Cardioembolic Sources in Patients With Small Single Subcortical Infarcts
Transthoracic echocardiogram (TTE) is widely used as part of the work-up for ischemic stroke. However, the added utility of TTE in the management of small single subcortical infarcts (SSSI) has not been extensively evaluated. Therefore, we aimed to determine the frequency of high-risk and medium-risk cardioembolic sources diagnosed by TTE in SSSI patients, and whether the findings altered clinical management. We performed a retrospective analysis of a prospective cohort of patients with confirmed acute ischemic stroke enrolled in a single-center observational registry between August 2012 and July 2014. We assessed infarct ...

Endovascular closure of patent foramen ovale: a critical appraisal of published trials
Conclusion Young patients with cryptogenic strokes seem to benefit from endovascular closure of a PFO in the presence of a large right-to-left shunt or an associated atrial septum aneurysm. For most other patients, a highly-individualized decision must be made, taking into account the low risk of recurrence in patients with a cryptogenic stroke attributable to a PFO, the high numbers needed to treat and the risks related to the procedure.RESUMO O tratamento de pacientes com infarto cerebral criptog ênico e forame oval patente (FOP) é controverso. Uma revisão crítica destes estudos é apresentada. Métodos São descrito...

Left Atrial Enlargement Could Be Detected on Extended Computed Tomography Angiography —Reply
In Reply We agree with Popkirov that left atrial volume is a better marker of incident atrial fibrillation than atrial diameter; however, we did not have these data available at all of the sites participating in the New Approach Rivaroxaban Inhibition of Factor Xa in a Global Trial Versus ASA to Prevent Embolism in Embolic Stroke of Undetermined Source (NAVIGATE-ESUS) trial. Recently completed and ongoing studies will help determine if our observation has a clinical role and may provide additional data on left atrial size and function. Once all of these data are available, guidelines committees will likely make recommendat...

Paradoxical brain embolism in a young man: Is it only a patent foramen ovale?
Abstract Paradoxical embolism is considered the major cause of cerebral ischemic events in young patients. The most common cause of paradoxical embolism, which has been widely described, is right-to-left shunting (RLS) at cardiac level through a patent foramen ovale (PFO). Rarely paradoxical embolism can also be caused by RLS at pulmonary level due to pulmonary arteriovenous fistula (PAVF). Herein, we present a case of a young man, who experienced transient ischemic attack (TIA) due to paradoxical embolism, in whom both abovementioned abnormalities coexisted. This coincidence is very rare (noted in only 1% of pati...

Prevalence of Patent Foramen Ovale in Cryptogenic Strokes of North Indian population (P1.007)
CONCLUSIONS:TCD with bubble contrast is an excellent tool for the diagnosis of RLS in cryptogenic young strokes .North Indian cryptogenic strokes have a high prevalence of PFO as indicated by RLS. This is the first study on prevalence of PFO in Indian population Study Supported by:NoDisclosure: Dr. Petluri has nothing to disclose. Dr. Khurana has nothing to disclose. The RoPE Score and Right-to-Left Shunt Severity by Transcranial Doppler in the CODICIA Study
Conclusions: For patients with CS, severity of RLSh by c-TCD is positively correlated with the RoPE score, indicating that this technique for shunt grading identifies patients more likely to have pathogenic rather than incidental PFOs. c-TCD is also more sensitive in detecting RLSh than TEE. These findings suggest an important role for c-TCD in the evaluation of PFO in the setting of CS.Cerebrovasc Dis 2015;40:52-58

The diagnostic yield of transesophageal echocardiography in patients with cryptogenic cerebral ischaemia: a meta‐analysis
ConclusionRoutine TEE in patients with cryptogenic IS/TIA commonly identifies abnormal findings. However, the prevalence of cardiac conditions considered to be causally associated with cerebral ischaemia (intracardiac thrombi and tumors) is low.

Left Dorsomedial Thalamic Damage Impairs Verbal Recall More Than Recognition: A Case Report
Damage to the dorsomedial thalamus usually leads to impaired episodic memory, attention, and executive function, but the role of the dorsomedial thalamus in memory processing is still not fully understood. Clinical evidence is inconclusive about whether dorsomedial thalamic damage impairs recall or whether it impairs recognition. I report a unique patient who suffered a cardioembolic stroke in the paramedian artery territory, caused by a patent foramen ovale. He was left with a chronic ischemic lesion centered in the parvocellular and, to a lesser extent, the magnocellular portions of the left dorsomedial thalamic nucleus,...

Paradoxical Embolism in the Setting of Inferior Vena Cava Filter Removal (P1.283)
Conclusions:Although clinically relevant events secondary to IVC filter migration and fracture are rare, they can be life-threatening. Care should be taken when choosing to place these devices, and alternative therapies should be considered when possible. They should be removed when no longer needed, as fracture rates are increased with longer dwell times. Up to half of complications can be avoided with retrieval within 3 months.Disclosure: Dr. Yost has nothing to disclose. Dr. Klaas has nothing to disclose.

Neurologic Complications of Cardiac and Aortic Disease
This article discusses neurologic complications that can arise from cardiac and aortic disease and dysfunction. Recent Findings: Advances in the care of patients with cardiac or aortic disease include the use of prolonged cardiac monitoring in cryptogenic stroke and the approval of the use of left atrial appendage closure devices for stroke prevention in patients with atrial fibrillation who are not candidates for anticoagulation. Continuing controversy surrounds patent foramen ovale closure, and new evidence indicates that cognitive impairment following coronary artery bypass grafting surgery may be less common than prev...

Risk factors and complications associated with intra-operative or post-operative identification of a PFO in cardiac surgery patients: A cohort study
ConclusionPatent foramen ovale finding incidence in peri-operative cardiac surgery care was rare (2%) but post-operative finding of PFO was associated with a increased risk of ischemic strokes, worsened respiratory outcomes, and prolonged hospitalization.

Intracranial Hemorrhage as a Marker for Mycotic Aneurysm in Patients With Infective Endocarditis
Discussion Patients with left-sided and right-sided endocarditis with PFO and ICH on CNS imaging were more likely to have mycotic aneurysms detected through DSA than similar patients without ICH. Routine DSA screening for mycotic aneurysms may not be required in patients with endocarditis undergoing evaluation for valve surgery who present without ICH on CNS imaging.
