Supportive care and antiviral treatments in primary herpetic gingivostomatitis: a systematic review
CONCLUSIONS: Although PHGS is a disease with a high worldwide prevalence, the lack of consensus about therapeutic management indicates gaps in existing evidence. Most of the proposed treatment consists in symptomatic drugs with empiric regimens which are ineffective for the viral replication. The main limit to realize randomized clinical trial is due to the rapid onset and remission of the disease. In fact, the diagnostic delay, estimated in 72 h, decreases the effectiveness of any antiviral drugs.CLINICAL RELEVANCE: Out of the five studies included in this systematic review, only one was able to provide some weak evidence...

The effect of herpes zoster vaccination on the occurrence of deaths due to dementia in England and Wales
CONCLUSIONS: Our findings indicate that HZ vaccination improved cognitive function at a fairly advanced stage of the dementia disease process because most individuals whose underlying cause of death was dementia during our nine-year follow-up period were likely already living with dementia at the start of the HZ vaccination program. By using a different population, type of data, and outcome than our prior study in Welsh electronic health record data, this analysis adds to the evidence base that HZ vaccination slows, or potentially even prevents, the natural history of dementia.PMID:37732219 | PMC:PMC10508823 | DOI:10.1101/2023.09.08.23295225

Dabrafenib and trametinib in Langerhans cell histiocytosis and other histiocytic disorders
Haematologica. 2023 Sep 21. doi: 10.3324/haematol.2023.283295. Online ahead of print.ABSTRACTStandard treatment for Langerhans Cell Histiocytosis (LCH) is chemotherapy, with high failure rates. Since MAP-kinase activating mutations are found in most cases, BRAF- and MEKinhibitors have been used successfully to treat patients with refractory or relapsed disease. However, data on long-term responses in children are limited and no data exist on their use as first-line therapy. We treated thirty-four patients (26 LCH, 2 Juvenile Xanthogranuloma, 2 Rosai-Dorfman Disease, 4 presumed single site-CNS histiocytosis) with either dab...

Non-coding RNAs: The potential biomarker or therapeutic target in hepatic ischemia-reperfusion injury
World J Gastroenterol. 2023 Sep 7;29(33):4927-4941. doi: 10.3748/wjg.v29.i33.4927.ABSTRACTHepatic ischemia-reperfusion injury (HIRI) is the major complication of liver surgery and liver transplantation, that may increase the postoperative morbidity, mortality, tumor progression, and metastasis. The underlying mechanisms have been extensively investigated in recent years. Among these, oxidative stress, inflammatory responses, immunoreactions, and cell death are the most studied. Non-coding RNAs (ncRNAs) are defined as the RNAs that do not encode proteins, but can regulate gene expressions. In recent years, ncRNAs have emerg...

Very early onset perinatal constipation: Can it be cow's milk protein allergy?
World J Gastroenterol. 2023 Sep 7;29(33):4920-4926. doi: 10.3748/wjg.v29.i33.4920.ABSTRACTDelayed passage of meconium or constipation during the perinatal period is traditionally regarded as a signal to initiate further work up to evaluate for serious diagnoses such as Hirschsprung's disease (HD), meconium ileus due to Cystic Fibrosis, etc. The diagnosis of HD particularly warrants invasive testing to confirm the diagnosis, such as anorectal manometry or rectal suction biopsy. What if there was another etiology of perinatal constipation, that is far lesser known? Cow's milk protein allergy (CMPA) is often diagnosed in infa...

Prevention and management of hepatitis B virus reactivation in patients with hematological malignancies in the targeted therapy era
World J Gastroenterol. 2023 Sep 7;29(33):4942-4961. doi: 10.3748/wjg.v29.i33.4942.ABSTRACTHepatitis due to hepatitis B virus (HBV) reactivation can be serious and potentially fatal, but is preventable. HBV reactivation is most commonly reported in patients receiving chemotherapy, especially rituximab-containing therapy for hematological malignancies and those receiving stem cell transplantation. Patients with inactive and even resolved HBV infection still have persistence of HBV genomes in the liver. The expression of these silent genomes is controlled by the immune system. Suppression or ablation of immune cells, most imp...

Direct oral anticoagulants for the treatment of splanchnic vein thrombosis: A state of art
World J Gastroenterol. 2023 Sep 7;29(33):4962-4974. doi: 10.3748/wjg.v29.i33.4962.ABSTRACTSplanchnic vein thrombosis (SVT) is a manifestation of venous thromboembolism in an unusual site. Portal, mesenteric, and splenic veins are the most common vessels involved in SVT which occurs mainly in patients with liver cirrhosis, although non-cirrhotic patients could be affected as well. Thrombosis of hepatic veins, also known as Budd-Chiari syndrome, is another manifestation of SVT. Prompt diagnosis and intervention are mandatory in order to increase the recalization rate and reduce the risk of thrombus progression and hypertensi...

The effects of cycling on walking outcomes in adults with stroke: a systematic review
CONCLUSION: Considerable heterogeneity was observed across cycling protocols with respect to intensity, frequency, exercise duration and protocol duration. However, none of the studies reported declines in walking outcomes and improvements were measured in the absence of task-specific gait training. Cycling interventions employing moderate- to high-intensity aerobic training and 24 sessions or more may be optimal in improving gait velocity and walking capacity.PMID:37732513 | DOI:10.1080/10749357.2023.2259167
