Diffuse alveolar hemorrhage following intravenous thrombolytic treatment in acute ischemic stroke: a case series
AbstractDiffuse alveolar hemorrhage (DAH) following fibrinolytic therapy is a rare but a life-threatening complication. In acute ischemic stroke (AIS), DAH as a complication was not reported following intravenous thrombolytic therapy (ITT). The demographic, clinical, radiologic, and prognostic data of 4 patients with AIS who developed DAH after ITT are presented in this case series, along with a literature review.

Statin, cholesterol, and sICH after acute ischemic stroke: systematic review and meta-analysis
ConclusionPrior/new statin use and lower cholesterol level are not risk factors for sICH and overall ICH in AIS patients, whether or not the patient has received recanalization therapy. New statin use is likely associated with decreased sICH.

Prognostic factors for long-term improvement from stroke-related aphasia with adequate linguistic rehabilitation
AbstractIn the past decade, several studies have reported potential prognostic factors for aphasia after stroke. However, these reports covered no more than 1  year after stroke onset, even though patients often continue to improve over longer periods. The present study included 121 patients with aphasia who received cognitive-based linguistic rehabilitation for at least 2 years post-onset. All were right-handed and had a lesion only in the left hemisph ere. Aphasia outcome was predicted using multiple linear regression analysis. Age at onset, lesion in the left superior temporal gyrus including Wernicke’s area, and ba...

Planning a campaign to fight stroke: an educational pilot project in La Spezia, Italy
ConclusionsOur results suggest that a personalised education can improve knowledge on stroke symptoms and signs, independently of gender and personal risk factors. The results should be verified in larger and less selection population.

Temporal dynamics of peripheral neutrophil and lymphocytes following acute ischemic stroke
ConclusionsThe results of this study improve the understanding of the immune response in AIS. Low neutrophil count relative to high lymphocyte count at 48 –72 h post-AIS should be considered a predictor of a favorable stroke outcome. Conversely, high neutrophil count relative to low lymphocyte count at 48–72 h post-AIS should be considered a predictor of a poor stroke outcome.

Hypertension, seizures, and epilepsy: a review on pathophysiology and management
ConclusionsCurrent evidence from preclinical and clinical studies supports the vision that hypertension may be a cause of seizures and epilepsy through direct or indirect mechanisms. In both post-stroke epilepsy and small vessel disease-associated epilepsy, chronic antiepileptic treatment is recommended. In posterior reversible encephalopathy syndrome blood pressure must be rapidly lowered and prompt antiepileptic treatment should be initiated.

Gender and thrombolysis therapy in acute ischemic stroke patients with incidence of obesity
ConclusionElderly obese male stroke patients with elevated diastolic blood pressure, history of peripheral vascular disease, and obese female stroke patients with a history of coronary artery disease were more likely to be excluded from rtPA.

The predictive dysphagia score (PreDyScore) in the short- and medium-term post-stroke: a putative tool in PEG indication
ConclusionBMI<  20 and mRS ≥ 3 are easily measurable bedside predictive factors of persistent dysphagia. PreDyScore showed good sensitivity and very good specificity and enables the prediction of persistent dysphagia with great accuracy in any clinical setting.

Rivaroxaban in cervical and “cervico-cerebral” artery dissections: a new therapeutic option?
We describe four cases of CADs and, firstly in literature, cervico-cerebral (CCADs) in young patients (average age of 42  years) treated with rivaroxaban 20 mg daily. Three of these four dissections had affected the vertebral artery (condition with an unfavorable prognosis and more often complicated by subarachnoid hemorrhages), and the other one was a carotid dissection at the extra-intracranial passage. All patien ts were followed clinically and with serial neurosonological examinations at 1, 3, and 6 months and with magnetic resonance angiography (MRA) at 6 months. All patients presented a good outcome with vascular...

One-year single-center experience with the Aperio thrombectomy device in  large vessel occlusion in the anterior circulation: safety, efficacy, and clinical outcome
ConclusionThe Aperio stent retriever mechanical thrombectomy device demonstrated high rates of successful reperfusion and a good safety profile in patients with acute ischemic stroke due to LVO in the anterior circulation.

Convexal subarachnoid hemorrhage and acute ischemic stroke: a border zone matter?
ConclusionsWe proposed the embolism or hemodynamic changes of the border zone arterioles as a unifying pathogenetic hypothesis of coexisting c-SAH and AIS.

Adherence to recommendations and clinical outcomes of patients hospitalized for stroke: the role of the admission ward —a real-life investigation from Italy
ConclusionsFor patients with acute ischemic and hemorrhagic stroke, admission to neuro vs. general wards is associated with better out-of-hospital healthcare and long-term adverse outcomes.
