic Development
Conclusion The expression of the components of the PTN-MK-RPTPβ/ζ axis in immune cells and in inflammatory diseases suggests important roles for this axis in inflammation. Pleiotrophin has been recently identified as a limiting factor of metainflammation, a chronic pathological state that contributes to neuroinflammation and neurodegeneration. Pleiotrophin also seems to potentiate acute neuroinflammation independently of the inflammatory stimulus while MK seems to play different -even opposite- roles in acute neuroinflammation depending on the stimulus. Which are the functions of MK and PTN in chronic neuroi...

Complications of Solid Organ Transplantation
Despite improvements in overall graft function and patient survival rates after solid organ transplantation, complications can lead to significant morbidity and mortality. Cardiovascular complications include heart failure, arrhythmias leading to sudden death, hypertension, left ventricular hypertrophy, and allograft vasculopathy in heart transplantation. Neurologic complications include stroke, posterior reversible encephalopathy syndrome, infections, neuromuscular disease, seizure disorders, and neoplastic disease. Acute kidney injury occurs from immunosuppression with calcineurin inhibitors or as a result of graft failu...

Surgical outcomes in Behcet’s disease patients with severe aortic regurgitation
ConclusionsThe findings suggest that mechanical root replacement combined with a low post-operative C-reactive protein level maintained through adjunctive immunomodulation therapy may lead to optimal surgical outcomes in Behcet’s disease associated with severe aortic regurgitation.
