Viruses, Vol. 14, Pages 969: A Review on Chikungunya Virus Epidemiology, Pathogenesis and Current Vaccine Development
Franca Chikungunya virus (CHIKV) is a mosquito-borne alphavirus that recently re-emerged in many parts of the world causing large-scale outbreaks. CHIKV infection presents as a febrile illness known as chikungunya fever (CHIKF). Infection is self-limited and characterized mainly by severe joint pain and myalgia that can last for weeks or months; however, severe disease presentation can also occur in a minor proportion of infections. Among the atypical CHIKV manifestations that have been described, severe arthralgia and neurological complications, such as encephalitis, meningitis, and Guillain–Barr&am...

Fever, Myalgias, and Weakness in a Patient with HIV
A 62-year-old Bolivian man residing in Spain since 2002, diagnosed with human immunodeficiency virus (HIV) infection in 2014 and with poor adherence to antiretroviral therapy (bictegravir/emtricitabine/tenofovir alafenamide), presented to the Emergency Department with 2-month history of asthenia, myalgias, and weakness. He complained of increased weakness and pain in his right arm, in addition to edema and distal paresthesia 10 days after he received the pneumococcal conjugate vaccine (Prevenar 13) in the same arm.

His most recent CD4+ T cell count was 174 cells/ml and he had a viral load of 114 copies/ml.

Safety surveillance of meningococcal group B vaccine (Bexsero ®), Vaccine Adverse Event Reporting System, 2015-2018
CONCLUSIONS: Analysis of passive surveillance data from over 5.6 million doses of MenB-4C distributed in the United States did not reveal new safety concerns. The large majority of reports were classified as non-serious and the reported AEs were generally consistent with the safety experience described in clinical studies and the product's package insert. While our results are reassuring, continued post-marketing surveillance is warranted.PMID:34887130 | DOI:10.1016/j.vaccine.2021.11.071

What Causes Irritability?
Discussion Child abuse or non-accidental trauma is a clinical diagnosis. It is the constellation of history, physical examination, radiographic evaluation, laboratory testing and investigation that determines if child abuse has occurred. Irritability caused by fractures or other trauma can be a presenting symptom of abuse. Fractures can be a presentation but they are also common problems not associated with abuse. Femoral shaft fractures are estimated to account for only 1.6% of all fractures in children, therefore they are potentially more suspicious for child abuse. A review of femur fractures can be found here. A review...
