Characterisation of patients with chronic obstructive pulmonary disease initiating single-device inhaled corticosteroids/long-acting β < sub > 2 < /sub > -agonist dual therapy in a primary care setting in England
CONCLUSION: Patient characteristics, prior treatments, prior COPD-related HCRU and direct healthcare costs were similar across single-device ICS/LABAs in primary care in England. A high proportion of patients were not receiving any respiratory medication pre-index, indicating that prescribing in primary care in England is more closely aligned with national guidelines than global treatment strategies. Comorbid asthma may have influenced prescribing decisions. Less than half of users had preindex exacerbations, suggesting that ICS/LABA is not being prescribed principally based on exacerbation history.PMID:36171051 | DOI:10.1...

Using SMART Regimens in PHM
PHM Session: Get SMART: Implementation of updated asthma guidelines for pediatric hospitalists Presenters: Alexander Hogan, MD, MS, Connecticut Children’s Medical Center, Hartford, Conn., Kathryn Kyler, MD, MSc, and Claire Seguin, MD, Children’s Mercy Kansas City, Mo. This session focused on how pediatric hospitalists can improve the asthma care of patients discharged from hospitalizations by instituting single maintenance and reliever therapy (SMART) regimens. More specifically, this is a single inhaler that includes both an inhaled corticosteroid (ICS) and a long-acting beta-agonist (LABA), which have U.S. Food and D...

Narrative Review of the Role of Patient-Reported Outcomes and Inhaler Handling Errors in the Control of Asthma and COPD
AbstractPurpose of ReviewAsthma and chronic obstructive pulmonary disease (COPD) are chronic respiratory diseases that remain uncontrolled in many patients, despite the wide range of therapeutic options available. This review analyzes the available clinical evidence on 3 budesonide/formoterol DPI devices, Spiromax®, Turbuhaler®, and Easyhaler®, in terms of patient-reported outcomes (PROs), inhaler errors, and asthma and COPD control.Recent FindingsThe effectiveness of dry powder inhalers (DPI) depends largely on the device and the patient ’s inhaler technique. Equally important are the patient’s perception of the in...

Repeated dose budesonide/formoterol compared to salbutamol in adult asthma: a randomised crossover trial
Conclusions The comparative bronchodilator responses of repeated administration of salbutamol 200 µg and budesonide/formoterol 200/6 µg differed depending on the time of measurement. Salbutamol caused greater systemic β2-agonist and cardiovascular effects and more adverse events. (Source: European Respiratory Journal)

Oral leukoplakia in a smoking patient using inhaled formoterol and budesonide
A 67-year-old non-White female patient was referred to our service for evaluation of a white oral lesion. The patient reported being a heavy smoker for 56 years and a 15-year history of asthma controlled with inhaled formoterol fumarate and budesonide for 15 years. Extraoral examination showed no relevant alterations. Intraoral examination revealed a nondetachable white verrucous plaque on the right side of the soft palate close to the limit with the hard palate. The lesion measured approximately 1.5  × 1.0 cm and had regular borders and well-defined limits. (Source: Oral Surgery, Oral Medicine, Oral Pathology, Oral R...

SMART in treatment of asthma exacerbations
CONCLUSION: Although not all studies demonstrated the effectiveness of SMART, the majority revealed a significant reduction in asthma exacerbation frequency and severity.PMID:35914659 | DOI:10.1016/j.anai.2022.07.024 (Source: Annals of Allergy, Asthma and Immunology)
