Drifting Off To Sleep
The more I do this job, I realize what an honor and privilege it is to take care of patients. As anesthesia, we are the last ones our patients see and hear before drifting off to sleep. Rarely, sometimes unexpectedly, they may have complications and never wake up. In light of knowing you may be the last person a patient sees/hears, what do you guys say to your patients as you induce to comfort them? Is how I envision my sleep career realistic??
Going into pedi pulm/sleep. Hoping to make 200k+, have maybe 3 days a week in clinic and a couple of days per week to read sleep studies/do other office work or clinical research. Maybe some inpatient time but not too too much, like maybe a month per year (Source: Student Doctor Network)

Any other nocturnists here? Do u think nightshifts has affected u negatively?
I've been an IM nocturnist for the past 5 years, first job out of residency. I work a relatively light shift 18 shifts/month ~avg 2 admits per shift - though the patients in my hmo group are generally fairly sick/complicated theres two issues that i'm not sure are actual issues, want to know what other people's experiences are 1. Do you think working nights chronically has affected your health at all? -I've generally been trying to keep a regular sleep schedule by sleeping during the day... Any other nocturnists here? Life as an associate!
Up all night and can’t sleep. I was curious if your mentor assists you with an extraction or extractions on a patient that are difficult have you had them take those extractions out of your pay? Daylight Savings Time and Mood Disorders
I can't stand the time change. It is initially somewhat disruptive to my sleep regimen and getting dark at 5pm is just lousy. With regard to patients there is a definite uptick in mood dysregulation both in the fall and spring. I keep hoping it will become a thing of the past and wonder if looking at it from a financial standpoint ie. increased inpatient hospitalizations, MVAs etc might aid in abolishing this outdated tradition. How to Cope with Exam anxiety?
First of all, I’ve never had anxiety before college. When I started college this became a problem for me. My exam anxiety is absolutely horrible. The night before exams I just can’t sleep, and during the exam I get sweaty and cold and shaky and I make mistakes I simply wouldn’t have made otherwise. Following the exam, I get little sleep and am constantly worried about little mistakes I did that I wish I could fix. I just wrapped up a genetics exam today and a biostat exam, and all day... New Meds
There have been several new meds in neurology over the past few years. While many are horrible 'me too,' or the ultimate 'me too' meds: XR versions, poly pills like Aricept+memantine, there has been very real innovation that's helped patients a great deal. I'm thinking of all the new meds in MS: pills, biologics, including ocrelizumab for PPMS. Two Chief Complaints that drive me nuts.
Title says it all. #1.) "My widget is broken". - This is generally a PEG tube that is broken, or a line that won't flush, or a whatever. In nearly every circumstance, this can either WAIT UNTIL MORNING and/or CAN BE TAKEN CARE OF IN A THOUSAND OTHER SETTINGS THAN THE ER. 3:30 AM PEG tube dysfunction?! Put the patient back to bed, and then call the facility MD at 7 AM. 10:30 PM catheter that won't drain urine? Flush it, and let the old man sleep. He'll sleep hard overnight. Then, sort... Waking up at 4pm every day
I only have classes 2 days of the week (Tuesday and Thursday) (Its my senior year before med school), and they go from 8 am to about 4 pm. On these days, I'm massively productive and get most of my studying done. However, on the other 5 days, I'm grossly unproductive. I would go to bed at a normal time (12-1 am). I intend to get up at 8, but often get up at 11 or noon (not that I need the extra sleep, but I can sleep 24 hours at a time if given the chance. Then, I twiddle my fingers until 4...
