Optimizing Cefazolin Prophylaxis in Obstetrical Patients with Reported Beta-Lactam Allergy Undergoing Cesarean Delivery
CONCLUSIONS: Implementation of a simple allergy history guided algorithm in obstetrical patients with reported beta-lactam allergy resulted in a sustained increase in peri-operative cefazolin prophylaxis.PMID:37245613 | DOI:10.1016/j.jogc.2023.05.026

Characteristics of immune response profile in patients with immediate allergic and autoimmune urticarial reactions induced by SARS-CoV-2 vaccines
In conclusion, our results revealed that multiple vaccine components, inflammatory cytokines, and autoreactive IgG/IgE antibodies contribute to SARS-COV-2 vaccine-induced immediate allergic and autoimmune urticarial reactions.PMID:37245259 | PMC:PMC10183637 | DOI:10.1016/j.jaut.2023.103054

Debilitating Gentamicin Ototoxicity: Case Report and Recommendations Against Routine Use in Surgical Prophylaxis
We present a case of a woman with preexisting Meniere's Disease who received gentamicin 400 mg perioperatively for a sigmoidectomy due to a penicillin allergy listed in the patient's medical record. The patient developed severe ototoxicity preventing her from working or driving. Physical examination was remarkable for a broad-based gait requiring assistance to walk and bilateral corrective saccades. Vestibular testing revealed high-grade bilateral vestibular loss associated with all semicircular canals, a considerable decline compared to her function 3 years prior.DISCUSSION: Gentamicin is indicated for surgical prophylaxi...

Zymogen Granule Protein 16-like and Carboxypeptidase B, new allergens in Hedgehog hypersensitivity
Pet ownership is becoming more and more common.1 Even most exotic ones such as hedgehog, seem to be getting fashionable. However they are not exempt from causing allergies. Hedgehogs are spiny mammals that belong to the subfamily Erinaceinae, order Erinaceomorpha. They are originally from Europe, Asia and Africa. There are 6 genera and 15 species of hedgehogs. Among these, the one most frequently encountered in captivity is the african pygmy hedgehog. Pygmy Hedgehog has been reported to prompt different symptoms ranging from nasal itching to urticaria or even respiratory problems.

Itch in elderly patients: Origin, diagnostics, management
Pruritus affects persons of all ethnicities during all periods of life. However, in younger people, pruritus is caused mostly by chronic inflammation (eg, atopic dermatitis, urticaria) or a well-defined, often self-limiting skin disorder. In elderly patients (aged ≥60 years) pruritic eruptions occur increasingly on clinically unremarkable skin, thus making them more difficult to diagnose accurately.1,2 Recent scientific discoveries regarding physiologic and molecular changes in the skin of elderly patients that occur during the aging process have presented the first clues as to why pruritic dermatoses occur more commonly...
