Liquid Biopsy for Colorectal Cancer Screening: Not So Bloody Fast
Colorectal cancer (CRC) screening is an integral part of preventive care. Many countries worldwide have adopted centrally administered national programs, and others, like the United States, perform opportunistic screening. Regardless of delivery system or method —predominantly fecal immunochemical test [FIT] or colonoscopy—periodic screening cost-effectively reduces CRC incidence and mortality.1 CRC screening works through the following 2 mechanisms: early identification of colon neoplasia before malignant transformation and detection of cancers at an e arlier stage than unscreened patients. Gluten-free diet induces rapid changes in phenotype and survival properties of gluten-specific T cells in celiac disease
The treatment of celiac disease (CeD) with gluten-free diet (GFD) normalizes gut inflammation and disease-specific antibodies. CeD patients have HLA-restricted, gluten-specific T cells persisting in the blood and gut even after decades of GFD, which are re-activated and disease driving upon gluten exposure. Our aim was to examine the transition of activated gluten-specific T cells into a pool of persisting memory T cells concurrent with normalization of clinically relevant biomarkers during the first year of treatment. Of Humans and Machines in Endoscopy: Flying Solo, Instrument Aided, or on Autopilot?
Endoscopy is a triumph of science and engineering.1,2 During a busy day of procedures, we rarely stop and marvel at our routine: we delve deep into the tortuous gastrointestinal tract, topologically “outside of our bodies” but viscerally far “inside us”; we examine the barrier between our bodies’ substance and the outside world with high definition, magnification, and enhanced lighting, and we even peer beyond it; we advance instruments to diagnose and treat conditions that afflict hu mankind with substantial morbidity and mortality. Effectiveness and Cost-Effectiveness of Colorectal Cancer Screening With a Blood Test That Meets the Centers for Medicare & Medicaid Services Coverage Decision
Current test performance characteristics of blood-based colorectal cancer screening tests are insufficient to justify their high costs compared with less expensive and more effective alternatives such as fecal immunochemical testing, triennial stool DNA testing combined with a fecal immunochemical testing assay, and colonoscopy. Comparative Effectiveness and Cost-Effectiveness of Colorectal Cancer Screening With Blood-Based Biomarkers (Liquid Biopsy) vs Fecal Tests or Colonoscopy
Blood tests to screen for colorectal cancer could be effective in people who decline stool tests or colonoscopy, but they should not substitute for established methods unless they achieve high detection rates for colorectal cancer and its precursors. (Source: Gastroenterology)
