In the Trenches: Week One of ICD-10
Welcome back, blog fans. Sorry for the prolonged radio silence, but as some of you may have heard (or not, if you don’t happen to work in the medical field) this past Thursday, October 1, 2015 marked the official switch-over to ICD-10, and to say things have been a little hectic is like saying Congress is a little contentious. What am I talking about, you say? Diagnosis coding. At the end of my residency, the program sent the third year residents to a two-day seminar on practice management. Knowing I was about to go out on my own, I paid close attention, and ending up being able to use a great deal of what I learne...

5 Year Old New Jersey Girl Died from the MMR Vaccine, Holly’s Law Created
Conclusion We leave you with words from Robin: “My family and I hope that Holly’s story will make a difference and help you realize that you must be aware of the risks of vaccinations, just as you make yourself aware of the risks of any medical procedure. We hope to make change, and one very important improvement must be that the pediatricians acknowledge that there are vaccine reactions, that moderate to serious and even fatal vaccine reactions do exist and occur at least 100 times more than is reported to the Vaccine Adverse Event Reporting System (VAERS).  We are still very disappointed and disgusted with the actio...

Safety and immunogenicity of a trivalent recombinant PcpA, PhtD, and PlyD1 pneumococcal protein vaccine in adults, toddlers, and infants: A phase I randomized controlled study.
CONCLUSIONS: The candidate trivalent PPrV was safe and immunogenic in adults, toddlers, and infants. Addition of aluminum adjuvant improved immunogenicity in infants without changing the safety profile. PMID: 26143615 [PubMed - as supplied by publisher]

Mumps disease outbreak in Davangere district of Karnataka, India
Conclusion: We confirmed the cases by serological as well as a sensitive RT-nested PCR-based method and sequencing results for the molecular identification of mumps infection. Sequencing results of the SH gene identified outbreak strain as genotype C, which was consistent with other outbreaks in India.

Immunogenicity and Safety of 13-Valent Pneumococcal Conjugate Vaccine in HIV-Infected Adults Previously Vaccinated With Pneumococcal Polysaccharide Vaccine
Conclusions. Vaccination with PCV13 induces anticapsular immunoglobulin G and opsonophagocytic antibody responses in HIV-infected adults with prior PPSV23 vaccination and CD4 cell counts ≥200 cells/mm3. The observations support the use of PCV13 in this population. Clinical Trials Registration. NCT00963235.

4 Month-Old Infant From Tennessee Passed Away After Given 7 Vaccines
Parents in Memphis, Tennessee, are mourning the loss of their baby girl Ja’Liyah Cortize Turner, after she passed away in her sleep, less than four days after receiving seven vaccines. Her mother Quavia felt pressured into getting her daughter further vaccinated, even though she didn’t want to, because her daughter had a bad reaction from the round of vaccines given to her earlier. Ja’Liyah’s autopsy report did not mention the vaccines even though she still had the knots on her legs at the injection sites. Her cause of death was ruled “Unknown, Undetermined.” The autopsy stated Ja’Liyah had no significant...

Pneumococcal lower respiratory tract infections in adults: an observational case-control study in primary care in Belgium
Conclusions: SP was responsible for approximately 5 % of SLRTIs observed in primary care in Belgium. Pneumococcal infection was associated with a significant increase in morbidity. Sixty-eight percent of serotypes causing SLRTI were potentially preventable by PCV13.

Bacteraemia and antibiotic-resistant pathogens in community acquired pneumonia: risk and prognosis
The sensitivity of blood cultures in the diagnosis of bacteraemia for community-acquired pneumonia is low. Recommendations, by guidelines, to perform blood cultures are discordant. We aimed to determine the incidence, microbial aetiology, risk factors and outcomes of bacteraemic patients with community-acquired pneumonia, including cases with antibiotic-resistant pathogens (ARP). A prospective, observational study was undertaken on consecutive adult patients admitted to the Hospital Clinic of Barcelona (Barcelona, Spain) with community-acquired pneumonia and blood cultures were obtained. Of the 2892 patients included, bact...

Acute Otitis Media in Children—Current Treatment and Prevention
Abstract Acute otitis media (AOM) is the most common bacterial infection in children and has a very varied clinical spectrum, ranging from spontaneous resolutions to serious complications. The effect of antibiotics in AOM depends on the chosen outcome, but has been shown to reduce pain somewhat, and have a greater beneficial effect in severe cases of AOM. Today, not all episodes of AOM are treated with antibiotics, but most countries have issued guidelines that include an option of watchful waiting in many cases. Prevention of AOM reaches from modification of environmental risk factors to vaccinations and surgery...

Nickson Would be Celebrating Life if it Weren’t for Vaccines
Conclusion Parents need to be able to choose what goes into their child’s body, especially when their babies are not born full-term or have any underlying health problems at birth. When a product such as a vaccine is injected into a child, known to be associated with severe risks, including death, there should be a standard protocol in place for these families to get needed support when the risks outweigh the benefits. No one can predict how a vaccine will negatively affect a person. Lindsey and other families going through this, suffering the loss of a child likely caused by the vaccine(s) given to them, when no other p...

Why Target Vaccines?
Anti-vaccination campaigners have reached the forefront of the news. When a public health issue has reached the Jimmy Kimmel show, you know that it has reached the "big time." Seth Mnookin wrote the book The Panic Virus: The True Story of the Vaccine-Autism Controversy. Both Kimmel and Mnookin mentioned Jenny McCarthy, who has been quite vocal about her concerns about vaccines. More measles outbreaks have occurred in the past year than in the past decade, with the majority occurring among unvaccinated children. Buzzfeed asked vaccine expert Kate O'Brien of the International Vaccine Access Center (IVAC) at the Johns Hopkins...

Persistence of specific bactericidal antibodies at 5 years of age after vaccination against serogroup B meningococcus in infancy and at 40 months.
Abstract BACKGROUND: The multicomponent serogroup B meningococcal (4CMenB) vaccine induces antibodies against indicator strains of serogroup B meningococcus under various schedules. We investigated the persistence of antibodies in 5-year-old children 18-20 months after their last dose (at about 3.5 years of age). METHODS: We assessed 5-year-old children who received the 4CMenB vaccine in a previous randomized trial. We also recruited 50 vaccinenaive 5-year-olds and administered 2 doses of 4CMenB to each child. We measured serum bactericidal antibody titres against 4 indicator strains of serogroup B meningococ...

Update on paroxysmal nocturnal haemoglobinuria: on the long way to understand the principles of the disease
Abstract The understanding of the clinical manifestations in paroxysmal nocturnal haemoglobinuria (PNH) has made great progress. The main symptoms of this disease such as abdominal pain, renal failure or pulmonary hypertension and even the basis of the dramatic thrombophilia can be related to intravascular haemolysis and liberation of free haemoglobin resulting in NO depletion. In addition, there has been a recent great progress in elucidating the pathophysiology of clonal expansion within PNH bone marrow. In the majority of patients with haemolytic PNH, there are additional mutations within genes beyond PIG‐A and rather...
