Impact of Percutaneous Closure of Interatrial Shunts on Migraine Attacks: Single-Operator Series and Review of the Literature.
CONCLUSION: A high percentage (49%) of PFO/ASD patients in this series were also migraine sufferers. Percutaneous closure offered migraine relief in 83% of patients, 82% in PFO patients and 89% in ASD patients. Thus, based on this experience and on literature review, a strategic approach for device closure is proposed for migraineurs with a PFO or ASD. PMID: 28356032 [PubMed - as supplied by publisher]

Arrhythmias in Patients with Atrial Defects
Publication date: June 2017 Source:Cardiac Electrophysiology Clinics, Volume 9, Issue 2 Author(s): Tahmeed Contractor, Ravi MandapatiTeaser Atrial arrhythmias are common in patients with atrial septal defects. A myriad of factors are responsible for these that include remodeling related to the defect and scar created by the repair or closure. An understanding of potential arrhythmias, along with entrainment and high-density activation mapping can result in accurate diagnosis and successful ablation. Atrial fibrillation is being seen increasingly after patent foramen ovale closure and may be the primary etiology of recurre...

Ebstein's anomaly in adults: Modified cone reconstruction of the tricuspid valve is associated with promising outcomes.
CONCLUSION: The cone repair of the tricuspid valve in adults with Ebstein's anomaly provided excellent mid-term results and significantly improved functional status. This procedure might be considered even in mildly symptomatic patients in the presence of severe valve regurgitation. PMID: 28566198 [PubMed - as supplied by publisher]

Paradoxical Embolism in a Girl with Atrial Septal Defect After Paroxysmal Supraventricular Tachycardia Ablation
Paradoxical embolism is a kind of important pathogenesis of cardiocerebrovascular diseases. In our case, radiofrequency current catheter ablation was accomplished successfully in a 15-year-old girl who had been experiencing supraventricular tachycardia for 6 months. She presented with abrupt onset of left hemiplegia and loss of consciousness on the first postoperative day. An urgent cerebral computed tomography showed a hyperdense right middle cerebral artery. Transthoracic echocardiogram examination revealed an atrial septal defect (ASD), with the diameter measuring 39  mm.

Simplified Percutaneous Closure of Patent Foramen Ovale and Atrial Septal Defect With Use of Plain Fluoroscopy: Single Operator Experience in 110 Consecutive Patients
Conclusion Percutaneous placement of an Amplatzer occluder was safe and effective with use of local anesthesia and fluoroscopy alone. There were no recurrent strokes over >4years. Migraine relief was reported by >80% of patients.

Intravenous enoxaparin anticoagulation in percutaneous left atrial cardiac procedures.
CONCLUSIONS: IV enoxaparin without monitoring appears as a potential safe and easy-to-use anticoagulation regimen in percutaneous LA cardiac interventions. Further investigations with larger cohorts of patients are warranted. PMID: 28804058 [PubMed - as supplied by publisher]

Variation in practice patterns in device closure of atrial septal defects and patent ductus arteriosus: An analysis of data from the IMPACT ® Registry
Conclusion There is measurable variation in transcatheter closure of PDA and ASD. Further research is necessary to study whether this affects outcomes or resource utilization.

Recognizing and reacting to complications of trans-septal puncture.
Authors: Salghetti F, Sieira J, Chierchia GB, Curnis A, de Asmundis C Abstract INTRODUCTION: The transseptal puncture (TSP) enables access to the left heart, through the fossa ovalis (FO), both in electrophysiology and in cardiac interventional procedures. TSP is usually safe in experienced hands. Sometimes TSP can be technically demanding and carries the risk of severe complications in approximately 1%. While performing a TSP, every effort should be taken in order to avoid complications. In the event of complications, prompt recognition and reaction are essential and a combined "heart-team" management may be the m...

Alterations in ventricular pumping in patients with atrial septal defect at rest, during dobutamine stress and after defect closure
ConclusionPumping mechanisms in patients with RV volume overload depend on the aetiology for the RV dilation and not the size of the RV.

Comparison of hemodynamic effects of sevoflurane and ketamine as basal anesthesia by a new and direct monitoring during induction in children with ventricular septal defect: A prospective, randomized research
Background: Sevoflurane and ketamine are commonly used to obtain sedation and facilitate intravenous anesthetic induction in children undergoing cardiac surgery who are uncooperative. We used a new and direct systemic hemodynamic monitoring technique pressure recording analytical method and compared the hemodynamic effects of sevoflurane and ketamine to facilitate intravenous anesthetic induction. Methods: Forty-four children with ventricular septal defect (2.2 ± 1.2 years) were enrolled and randomized to receive sevoflurane (Group S) or intramuscular ketamine (Group K) for sedation, followed by intravenous mid...

Noninvasive quantification of left-to-right shunt by phase contrast magnetic resonance imaging in secundum atrial septal defect: the effects of breath holding and comparison with invasive oximetry
AbstractTo investigate the effect of breath-holding on left-to-right shunts in patients with a secundum atrial septal defect (ASD). Thirty-five consecutive patients with secundum ASDs underwent right heart catheterization and invasive oximetry. Phase-contrast magnetic resonance imaging (MRI) was performed for the main pulmonary artery and ascending aorta. All measurements were obtained during free breathing (FB) (quiet breathing; no breath-hold), expiratory breath-hold (EBH), and inspiratory breath-hold (IBH). Pulmonary circulation flow (Qp) and systemic circulation flow (Qs) were calculated by multiplying the heart rate b...

Intracardiac echocardiography for percutaneous patent foramen ovale and atrial septal defect occlusion.
CONCLUSION: ICE is a safe and high-quality imaging technique for guiding transcatheter ASD and PFO occlusion. Additionally, ICE can both facilitate device implantation and detect cardiac abnormalities that are not identified with TEE during the initial diagnostic investigation. Clinical Use of the Amplatzer Device in the Management of Intracardiac Defects: A Single-Center Experience
Int J Angiol DOI: 10.1055/s-0038-1626718Device therapy is a viable alternative to open heart surgery in the management of intracardiac defects. The Amplatzer septal occluder (ASO) is one such device that has been adapted to close a wide variety of cardiac defects with few complications and a high success rate. This is a retrospective review of all the patients who received the ASO between 2012 and 2016 at the University of Kentucky. In total, 169 patients underwent percutaneous closure of a septal defect with Amplatzer during the timeframe studied, and of those, 91% received the device for an atrial septal defect or patent...
