Assessment of Hypertension, Guideline-Directed Counseling, and Outcomes in the ACHD Population
AbstractGuidelines for the diagnosis and treatment of hypertension were published by the American Heart Association (AHA) in 2017. The prevalence of hypertension in adults with congenital heart disease (ACHD) under these guidelines has yet to be characterized. We sought to assess the prevalence, impact, and provider response to hypertension under current guidelines. Data were obtained retrospectively from records of routine clinic visits over a 10  year period. Potential hypertension-related adverse outcomes including stroke, myocardial infarction, surgical intervention for aortic aneurysm, aortic dissection, atrial fibri...

Extent of mural thrombus is not associated with increased 5-year mortality following elective AAA repair
CONCLUSIONS: In our experience, the extent of mural thrombus in AAA does not influence long-term survival after elective repair. AAA repair may provide protection against circulating components of mural thrombus which have the potential to promote atherosclerotic-related adverse events. Patients with renal insufficiency and larger AAA have increased risk of mortality 5 years after elective repair.PMID:35331063 | DOI:10.1177/17085381211063282

Protocol for GUos renovisceral Artery reconstruction-1: a prospective, multicentre, single-arm clinical trial to evaluate the safety and efficacy of a multibRANched sTEnt graft systEm for thoracoabdominal aortic aneurysm (GUARANTEE study)
Introduction The multibranched off-the-shelf stent graft is a promising treatment option for thoracoabdominal aortic aneurysm (TAAA). A commercially available, multibranched, off-the-shelf endograft called the t-Branch stent graft has demonstrated favourable midterm outcomes. Another two investigational off-the-shelf endografts, the GORE EXCLUDER Thoracoabdominal Branch Endoprosthesis and E-nside multibranch stent graft system, are still being developed. However, these three endografts have an unsatisfactory anatomic feasibility rate in patients with TAAA. Based on the concept of Guo’s renovisceral artery reconstruct...

Athero-occlusive Disease Appears to be Associated with Slower Abdominal Aortic Aneurysm Growth: An Exploratory Analysis of the TEDY Trial
CONCLUSION: In an exploratory analysis of a selective cohort from the TEDY trial, AOD was associated with slower AAA growth. Validation of these findings in other cohorts is needed.PMID:35277322 | DOI:10.1016/j.ejvs.2021.12.038

Safety of Transcatheter Aortic Valve Replacement in Patients with Aortic Aneurysm: A Propensity-Matched Analysis
ConclusionsData from a large nationwide database demonstrated that patients with AA undergoing TAVR are associated with similar in-hospital outcomes compared with patients without AA.

Functional noncoding SNPs in human endothelial cells fine-map vascular trait associations RESEARCH
Functional consequences of genetic variation in the noncoding human genome are difficult to ascertain despite demonstrated associations to common, complex disease traits. To elucidate properties of functional noncoding SNPs with effects in human endothelial cells (ECs), we utilized our previous molecular quantitative trait locus (molQTL) analysis for transcription factor binding, chromatin accessibility, and H3K27 acetylation to nominate a set of likely functional noncoding SNPs. Together with information from genome-wide association studies (GWASs) for vascular disease traits, we tested the ability of 34,344 variants to p...

A systematic review and meta-analysis of stroke rates in patients undergoing Thoracic Endovascular Aortic Repair for descending thoracic aortic aneurysm and type B dissection.
We performed a systematic review and meta-analysis aiming to assess the stroke rates following thoracic endovascular aortic repair (TEVAR) for descending thoracic aortic aneurysms and/or dissections.

Triple bypass for multisystem smooth muscle dysfunction syndrome due to Arg179His ACTA2 mutation
Missense mutations in the smooth muscle-specific alpha-actin isoform (ACTA2) gene cause multisystem smooth muscle dysfunction syndrome (MSMDS) (OMIM #613834),1,2 which is associated with congenital mydriasis and arterial diseases, including patent ductus arteriosus, premature coronary artery stenosis, and premature thoracic aortic aneurysm.1 Some other characteristic findings, such as flaccid bladder and gastrointestinal problems are known, but not all of them are present.3 ACTA2 cerebral arteriopathy (ACTA2CA), which may be caused by MSMDS, is characterized by abnormal linear blood vessel morphology that may lead to ischemic stroke.

Chimney endovascular aneurysm repair (ChEVAR) for hostile neck complex aneurysm
CONCLUSION: Reintervention to multiple chimney grafts and for persistent gutter-flow is higher compared to single chimneys and demands close surveillance. However, based upon this combined transantlantic experience, we believe multiple ChEVAR provides a reasonable and safe option for complex aortic aneurysm repair when open or custom endografts are not available or indicated based on their Instructions For use, even when triple chimney grafts are required. The optimal configuration for multiple ChEVAR still warrants further study, although theoretical preliminary advantages may exist for a combination of antegrade and retr...
