Antithrombotic Therapy for Atrial Fibrillation and Coronary Disease de-Mystified
Publication date: Available online 20 August 2018Source: Canadian Journal of CardiologyAuthor(s): Jason G. Andrade, Marc W. Deyell, Graham C. Wong, Laurent MacleABSTRACTAtrial fibrillation (AF) is a progressive chronic disease characterized by exacerbations and periods of remission. It is estimated that up to 20-30% of those with AF also have coronary artery disease (CAD), and 5-15% will require percutaneous coronary intervention (PCI). In patients with concomitant AF and CAD, management remains challenging and requires a careful and balanced assessment of the risk of bleeding against the anticipated impact on ischemic out...

The importance and future of population screening for atrial fibrillation
Publication date: Available online 17 August 2018Source: Canadian Journal of CardiologyAuthor(s): Seung Yong Shin, Gregory Y.H. LipAbstractAtrial fibrillation (AF) is a common and progressive heart rhythm disorder that causes structural, functional and electrical remodelling of the heart. Although we do not fully understand AF yet, this arrhythmia is one clinical feature of a syndrome that is represented by irregularly irregular atrial rhythm accompanied by progressive atrial structural and functional remodelling. Although ischemic stroke, most feared complication of AF, can be prevented by anticoagulation, asymptomatic or...

Determining the Optimal Systolic Blood Pressure for Hypertensive Patients: A Network Meta-analysis
ConclusionsSBP <130 mmHg is associated with a lower risk of stroke and major adverse cardiovascular events. Further lowering to <120 mmHg can be considered to reduce stroke risk if the therapy is tolerated. Long-term SBP should not exceed 150 mmHg because of increased risk of myocardial infarction and cardiac deaths.

Variability in Non–Vitamin K Antagonist Oral Anticoagulants Dose Adjustment in Atrial Fibrillation Patients With Renal Dysfunction: The Influence of Renal Function Estimation Formulae
ConclusionsMDRD and CKD-EPI eGFR fail to correctly identify a significant proportion of patients who require NOAC dose adjustment, limiting their clinical utility. Cockcroft-Gault eCrCl should be calculated for all patients in whom a NOAC is being prescribed.RésuméContexteL’administration d’un nouvel anticoagulant oral non-antivitamine K (NACO) nécessite un ajustement de la dose en fonction des paramètres rénaux. La mesure de la fonction rénale la plus communément utilisée est le taux de filtration glomérulaire estimé (TFGe) selon la formule « Modified Diet in Renal Disease » (TFGe MDRD) ou « Chronic Kidne...

New Insights into the Use of Cardiac Magnetic Resonance Imaging to Guide Decision-Making in AF Management
Publication date: Available online 12 July 2018Source: Canadian Journal of CardiologyAuthor(s): Tarek Zghaib, Saman NazarianAbstractRecent advances in cardiac magnetic resonance (CMR) imaging acquisition techniques have enabled the visualization of thin atrial myocardium with high temporal and spatial resolution and have the potential to shift atrial fibrillation (AF) management paradigms. Late gadolinium-enhancement (LGE) CMR can non-invasively identify atrial arrhythmogenic scar substrates and has been shown to spatially correlate with low-voltage areas. Immediately post-ablation, a combination of native T1-weighted, LGE...

Integrating Individuals with Stroke into Cardiac Rehabilitation Following Traditional Stroke Rehabilitation: Promoting a Continuum of Care
Publication date: Available online 11 July 2018Source: Canadian Journal of CardiologyAuthor(s): Susan Marzolini, R. KinUnstructured AbstractIndividuals following stroke are at a high risk for a repeat stroke and for complications related to coronary artery disease. Indeed, stroke and coronary artery disease share many of the same risk factors. Unfortunately, patients become sedentary after stroke leading to cardiorespiratory deconditioning as well as muscle atrophy and weakness that leads to deterioration in metabolic, cardiorespiratory, and functional health. Access to intensive secondary prevention programs with a struct...
