Update on aortopexy and posterior tracheopexy for tracheomalacia in patients with esophageal atresia
AbstractDespite improving the survival after repair of esophageal atresia (EA), the morbidity of EA repair remains high. Specifically, tracheomalacia (TM) is one of the most frequent complications of EA repair. Continuous positive airway pressure is generally applied for the treatment of TM. However, surgical intervention is required against an apparent life-threatening event or inability to perform extubation for a long period. According to our review, most cases of TM showed symptom improvement after aortopexy. The ratio of the trachea ’s lateral and anterior–posterior diameter at the brachiocephalic artery crossing ...

Minimally invasive surgical approach in children treated for oesophageal atresia is associated with attention problems at school age: a prospective cohort study
Conclusion: Children who undergo minimally invasive surgery for OA correction are at risk for sustained attention problems at school age. Future studies unravelling the effects of perioperative events on neurodevelopment should lead to optimal surgical, anaesthesiological, and intensive care management in the neonatal period. What is Known:• School-aged children born with oesophageal atresia have normal intelligence but problems with sustained attention at eight years. What is New:• Oesophageal atresia patients, who undergo minimally invasive surgery or who have a background of lower socioeconomic status are at serious r...

Long-term airway outcomes and interventions in children with oesophageal atresia with tracheoesophageal fistula: A 20-year single centre observational study
Airway anomalies, symptoms and interventions are commonly reported in children with oesophageal atresia with tracheoesophageal fistula (OA/TOF). The purpose of this study was to assess the incidence of these airway pathologies and those requiring interventions in the long-term. (Source: Journal of Pediatric Surgery)

Letter to the Editor in Response to: What Proportion of Children with Complex Oesophageal Atresia Require Oesophageal Lengthening Procedures?
We read with interest Phillips and Jaffray ’s article on success of delayed primary anastomosis (DPA) for complex oesophageal atresia (OA)[1]. While we value the ‘words of caution’ regarding adoption of novel procedures, the manuscript is biased. For debate, one must consider the merits and drawbacks of DPA and traction for challenging cases of OA. (Source: Journal of Pediatric Surgery)

Repair of complex esophageal atresia with tracheobronchial remnant using special magnets
We describe our experience with a patient found to have long-gap EA with a distal cartilaginous ring who was managed using a combination of esophageal lengthening and magnetic compression anastomosis. A 5-month-old girl was referred to us from an outside hospital with type C EA including a very high upper pouch. She had undergone a prior thoracotomy with fistula ligation during which a clip was placed on the lower esophagus, leaving a 2-cm diverticulum on the trachea and a short lower esophageal pouch. Upon endoscopic evaluation at our center, we found a tracheobronchial remnant in the lower esophagus between the clip and ...

Neurodevelopmental evaluation of children who were operated due to congenital anomaly with the ‘Ages and Stages Questionnaire (ASQ)’ and ‘ASQ: Social–Emotional’
ConclusionsIn children who underwent surgery due to congenital anomalies, an evaluation through developmental tests, a post-surgical follow-up process, and a referral to the relevant disciplines when necessary may increase the success of surgery as well as increase the life quality of the patient. To assess the cell lines on esophagogastroduod enoscopies of children with feeding difficulty, esophageal fistula and asthma with histologic abnormality
Cell Mol Biol (Noisy-le-grand). 2023 Dec 31;69(15):12-20. doi: 10.14715/cmb/2023.69.15.3.ABSTRACTThe purpose of this study is to evaluate the cell lines seen on esophagogastoduodenoscopy (EGD) of children who have difficulties feeding, esophageal fistula, and asthma that has some histologic abnormalities. This is a study that looked back at the medical records of 100 children whose cells had been surgically restored after being impacted by EA or TEF. A review of the instrumental tests that were carried out at our facility has been conducted in order to identify any lingering anatomic or functional abnormalities of the airw...

Surgical treatment for severe pediatric tracheobronchomalacia: the 20-year experience of a single center
CONCLUSIONS: Isolated or combined surgical procedures improved respiratory symptoms in 80 % of children with severe tracheobronchomalacia. The choice of procedure should be individualized and guided by etiology: anterior aortopexy for anterior compression, posterior tracheopexy for membranous intrusion, and posterior descending aortopexy for left bronchus obstruction.PMID:38278512 | DOI:10.1016/j.jped.2023.10.008 (Source: Jornal de Pediatria)

A Narrative Review of Patient-Reported Outcome Measures and Their Application in Recent Pediatric Surgical Research: Advancing Knowledge and Offering New Perspectives to the Field
Conclusions This review reveals gaps in the application of PROMs in recent pediatric surgical research. Emphasis should be placed on the development and utilization of psychometrically robust PROMs, broadening the scope of covered diseases, conducting impact assessments, and evaluating implementation strategies.
