Preface
The past decade has seen large shifts in our approach to asthma, with the achievement of many advances in our understanding of its underlying mechanisms and the development of new approaches to its diagnosis and treatment. Indeed, much has changed from our new understanding of disease heterogeneity and related comorbid conditions, to the use of variable dosing of inhaled steroids and reliever combination therapies and to the development of targeted biologics. In this special issue of Respiratory Medicine entitled “Innovations in Asthma”, several publications highlight innovations that are at the forefront of asthma res...

Prolonged-release pirfenidone in patients with pulmonary fibrosis as a phenotype of post-acute sequelae of COVID-19 pneumonia. Safety and efficacy
One of the major concerns with post-acute sequelae of COVID-19 (PASC) is the development of pulmonary fibrosis, for which no approved pharmacological treatments exist. Therefore, the primary aim of this open-label study was to evaluate the safety and the potential clinical efficacy of a prolonged-release pirfenidone formulation (PR-PFD) in patients having PASC-pulmonary fibrosis. Novel pneumonia score based on a machine learning model for predicting mortality in pneumonia patients on admission to the intensive care unit
Scores for predicting the long-term mortality of severe pneumonia are lacking. The purpose of this study is to use machine learning methods to develop new pneumonia scores to predict the 1-year mortality and hospital mortality of pneumonia patients on admission to the intensive care unit (ICU). Effect of bronchial thermoplasty on static and dynamic lung compliance and resistance in patients with severe persistent asthma
Bronchial thermoplasty (BT) reduces severity and frequency of bronchoconstriction and symptoms in severe, persistent asthmatics although it is usually not associated with change in spirometric variables. Other than spirometry. there are almost no data on changes in lung mechanics following BT. (Source: Respiratory Medicine)

Home-based pulmonary rehabilitation for adults with severe asthma exposed to psychosocial chronic stressors
To evaluate the effects of a home-based pulmonary rehabilitation (PR) programme on hyperventilation symptoms, anxiety and depressive symptoms, general fatigue, health-related quality of life (HRQoL) and exercise capacity in adults with severe asthma who have been exposed to psychosocial chronic stressors. Severe asthma clinical remission after biologic treatment with anti-IL4/IL13: A real-life experience
Dupilumab, a fully human anti-interleukin-4/interleukin-13 monoclonal antibody, has shown efficacy in many aspects of Type-2 severe asthma management. Currently, we lack real-life studies addressing the achievment of clinical remission in patients treated with this biologic. (Source: Respiratory Medicine)

Angiographic classification of pulmonary vascular lesion in Takayasu arteritis: A cohort study
Percutaneous transluminal pulmonary angioplasty (PTPA) can significantly improve the haemodynamics and prognosis of patients with Takayasu arteritis and pulmonary artery involvement. However, the choice of PTPA is mainly based on clinical experience. We aim to classify pulmonary artery lesions in Takayasu arteritis according to pulmonary angiogram and evaluate the PTPA success rate and technical difficulty of different types of lesions. Underdiagnosis and misclassification of COPD in Sweden – A Nordic Epilung study
The prevalence of COPD tends to level off in populations with decreasing prevalence of smoking but the extent of underdiagnosis in such populations needs further investigation. (Source: Respiratory Medicine)

Comment on: Identifying responders and exploring mechanisms of action of the endobronchial coil treatment for emphysema
I have read the article titled “Identifying responders and exploring the mechanism of action of endobronchial coil treatment for Emphysema” by Jorine E. Hartman et al. [1] I would like to express my appreciation for its informative and well-structured nature. I agree with the conclusion that patients suffering from emphysema can benefit significantly from endobronchial coil treatment, primarily through the reduction of static hyperinflation, which leads to improvements in airway resistance and dynamic lung mechanics. Dyspnea-induced Limitation (DYSLIM), a new self-administered concise questionnaire to evaluate dyspnea-related activity limitation in chronic respiratory diseases
Few questionnaires are available for routine assessment of dyspnea. The study aimed to design a self-administered questionnaire assessing the impact of chronic dyspnea on daily activities, named DYSLIM (Dyspnea-induced Limitation). (Source: Respiratory Medicine)

Efficacy of mometasone/indacaterol/glycopyrronium in patients with inadequately controlled asthma with respect to baseline eosinophil count: Post hoc analysis of IRIDIUM study
Baseline characteristics could potentially guide asthma treatments. We evaluated whether baseline eosinophil levels affect the efficacy of mometasone/indacaterol/glycopyrronium (MF/IND/GLY) in patients with inadequately controlled asthma. (Source: Respiratory Medicine)

Differential impacts between fat mass index and fat-free mass index on patients with COPD
Differences in the clinical impacts of fat mass index (FMI) and fat-free mass index (FFMI) remain unclear in patients with chronic obstructive pulmonary disease (COPD). We hypothesized that FMI and FFMI have different impacts on 1) emphysema and 2) pulmonary function and health-related quality of life of COPD patients. (Source: Respiratory Medicine)

Abnormalities on baseline chest imaging are risk factors for immune checkpoint inhibitor associated pneumonitis
Chronic lung disease is a proposed risk factor for immune checkpoint inhibitor pneumonitis (ICI-pneumonitis); however, data is sparse regarding the impact of pre-existing lung disease and baseline chest imaging abnormalities on the risk of developing ICI-pneumonitis. (Source: Respiratory Medicine)
