Thrombolysis Guided by Perfusion Imaging up to 9 Hours after Onset of Stroke

Can imaging extend the thrombolytic time window after stroke?
Nature Reviews Neurology, Published online: 24 June 2019; doi:10.1038/s41582-019-0232-yA new trial has demonstrated the benefits of intravenous thrombolysis, guided by perfusion imaging, 4.5–9.0 h after stroke onset and in individuals with wake-up stroke. In addition to extending the time window for thrombolysis after acute ischaemic stroke, these findings could aid the refinement of imaging and thrombolytic protocols.

Conclusions: Up to date, no data about PCIS and IVT are available from RTCs. Based on limited results from retrospective clinical studies and case series, IVT is safer for use in PCIS than in ACIS. Conclusion Our data suggest that women who suffer from IS present with a poorer functional outcome than men at 3-months, regardless of other preclinical and clinical factors during the acute phase. Current Perspectives in Facilitated Angioplasty
Conclusions: With the current increased burden of acute coronary syndromes and the lack of immediate primary PCI facilities for all patients with STEMI, facilitated angioplasty seems a feasible therapeutic option. Another benefit of facilitated angioplasty may be represented by a major contribution of thrombolytic therapy in re-establishing microvascular myocardial blood flow.

Thrombolytic therapy for pulmonary embolism.
CONCLUSIONS: Low-quality evidence suggests that thrombolytics reduce death following acute pulmonary embolism compared with heparin. The included studies used a variety of thrombolytic drugs. Thrombolytic therapy may be helpful in reducing the recurrence of pulmonary emboli but may cause major and minor haemorrhagic events and stroke.
