The revascularization of the left subclavian artery with a cross over axilloaxillary bypass for the hybrid repair of thoracic aortic aneurysms
CONCLUSION: The risk of stroke, spinal cord ischemia, and upper extremity ischemia are found higher in the patients who underwent coverage of the left subclavian artery without revascularization. The axilloaxillary bypass grafting may be performed in the patients with high risk to prevent carotid artery manipulation and clamping during carotid-subclavian bypass with long term promising patency rates.PMID:35120094 | DOI:10.24875/CIRU.21000013

Prospective Comparison of Plasma Biomarker and Traditional Risk Factor Profiles for Incident Isolated Atherosclerotic Disease and Incident Isolated Abdominal Aortic Aneurysm
Conclusions: The data supports the view that components of vascular inflammation and cardiovascular stress drives AAA development, whereas glycated cross-links in abdominal aortic wall tissue may have a plausible role in reducing AAA risk in individuals with DM.PMID:35097031 | PMC:PMC8790118 | DOI:10.3389/fcvm.2021.818656

Arterial Atherosclerosis: Vascular Surgery Interventions
Am Fam Physician. 2022 Jan 1;105(1):65-72.ABSTRACTAtherosclerotic vascular disease is a leading cause of death worldwide. Atherosclerotic stenosis of the internal carotid or intracranial arteries causes up to 15% of strokes. Peripheral artery disease affects up to one in five people in the United States who are 60 years and older and nearly one-half of those who are 85 years and older. Renal artery stenosis may affect up to 5% of people with isolated hypertension and up to 40% of people with other atherosclerotic diseases. All patients with atherosclerotic vascular disease should receive a comprehensive program of guidelin...

AIM2 Inflammasome Activation Contributes to Aortic Dissection in a Sporadic Aortic Disease Mouse Model
Aortic aneurysm and dissection (AAD) have a high risk of morbidity and mortality and are particularly lethal when they involve the ascending thoracic aorta; timely diagnosis, careful surveillance, and prompt intervention are necessary to avoid catastrophic outcomes.1 Ascending thoracic aortic aneurysm and dissection (ATAAD) are associated with dire complications, including rupture, aortic valve regurgitation, myocardial infarction, and stroke.2 Even with modern surgical approaches, aortic dissection has a high risk of in-hospital death.

Dysfunctional Vascular Endothelium as a Driver of Atherosclerosis: Emerging Insights Into Pathogenesis and Treatment
Atherosclerosis, the chronic accumulation of cholesterol-rich plaque within arteries, is associated with a broad spectrum of cardiovascular diseases including myocardial infarction, aortic aneurysm, peripheral vascular disease, and stroke. Atherosclerotic cardiovascular disease remains a leading cause of mortality in high-income countries and recent years have witnessed a notable increase in prevalence within low- and middle-income regions of the world. Considering this prominent and evolving global burden, there is a need to identify the cellular mechanisms that underlie the pathogenesis of atherosclerosis to discover nov...

Additive Effects of Genetic Interleukin-6 Signaling Downregulation and Low-Density Lipoprotein Cholesterol Lowering on Cardiovascular Disease: A 2 ×2 Factorial Mendelian Randomization Analysis
Conclusions Genetically downregulated IL-6 signaling and genetically lowered LDL-C are associated with additively lower lifetime risk of cardiovascular disease. Future trials should explore combined IL-6 inhibition and LDL-C lowering treatments for cardiovascular prevention.PMID:34927447 | DOI:10.1161/JAHA.121.023277

Commentary
I have often said to interested trainees and students that vascular surgery is about women's health. Evidence suggests that cardiovascular disease affects women differently —the symptoms of myocardial infarction are often “atypical” in women. The number of hospital discharges for heart failure and stroke are higher in women than in men.1 When detected in women, abdominal aortic aneurysms are often anatomically unsuitable for commercially available endovascular de vices, and mortality is higher from either open or endovascular surgery.

Effectiveness of Proximal Landing Zones 0, 1, and 2 Hybrid Thoracic Endovascular Aortic Repair: A Single Centre 12 Year Experience
CONCLUSION: Satisfactory early and long term results of hybrid arch repair at zones 0, 1, and 2 were achieved. To avoid complications and aortic events, the treatment strategy of hybrid arch repair for aortic arch pathologies should be tailored using accurate pre-operative assessment of the ascending aorta and the aortic arch.PMID:34916108 | DOI:10.1016/j.ejvs.2021.10.043

Abdominal aortic aneurysm ultrasound screening in men with risk factors in Primary Care
CONCLUSIONS: The prevalence of aneurysms in our sample was 2.6%, which was lower than expected. The wide use of ultrasound and its progressive generalisation in the Primary Care setting should lead to a decrease in the number of undiagnosed AAA.PMID:34920344 | DOI:10.1016/j.aprim.2021.102234
