B-1 B Cell-Derived Natural Antibodies against N-Acetyl-d-Glucosamine Suppress Autoimmune Diabetes Pathogenesis
We report that neonatal immunization with group A Streptococcus suppresses T1D development in NOD mice by promoting clonal expansion of N-acetyl-d-glucosamine (GlcNAc)-specific B-1 B cells that recognize pancreatic β cell-derived Ags bearing GlcNAc-containing posttranslational modifications. Early exposure to Lancefield group A cell-wall carbohydrate Ags increased production of GlcNAc-reactive serum Abs and enhanced localization of innate-like GlcNAc-specific B cells to pancreatic tissue during T1D pathogenesis. We show that B-1 B cell-derived GlcNAc-specific IgM engages apoptosis-associated β cell Ags, thereby suppressi...

Primary Peritonitis due to Group A Streptococcus Successfully Treated with Intraperitoneal Drainage
Intern Med. 2023 Sep 15. doi: 10.2169/internalmedicine.1933-23. Online ahead of print.ABSTRACTA 42-year-old woman was admitted to our hospital because of lower abdominal pain and diarrhea. Although the initial symptoms and imaging findings were similar to those of acute enteritis, blood and ascites cultures led to the diagnosis of primary peritonitis caused by group A Streptococcus. In many cases, the disease rapidly deteriorates, and laparotomy is performed for the early diagnosis and to reduce the number of bacteria in the abdominal cavity. In the present case, intraperitoneal drainage was effective for avoiding surgery....

Vulval disorders in paediatric and adolescent patients and their management
Key content Vulval disorders are one of the commonest complaints in the paediatric and adolescent population. They present with acute-on-chronic symptoms such as pruritus, vaginal discharge and vulval soreness. History-taking and examination of young patients requires a sensitive approach and age-appropriate use of language. The hypoestrogenic, exposed and neutral pH of the vulva in the paediatric patient is more susceptible to nonspecific inflammation and injury. Adolescents are more likely to present with dermatoses, thrush and infection caused by common pathogens such asEscherichia coli, group A streptococcus and sexual...

Adult Hand, Foot, and Mouth Disease
A previously healthy 22-year-old Japanese man presented with fever, sore throat, bilateral conjunctival congestion, and a skin rash, which he had been experiencing for 4 days before hospital admission. Two days before admission, he tested negative for Group A Streptococcus by rapid antigen testing and for SARS-CoV-2 by antigen testing. On the same day, he noticed a non-pruritic rash on both palms and soles. Despite taking non-steroidal anti-inflammatory drugs, he continued to have a high-grade fever and a sore throat caused by oral ulcers, prompting hospital admission due to inability to maintain adequate hydration. Necrotizing fasciitis
(NF) is a rare life-threatening skin and soft tissue infection. Most commonly the result of trauma, it rapidly spreads through the fascial planes and causes necrosis of the subcutaneous tissues. The disease is classified into different types depending on the underlying bacteria responsible, with Group A streptococcus reported as the most common organism found. Reaching a diagnosis of NF is often challenging due to its non-specific signs and symptoms. A delay to recognition and treatment significantly increases the risk of mortality. Increase in bloodstream infections caused by emm1 group A Streptococcus correlates with emergence of toxigenic M1UK, Belgium, May 2022 to August 2023
We present the epidemiology ofemm1 causing iGAS in Belgium during 2018–August 2023, and describe an emergence of the toxigenic M1UK lineage in Belgium in mid-2022 that was observed as an increase in bloodstream infections caused byemm1S. pyogenes that continued into 2023. (Source: Eurosurveillance)

Expansion of Invasive Group A Streptococcus M1 < sub > UK < /sub > Lineage in Active Bacterial Core Surveillance, United States, 2019 ‒2021
Emerg Infect Dis. 2023 Aug 28;29(10). doi: 10.3201/eid2910.230675. Online ahead of print.ABSTRACTFrom 2015-2018 to 2019‒2021, hypertoxigenic M1UK lineage among invasive group A Streptococcus increased in the United States (1.7%, 21/1,230 to 11%, 65/603; p<0.001). M1UK was observed in 9 of 10 states, concentrated in Georgia (n = 41), Tennessee (n = 13), and New York (n = 13). Genomic cluster analysis indicated recent expansions.PMID:37640370 | DOI:10.3201/eid2910.230675 (Source: Emerging Infectious Diseases)
