Stroke Severity and Outcomes in Patients With Newly Diagnosed Atrial Fibrillation
Conclusion: Stroke severity and in-hospital outcomes in patients with newly diagnosed AF did not differ from those in patients with known AF after adjustment for clinically relevant factors. The importance of detection of latent AF and subsequent anticoagulation in preventing severe stroke should be further emphasized.

Left Atrial Dilatation and Reduced Left Ventricular Ejection Fraction Are Associated With Cardioembolic Stroke
Conclusions: LA dilatation along with reduced EF is a reliable predictor of Afib and cardioembolic stroke. Further studies are warranted to determine the benefit of anticoagulation for secondary stroke prevention in such patient population.

Cardiac Risk Factors for Stroke: A Comprehensive Mendelian Randomization Study
CONCLUSIONS: Available genetic data do not support substantial effects of cardiac traits on the risk of stroke beyond known clinical risk factors. Our findings highlight the need to carefully control for confounding and other potential biases in studies examining candidate cardiac risk factors for stroke.PMID:34911345 | DOI:10.1161/STROKEAHA.121.036306

Risks and Benefits of Anticoagulation in Atrial Fibrillation: Insights From the Outcomes Registry for Better Informed Treatment of Atrial Fibrillation (ORBIT-AF) Registry Original Articles
Conclusions— In community-based outpatients with AF, use of OAC was high and driven by not only predominantly stroke but also bleeding risk. Stroke risk significantly affects OAC use among those with low bleeding risk, whereas those with high bleeding risk demonstrate consistently lower use of OAC regardless of stroke risk.

Clinical Outcomes after Intravenous Fibrinolysis in Cryptogenic Strokes with or without Patent Foramen Ovale
Conclusions: Paradoxical strokes had better outcomes after IV fibrinolysis than other ischemic stroke subtypes, but this difference is attributable to younger age and milder stroke severity on presentation.

National registry‐based case–control study: comorbidity and stroke in young adults
ConclusionsIn addition to vascular risk factors, comorbidity (represented by ICD‐10 chapters) was associated with increased risk of stroke in young individuals. The risk of stroke was further increased with the number of diagnosed ICD‐10 chapters.

Clinical characteristics, precipitating factors, management and outcome of patients with prior stroke hospitalised with heart failure: an observational report from the Middle East
Conclusions This observational study reports high prevalence of prior stroke in patients hospitalised with HF. Internists rather than cardiologists were the predominant caregivers in this high-risk group. Patients with stroke had higher risk of in-hospital recurrent strokes and long-term mortality rates. Trial registration number NCT01467973.

Stroke After Left Ventricular Assist Device Implantation: Outcomes in the Continuous-Flow Era.
CONCLUSIONS: Stroke while on CF-LVAD support was associated with significant mortality. To reduce the risk of stroke, it is essential to further elucidate risk factors, to optimize anticoagulation, and to further understand the impact of LVAD-related infections. Study: Manual thrombectomy increases stroke risk
Routine percutaneous coronary interventions with manual thrombectomies increase the risk of stroke compared to PCI alone, according to a new substudy published last month. The substudy, published online already and set to go to print in the European Heart Journal, examined a 10,058-patient cohort from the earlier Total study of PCI and manual thrombectomies to better understand the increase in stroke rate. The new analysis reported that the stroke difference was evident in as little as 48 hours, with 15 thrombectomy patients reporting an event versus 5 treated with only PCI. The overall risk was still low however, with a 0...

Incidence and predictors of ischemic stroke during hospitalization for congestive heart failure
Abstract Heart failure (HF) increases the risk of ischemic stroke. Data regarding the incidence and predictors of ischemic stroke during hospitalization for HF are limited. The study population of this retrospective cohort study consisted of patients with congestive HF, consecutively admitted to our center from October 2010 to April 2014. We excluded patients complicated with acute myocardial infarction, infective endocarditis, and takotsubo cardiomyopathy. We also excluded those with dialysis or mechanical circulatory support. We investigated the incidence of ischemic stroke during hospitalization for HF. Thereaf...

Stroke in Adults With Congenital Heart Disease: Incidence, Cumulative Risk and Predictors.
CONCLUSIONS: -Among ACHD-patients, one in eleven men and one in fifteen women suffered a stroke between age 18 and 64. Stroke-incidence was considerably higher than in the general population, especially at younger age. The most important predictors of ischemic stroke were heart failure, diabetes and recent myocardial infarction. Further research is required to see if advances in the management of ACHD may reduce this substantial stroke rate. PMID: 26597113 [PubMed - as supplied by publisher]
