Acute delirium is commonly underdiagnosed, and can be masked by chronic alterations in cognition and mentation. Delirium has many causes, and can be assessed using the acronym DELIRIUM. The most common presentations suggesting delirium over dementia are short-term memory loss, rapid fluctuation in condition, acute alteration, and a condition present that may be responsible for delirium.

Dental status is Associated with Incident Functional Disability in Community-Dwelling Older Japanese: A Prospective Cohort Study Using Propensity Score Matching.
CONCLUSIONS: In this prospective cohort study targeting community-dwelling older adults in Japan, less than 20 teeth was confirmed to be an independent risk factor for functional disability even after conducting propensity score matching. This study supports previous publications showing that oral health is associated with functional disability.

Conclusion: This study suggests that a clinically efficacious cardiovascular Chinese herbal medicine (DLP) can be successfully repurposed to treat a lung disease in pulmonary fibrosis guided by TCM theory. A prospective survey of the persistence of warfarin or NOAC in nonvalvular atrial fibrillation: a COmparison study of Drugs for symptom control and complication prEvention of Atrial Fibrillation (CODE-AF).
Conclusions: Nonpersistence was significantly lower with NOAC than VKA in both the maintenance and new-starter groups. In only the new-starter group, apixaban or edoxaban showed higher persistence rates than rivaroxaban.

Since the Women's Health Initiative (WHI) has been stopped 2002 due to increased risk of venous thromboembolism, myocardial infarction, stroke and breast cancer, safety issues have got great concern using hormone replacement therapy (HRT). However, using lower dosages, other hormonal components and/or transdermal instead of oral application can reduce those risks although not demonstrated in a placebo-controlled study comparable to WHI. However, this has been shown in large case/control- and cohort-studies reflecting more practical conditions in contrast to WHI.

Use of DOACs in real-world challenging settings: a Delphi Consensus from Italian cardiologists.
CONCLUSIONS: DOACs should represent the first-line anticoagulation therapy in non-valvular AF patients in the majority of challenging settings, underexplored by literature. Caution in their prescription is needed in case of severe renal impairment. Dose choice should follow the SmPC, although this is matter of debate in borderline patients.
