Impact of Longitudinal Alcohol Use Patterns on Long-Term Risk of Cirrhosis Among US Veterans With Steatotic Liver Disease
High-risk alcohol use in adults with steatotic liver disease increases the risk of liver disease progression. Timely alcohol use assessment and early interventions to address high-risk alcohol use are critical, particularly in patients with steatotic liver disease. (Source: Gastroenterology)

Impact of Longitudinal Alcohol Use Patterns on Long-Term Risk of Cirrhosis Among U.S. Veterans with Steatotic Liver Disease
Conflicting data exists on impact of alcohol use on risk of liver disease progression in patients with steatotic liver disease. We aim to evaluate the effect of longitudinal alcohol use on risk of cirrhosis among Veterans with steatotic liver disease. (Source: Gastroenterology)

Diagnostic and Prognostic Biomarkers of Chronic Pancreatitis: A Conceptual Framework Based on the PRoBE Design
Chronic pancreatitis (CP) is defined by clinical evidence of chronic fibroinflammatory pancreatic injury usually preceded by acute pancreatitis (AP) or recurrent AP. Clinical manifestations include AP, diabetes, exocrine insufficiency, metabolic bone disease, and pancreatic cancer.1 Abdominal pain, the most common symptom, significantly affects quality of life.2 Diagnosing CP relies on cross-sectional imaging features, which are not apparent until later stages. Additionally, the variable clinical presentation and course of CP make reliable diagnosis challenging. (Source: Gastroenterology)

TGF- β1-Induced SOX18 Elevation Promotes Hepatocellular Carcinoma Progression and Metastasis Through Transcriptionally Upregulating PD-L1 and CXCL12
We elucidated that SOX18 mediated by the TGF- β1-p-Smad2/3 axis shaped the immunosuppressive microenvironment. CXCR4 inhibitor AMD3100 or TGFβR1 inhibitor Vactosertib may boost anti-PD-L1 efficacy and represent novel combinational strategies. (Source: Gastroenterology)

TGF- β1 Induced SOX18 Elevation Promotes Hepatocellular Carcinoma Progression and Metastasis through Transcriptionally Upregulating PD-L1 and CXCL12
Hepatocellular carcinoma (HCC) is characterized by an immune-suppressive microenvironment, which contributes to tumor progression, metastasis and immunotherapy resistance. Identification of HCC-intrinsic factors regulating immunosuppressive microenvironment is urgently needed. Here, we aimed to elucidate the role of SYR-Related High-Mobility Group Box 18 (SOX18) in inducing immunosuppression and to validate novel combination strategies for SOX18-mediated HCC progression and metastasis. Efficacy and findings of a blinded randomized reintroduction phase for the low FODMAP diet in Irritable Bowel Syndrome
The efficacy of a low fermentable oligo-, di-, monosaccharides and polyols (FODMAP) diet in Irritable Bowel Syndrome (IBS) is well established. After the elimination period, a reintroduction phase aims to identify triggers. We studied the impact of a blinded reintroduction using FODMAP-powders to objectively identify triggers and evaluated the effect on symptoms, quality of life (QoL), and psychosocial co-morbidities. AGA Clinical Practice Guideline on Fecal Microbiota –Based Therapies for Select Gastrointestinal Diseases
Fecal microbiota –based therapies include conventional fecal microbiota transplant and US Food and Drug Administration–approved therapies, fecal microbiota live-jslm and fecal microbiota spores live-brpk. The American Gastroenterological Association (AGA) developed this guideline to provide recommendations on t he use of fecal microbiota–based therapies in adults with recurrent Clostridioides difficile infection; severe to fulminant C difficile infection; inflammatory bowel diseases, including pouchitis; and irritable bowel syndrome. Approaching Digestive Health Care in Transgender and Gender-Diverse Communities
Transgender and gender-diverse (TGD) people are more likely than cisgender people to experience adverse health outcomes and health care discrimination. These disparities remain uninvestigated in gastroenterology (GI) and hepatology. Here, we describe what is known about digestive clinical care in TGD people. Practical considerations are offered to assist in the provision of culturally humble care to TGD people. (Source: Gastroenterology)
