The clinical management and rehabilitation of post stroke aphasia in Italy: evidences from the literature and clinical experience
AbstractAphasia is one of the most devastating symptoms in stroke survivors and severely affects patients ’ communication, quality of life, and social interactions. Several factors are critical to the prediction of aphasia recovery, including acute stroke management and subsequent language rehabilitation. A comprehensive assessment of language skills with appropriate instruments in different phases of post stroke months and years is needed in patients, in order to monitor their language improvement and to guide speech therapies over time. Beyond spontaneous recovery, the effects of speech and language therapy in terms of...

Small vessel disease and clinical outcomes after endovascular treatment in acute ischemic stroke
ConclusionsPre-existing SVD burden, mainly driven by brain atrophy, negatively affects early and late clinical outcomes in anterior circulation ischemic stroke treated with endovascular therapy. Our results may help prognostic stratification of stroke patients treated with endovascular therapy.

Cortical and spinal excitability changes after repetitive transcranial magnetic stimulation combined to physiotherapy in stroke spastic patients
ConclusionOne-hertz rTMS associated with PT increased the unaffected hemisphere excitability, decreased spinal excitability, and reduced post-stroke ULS.

The global burden of cerebral small vessel disease related to neurological deficit severity and clinical outcomes of acute ischemic stroke after IV rt-PA treatment
ConclusionsThe global burden of CSVD was independently associated with neurological deficit severity and clinical outcomes of AIS after IV rt-PA treatment. The total CVSD score is a reliable predictor for poor outcomes of AIS after IV rt-PA treatment.

Stroke during sleep and obstructive sleep apnea: there is a link
ConclusionOSA is an important risk factor for ischemic stroke during sleep. Health care providers must be vigilant in inquiring about symptoms suggestive of OSA in every ischemic stroke patient, especially the patient whose stroke occurred during sleep.

Comprehensive risk factor evaluation of postoperative delirium following major surgery: clinical data warehouse analysis
ConclusionsBig data analytics could be applied to evaluate RFs in electronic medical records. We identified common RFs of POD, regardless of operation type. Big data analytics may be helpful for the comprehensive understanding of POD RFs, which can help physicians develop a general plan to prevent POD.

Dementia: new vistas and opportunities
AbstractOver the past four decades, Alzheimer disease has become near synonymous with dementia and the amyloid/tau hypothesis as its dominant explanation. However, this monorail approach to etiology has failed to yield a single disease-modifying drug. Part of the explanation stems from the fact that most dementias in the elderly result from interactive Alzheimer and cerebrovascular pathologies. Stroke and dementia share the same risk factors and their control is associated with a decrease in stroke and some dementias. Additionally, intensive control of risk factors and enhancement of protective factors improve cognition. M...

GEN-O-MA project: an Italian network studying clinical course and pathogenic pathways of moyamoya disease —study protocol and preliminary results
ConclusionAn accurate and detailed clinical and neuroimaging classification represents the best strategy to provide the characterization of the disease phenotype and clinical course. The collection of a large number of biological samples will permit the identification of biological markers and genetic factors associated with the disease susceptibility in Italy.
