The first ablation
Before the 1980s, management of supraventricular arrhythmias that were refractory to drugs were treated surgically. The procedure involved thoracotomy, cardiotomy, and ligation or cryoablation of the atrioventricular (AV) junction. In the late 1970s, I conceived the idea of trying to replicate the surgical procedure but without the need for open heart surgery. A literature survey described a number of direct techniques used in animals to promote AV block, but I was struck by an abstract published by Beazell et  al. (Source: Heart Rhythm)

The Effects of Various Approaches to Lobectomies on Respiratory Muscle Strength, Diaphragm Thickness, and Exercise Capacity in Lung Cancer
CONCLUSION: The VATS technique appears superior to the CT technique in terms of preserving respiratory muscle strength and functional exercise capacity. Thoracic surgeons should refer patients to physiotherapists before lobectomy, especially patients undergoing CT. If lobectomy with VATS will be technically difficult, MST may be an option preferable to CT because of its impact on exercise capacity.PMID:38679681 | DOI:10.1245/s10434-024-15312-x (Source: Ann Oncol)

Comparison of Early Postoperative Diaphragm Muscle Function after Lobectomy via VATS and Open Thoracotomy: A Sonographic Study
In conclusion, the VATS technique seems to be less invasive than conventional thoracotomy providing a better postoperative function of the main respiratory muscle.PMID:38672757 | DOI:10.3390/life14040487 (Source: Cancer Control)

Primary pulmonary monophasic synovial sarcoma initially presenting with bloody pleural effusion: A case report and literature review
We report the first case of a 57-year -old man with bloody pleural effusion as an initial manifestation of PPSS in the middle lobe of the right lung diagnosed after surgery. Diagnosis: Chest computed tomography (CT) revealed a mass in the middle lobe of the right lung, which was pathologically diagnosed as a monophasic SS after surgical resection. Interventions: Ten days after preoperative closed chest drainage, a right thoracotomy was performed to remove the right middle lobe of the lung. Outcomes: The patient recovered smoothly and was discharged from the hospital without any other postoperative treatment. A follow-up ch...

N-Butyl cyanoacrylate embolization of an extremely rare variant of sequestration complex — a high-flow left-to-left shunt between systemic artery and pulmonary vein
AbstractA 19-year-old female presented with hemoptysis. Computed tomography (CT) pulmonary angiography revealed aberrant vessels from descending thoracic aorta, draining into pulmonary veins (left-to-left shunt). She was managed by transcatheter embolization of the aberrant vessels using N-butyl cyanoacrylate (NBCA) with balloon occlusion. A systemic artery to pulmonary vein fistula is one of the least common congenital anomalies. Most of the reported cases have been managed by surgery. Only a few patients have been treated by transcatheter embolization, using coils or vascular plugs as the embolizing agents. To our knowle...

Robotic-Assisted Hybrid Coronary Revascularization
The authors reported 285 patients who underwent reverse robotic-assisted hybrid coronary revascularization (HCR). Reverse HCR consists of percutaneous coronary intervention in non-left anterior descending (LAD) coronary artery vessels performed within 30 days before robotic-assisted left internal thoracic artery (LTA) harvesting and LTA to LAD manual anastomosis through a 4 cm left mini-thoracotomy. Risk Factors for Empyema Following Penetrating Diaphragmatic Injuries
Am Surg. 2024 Apr 20:31348241248700. doi: 10.1177/00031348241248700. Online ahead of print.ABSTRACTEmpyema resulting as a complication of penetrating diaphragmatic injuries is a subject that requires further investigation, and the aim of this study was to determine the risk factors associated with empyema in patients with penetrating trauma. Consecutive adult trauma patients from a level 1 trauma center were searched for penetrating diaphragm injuries. Data were collected on patient demographics, pre-existing conditions, injury type and severity, hospital interventions, in-hospital complications, and outcomes. Patients wer...

Feasibility of an animal model for long-term mechanical circulatory support with Impella 5.5 implanted through carotid artery access in sheep
This study aimed to evaluate the feasibility of an animal model for long-term support with Impella 5.5 implanted through carotid artery access in sheep.Impella 5.5 was implanted in four sheep through the proximal region of the left carotid artery without a thoracotomy, and myocardial injuries were induced by coronary microembolization. Support by Impella 5.5 was maintained for 4 weeks, and the animals were observed. The position of Impella 5.5 and cardiac function was evaluated using cardiac computer tomography at 2 and 4 weeks after implantation. All four animals completed the 4-week study without major complications. The ...

Clinical Impact of the Endo-aortic Clamp for Redo Mitral Valve Surgery
AbstractAim of this study was to compare redo MV surgery patients undergoing right mini-thoracotomy and EAC with redo MV patients undergoing surgery through other approaches. Redo MV patients from 7 European centers were analyzed. Primary endpoint was 30-day mortality; secondary endpoints were stroke, re-exploration, low cardiac output syndrome (LCOS), respiratory failure, and intensive care unit (ICU) and in-hospital length-of-stay. Forty-nine patients underwent right mini-thoracotomy and EAC (22.7%), and 167 (77.3%) underwent surgery through other approaches (112 sternotomy, 40 unclamped mini-thoracotomies, and 15 mini-t...
