Pediatric fellowship after PP
Hi folks, I have been in an MD only private practice group as a generalist for the last 2.5 years that requires a fairly hefty pediatrics load of about ~20-25% of the practice. thoracic epidural tips?
Anyone who do a lot of these have tips for VATS/thoracotomies, especially with paramedian techniques? Place i trained didn't have a ton of paramedian numbers for thoracic cases. most of the thoracic epidurals i did was for abdominal cases were lower, which were just mostly midline I have done some paramedians with variable success long time ago in residency(n probably too low to tell how good i am), but i will be doing more in my new practice. ESP block
Seeing more and more in the literature. Several nice benefits. Used it again today: surgeon unexpectedly opened up a lap choley and pt had huge incision just under rib cage and I wasn’t sure if TAPS would do it. 0/10 pain in pacu. Even witnessed him cough without flinching a bit and ask for more ice chips. Lobectomy Fluid Management
Seeking comments and advice from those of you much wiser and more experienced than than I am. (All of you) Question about intraoperative fluid management in this theoretical case: Morbidly obese 67 yo F 5'2" 137kg with COPD and history of delayed emergence presents for right VATS upper lobectomy. Case goes for 8hrs induction to emergence (no conversion to thoracotomy). Difficult DLT placement takes 20 minutes of tweaking to sit right. Ultimately complicated by ~1200ml blood loss, lowest... Face Transplant Surgery
All, but one, of the lead surgeons in facial transplant cases, Siemionow etc, are plastic surgeons. Residency trained in 5 year general surgeons + 2/3 year plastic surgery residency. (Devauchelle from France is an OMF surgeon, but he's the only non plastic program lead.) Wouldn't an ENT residency followed by a 2/3 year plastic surgery residency be better training for the vastly complex facial reconstruction required for face transplants; compared to thoracotomies and bowel resections...
