Oxycodone for neuropathic pain in adults.
CONCLUSIONS: There was only very low quality evidence that oxycodone (as oxycodone MR) is of value in the treatment of painful diabetic neuropathy or postherpetic neuralgia. There was no evidence for other neuropathic pain conditions. Adverse events typical of opioids appeared to be common. Hyperprolactinemia Secondary to Pituitary Microadenoma versus Haloperidol-A Diagnostic Enigma: A Case Report and Brief Review.
CONCLUSION: Diagnosis and etiology of hyperprolactinemia were complicated by the minimal nature of clinical symptoms, the type of antipsychotic agent and the prolactin level. The MRI facilitated the diagnosis of pituitary microadenoma and further treatment option with bromocriptine. MRI of the pituitary is indicated for patients with hyperprolactinemia where the etiology is not clearly due to medication. Sporadic and familial myoclonic dystonia: Report of three cases and review of literature
We report one sporadic case and one familial case where the father and the son are affected. The cases were collected from the Movement Disorders Clinic of Bangur Institute of Neurosciences, Kolkata, West Bengal in a period of ten months. Myoclonic dystonia is a rare condition and to the best of our knowledge, this series is the first one reported from our country. Videos of the patients are also provided with the article. Notes from the Field: Thyrotoxicosis After Consumption of Dietary Supplements Purchased Through the Internet - Staten Island, New York, 2015.
Abstract On June 18, 2015, a woman aged 30 years was brought to the Staten Island University Hospital Emergency Department (ED) in New York by her mother, who reported that the patient had become acutely confused at home, was repeating herself, and did not recognize her family members. She had a diagnosis of bipolar disorder for which she took lithium, and she had a history notable for polysubstance abuse (use of three or more addictive drugs in the past 12 months). Her other medications included risperidone (an antipsychotic), benztropine (an anticholinergic), and bupropion (an antidepressant). ED staff m...

α-Synuclein-induced myelination deficit defines a novel interventional target for multiple system atrophy
In conclusion, this study defines the α-synuclein-induced myelination deficit as a novel and crucial pathomechanism in MSA. Importantly, the reversible nature of this oligodendrocytic dysfunction opens a novel avenue for an intervention in MSA. (Source: Acta Neuropathologica)

Prolonged Antimuscarinic Delirium in a Child Due to Benztropine Exposure Treated With Multiple Doses of Physostigmine
Abstract: An 11-year-old boy presented with an antimuscarinic toxidrome due to benztropine and risperidone ingestion. His delirium was prolonged and difficult to treat with benzodiazepines. Multiple doses of physostigmine successfully treated it. Benztropine is a potent antimuscarinic agent, whereas risperidone has not been reported to cause antimuscarinic toxicity. The use of physostigmine to treat benztropine intoxication in a pediatric patient has not previously been described. In this case, multiple doses were used and were well tolerated. Benztropine for the relief of acute non-traumatic neck pain (wry neck): a randomised trial
Conclusions Benztropine was ineffective for reducing pain or improving range of motion of the cervical spine in patients suffering from acute, non-traumatic neck pain, but frequently caused anticholinergic side effects. However, as the CI for the primary outcome included the minimum difference considered clinically significant, an important effect of benztropine cannot be ruled out. Persistence of racial disparities in prescription of first‐generation antipsychotics in the USA
ConclusionsRacial disparities in the pharmacological treatment of severe mental disorders persist 30 years after the introduction of second‐generation antipsychotics. The relatively high frequency of FGA of use among Black patients compared with White patients despite more Food and Drug Administration‐approved indications and lower EPS risk for second‐generation antipsychotics requires additional research. Oxycodone for neuropathic pain and fibromyalgia in adults.
CONCLUSIONS: No convincing, unbiased evidence suggests that oxycodone (as oxycodone CR) is of value in treating people with painful diabetic neuropathy or postherpetic neuralgia. There is no evidence at all for other neuropathic pain conditions, or for fibromyalgia. Adverse events typical of opioids appear to be common. Acute Benztropine Intoxication and Fatality.
Abstract A woman was found unresponsive with an empty bottle of Cogentin(®) prescribed to another. Admitted to an area hospital, her condition steadily declined until death 29 h after admission. Following toxicological screening on hospital (admission) whole blood, the only significant compound detected was benztropine. Benztropine was confirmed at 0.28 mg/L - the highest antemortem blood concentration recorded in a case of toxicity or fatality uniquely associated with benztropine. A second serum antemortem specimen showed a benztropine concentration of 0.19 mg/L. Despite over 24 h in the hospital, be...

Modulation of methamphetamine's locomotor stimulation and self-administration by JHW 007, an atypical dopamine reuptake blocker.
Abstract JHW 007 [N-(n-butil)-3α-[bis(4'-fluorophenil)methoxi]-tropane] belongs to the family of N-substituted benztropine (BZT) analogs, atypical dopamine transporter (DAT) blockers that are able to strongly modulate cocaine- and amphetamine-related behaviour. In the present study, we tested in rats the ability of JHW 007 to alter the stimulant and reinforcing properties of methamphetamine (METH) using locomotor activity, fixed ratio and progressive ratio (PR) self-administration tests. The results showed that JHW 007 attenuated METH-induced locomotor stimulation in a dose-dependent manner and had no sti...

Chronic and bedtime use of benztropine with antipsychotics: Is it necessary?
The cholinergic system plays an important role in the quality of life and physical health. Anticholinergic drugs cause adverse effects such as dry mouth, constipation, blurry vision, and even acute angle closure glaucoma. Benztropine (Cogentin) is an anticholinergic drug used to treat acute extrapyramidal symptoms (EPS) caused by antipsychotics. Benztropine is also a dopamine reuptake inhibitor, which makes it useful for people with EPS who are on antipsychotics. EPS includes various involuntary muscle movement disorders such as dystonia, bradykinesia, pseudoparkinsonism, and dyskinesia. (Source: Schizophrenia Research)

Treatment of Intraoperative Persistent Penile Erection in a Stallion
Abstract: This case report describes the occurrence of persistent penile erection in a breeding stallion that occurred while the horse was under inhalant anesthesia for a carpal arthroscopy. The horse had no history of breeding problems, and no abnormalities were detected on physical examination, complete blood count, or serum chemistry tests performed prior to surgery. Anesthesia was induced with guaifenesin and ketamine after sedation with xylazine and was maintained with isoflurane in 100% oxygen. Penile erection developed approximately 35 minutes after induction and persisted for over 2 hours despite various physical...

Nonfatal tramadol overdose may cause false-positive phencyclidine on Emit-II assay
This report describes 2 patients with positive phencyclidine (PCP) screens using the Emit II Plus Drugs-of-Abuse Test (Siemens Healthcare Diagnostics, Inc, Tarrytown, NY) after tramadol ingestion. Our first case was a 43-year-old woman with a history of bipolar disorder who presented to the emergency department(ED) for medical clearance after ingesting an unknown amount of tramadol. Her urine drugs-of-abuse screen was positive for PCP. A comprehensive gas chromatography/mass spectrometry qualitative urine drug screen demonstrated valproic acid metabolites, nicotine, gabapentin, benztropine, and a large peak of tramadol. Ou...
