Initial Experience with Upfront Arterial and Perfusion Imaging among Ischemic Stroke Patients Presenting within the 4.5-hour Time Window
Conclusions: An upfront CTA/CTP protocol aided stroke team decision-making in nearly half of cases. Implementation of a CTA/CTP protocol was associated with a learning curve of 6 months before door to needle time ≤60 minutes returned to similar rates as the pre-CTA/CTP protocol.

What Are the Classifications of Perinatal Stroke?
Discussion Perinatal stroke occurs in about 1:1000 live births and is a “focal vascular injury from the fetal period to 28 days postnatal age.” Perinatal stroke is the most common cause of hemiparetic cerebral palsy and causes other significant morbidity including cognitive deficits, learning disabilities, motor problems, sensory problems including visual and hearing disorders, epilepsy, and behavioral and psychological problems. Family members are also affected because of the potential anxiety and guilt feelings that having a child with a stroke presents, along with the care that may be needed over the child&#...

Evaluation and Acute Management of Ischemic Stroke in Infants and Children
This article provides an overview of stroke in neonates, infants, and children.RECENT FINDINGSArterial ischemic stroke and cerebral venous sinus thrombosis are increasingly recognized in childhood as important causes of lifelong morbidity and mortality. Diagnosis of arterial ischemic stroke is frequently delayed, as acute neurologic deficits can be challenging to detect in the young child, and stroke is often not considered in the differential diagnosis. Neurologic sequelae following stroke are common, and strategies to minimize stroke size and optimize recovery are being developed. Recurrent arterial ischemic stroke is no...

Device used to close small hole in heart may protect against recurrent stroke
A device used to close a small hole in the heart may benefit certain stroke patients by providing an extra layer of protection for those facing years of ongoing stroke risk, according to the results of a large clinical trial led by UCLA researchers.“It is a major new treatment option for some people,” said Dr. Jeffrey Saver, director of theUCLA Comprehensive Stroke Center and lead author of the study. However, he added, “Using the device is going to have to be a considered clinical decision between the doctor and the patient about who’s the right person to get it.”Thefindings appear in the Sept. 14 New England Jo...

Expression of Cytokines and Chemokines as Predictors of Stroke Outcomes in Acute Ischemic Stroke
Conclusions: Machine learning algorithms can be employed to develop prognostic predictive biomarkers for stroke outcomes in ischemic stroke patients, particularly in regard to identifying acute gene expression changes that occur during stroke.

Coagulopathy signature precedes and predicts severity of end-organ heat stroke pathology in a mouse model.
CONCLUSIONS: Heat stroke-induced coagulopathy may be a driving mechanistic force in heat stroke pathology, especially when exacerbated by prior infection. Coagulation markers may serve as accessible biomarkers for heat stroke severity and therapeutic strategies. PMID: 32367690 [PubMed - as supplied by publisher]

Prediction-Driven Decision Support for Patients With Mild Stroke: A Model Based on Machine Learning Algorithms
Conclusions: DAMS and R-DAMS, as prediction-driven decision support tools, were designed to aid clinical decision-making for mild stroke patients in emergency contexts. In addition, even within a narrow range of baseline scores, NIHSS on admission is the strongest feature that contributed to the prediction.

Milvexian Granted U.S. FDA Fast Track Designation for All Three Indications Under Evaluation in Phase 3 Librexia Program: Ischemic Stroke, Acute Coronary Syndrome and Atrial Fibrillation
RARITAN, NJ, May 25, 2023 – The Janssen Pharmaceutical Companies of Johnson & Johnson today announced that all three prospective indications for milvexian, an investigational oral factor XIa (FXIa) inhibitor (being developed in collaboration with Bristol Myers Squibb), have now been granted Fast Track Designation by the U.S. Food and Drug Administration (FDA). The designations cover all three indication-seeking studies within the Phase 3 Librexia development program (Librexia STROKE, Librexia ACS and Librexia AF), which are all dosing patients. The Librexia program is unrivaled as the most comprehensive FXIa clinical...

Inherited and Uncommon Causes of Stroke
This article is a practical guide to identifying uncommon causes of stroke and offers guidance for evaluation and management, even when large controlled trials are lacking in these rarer forms of stroke. Recent Findings: Fabry disease causes early-onset stroke, particularly of the vertebrobasilar system; enzyme replacement therapy should be considered in affected patients. Cerebral autosomal dominant arteriopathy with subcortical infarcts and leukoencephalopathy (CADASIL), often misdiagnosed as multiple sclerosis, causes migraines, early-onset lacunar strokes, and dementia. Moyamoya disease can cause either ischemic or he...

Clot-based radiomics model for cardioembolic stroke prediction with CT imaging before recanalization: a multicenter study
ConclusionThe proposed CT-based radiomics model could reliably predict CE stroke in AIS, performing better than the routine radiological method.Key Points• Admission CT imaging could offer valuable information to identify the acute ischemic stroke source by radiomics analysis.• The proposed CT imaging–based radiomics model yielded a higher area under the curve (0.838) than the routine radiological method (0.713; p = 0.007).• Several radiomic features showed significantly stronger correlations with two main thrombus constituents (red blood cells, |rmax|, 0.74; fibrin and platelet, |rmax|, 0.68) than routine radiolog...

Machine Learning and the Conundrum of Stroke Risk Prediction
Stroke is a leading cause of death and permanent disability worldwide.1 Ischaemic stroke is the most common stroke variety, comprising more than 80% of strokes in the US.2 One mechanism of ischaemic stroke is atherosclerosis in the extracranial and intracranial arteries, with plaque rupture leading to thrombosis. The second major category is embolic stroke, in which thrombi form in the heart or the arterial/venous ...

Association between thrombus composition and stroke etiology in the MR CLEAN Registry biobank
ConclusionThrombus composition is significantly associated with stroke etiology, with an increase in RBC and a decrease in F/P raising the odds for a non-cardioembolic cause. No difference between composition of cardioembolic thrombi and of undetermined origin was seen. This emphasizes the need for more extensive monitoring for arrhythmias and/or extended cardiac analysis in case of an undetermined origin.

Combination of hand-crafted and unsupervised learned features for ischemic stroke lesion detection from Magnetic Resonance Images
Publication date: Available online 14 February 2019Source: Biocybernetics and Biomedical EngineeringAuthor(s): G.B. Praveen, Anita Agrawal, Ponraj Sundaram, Sanjay SardesaiAbstractDetection of ischemic stroke lesions plays a vital role in the assessment of stroke treatments such as thrombolytic therapy and embolectomy. Manual detection and quantification of stroke lesions is a time-consuming and cumbersome process. In this paper, we present a novel automatic method to detect acute ischemic stroke lesions from Magnetic Resonance Image (MRI) volumes using textural and unsupervised learned features. The proposed method profic...

Evaluation of sex differences in acid/base and electrolyte concentrations in acute large vessel stroke.
CONCLUSION: Acid/base and electrolyte response to ischemic conditions differ between men and women, and these early changes could be used to predict local acid/base changes and how they develop differently in men and women during ischemia. These findings provide a novel insight into the pathology of large vessel stroke in humans, particularly potential variations based on sex. PMID: 31697942 [PubMed - as supplied by publisher]
