Altered Microbial Transcription in Long-term Proton Pump Inhibitor Use: Findings From a United States Cohort Study
Proton pump inhibitors (PPIs) are commonly prescribed to reduce gastric hydrochloric acid to treat disorders related to acid exposure, including gastroesophageal reflux disease (GERD) and peptic ulcer disease.1 Although PPIs are often prescribed for long-term use and generally considered safe, their functional impact on the gut microbiome, which may have implications for long-term health, is not fully understood. Their mechanism of action may exert evolutionary pressure on gut microbial communities through altered host immunity, increased gastric pH, and disruption of the gastric acid barrier. We thank Wei at al1 for their interest in our recent publication on the efficacy of Texture and Color Enhancement Imaging (TXI) for colonic adenoma detection.2 We agree with the assertion that given the inconspicuous nature of sessile serrated lesions (SSLs), TXI would be expected to improve SSL detection compared with white light imaging (WLI), as has been demonstrated in still image –based studies.3 Although we demonstrated a trend toward improved SSL detection with the use of TXI (10.4% vs 6.8%; P = .2), it did not reach statistical significance. Efficacy of Drugs Acting on Histamine 1 Receptors in Irritable Bowel Syndrome: Systematic Review and Meta-analysis
There is increasing evidence that a subset of patients with irritable bowel syndrome (IBS) have an organic explanation for their symptoms. Some patients exhibit evidence of mast cell activation,1,2 although the cause is unknown. Recent evidence suggests this could relate to a break in oral tolerance to a dietary antigen after an acute enteric infection.3 Degranulation of mast cells results in mucosal release of tryptase and histamine1 and, hence, immune activation and visceral afferent neuron hyperexcitability.
