1 year attending job?
Hi
I am a rads resident about to rank and match for fellowship. Spouse is an FM PGY 2. Rank list right now is all over the country, mostly in big metros/big academic centers. We will likely return to the area we are currently in after my fellowship year (its her hometown). We aren't interested in doing distance for a year. She isn't really interested in a fellowship year either.
Would it be feasible for wife to have a 1 year attending job wherever I end up for fellowship? Or would...

Boston University vs. Rush
I've been very fortunate to get to pick between these options but am feeling pretty torn. I was initially very set on BU because of the higher ranking/prestige (cost of attendance would be nearly identical), but after visiting Rush I am reconsidering. What to do with my hatred of academic medicine?
Essentially, I have grown to despise academic medicine. I see so many problems linked to academic medicine that plague medicine now. Attending subspecialty conferences during intern year?
I matched into psychiatry for the incoming 2024 cohort starting in July. I have long wanted to pursue forensic psychiatry even during undergrad, and I can see that while fellowships in general are not competitive, the more "prestigious" programs are getting more selective over time.

How to improve? 34 y/o, 2nd time, low GPA, mental health
I am 34 and it’s been over 10 years since completing my prerequisites. I “went out with a bang” and tanked my GPA my last year of undergraduate courses due to mental health concerns. I will address this in my explanation statement, but I need an academic comeback. I know my GPA is really low.
I am also a disadvantaged student (raised in abusive/addiction family), and therefore have been looking into Colorado’s Vet Prep program. However I know they only pick 10.

Huge upward GPA trend
I have an enormous cGPA trend. After my first 3 years, I transferred and did 5 more years of school (BS Biology & Math, BS Biomedical Engineering, MS Biomedical Engineering).
cGPA by year
1.72, 1.77, 1.16... ... ... 2.94, 3.57, 3.94, 3.98, 3.97
My cGPA over my entire academic career is a 3.06 (sGPA is comparable).
However, looking at just my last 4 years, I have a cGPA of 3.73 over 226 credits.
