Septic cerebral emboli as a risk factor for thrombolysis-related haemorrhagic transformation
For the sake of completeness the risk factors enumerated by Wanget al. for haemorrhagic transformation of acute cerebral infarcts after intravenous thrombolysis (IVT)1 ought to include infective endocarditis (IE)-related septic emboli.2 In the latter study, which covered the period 2002 –10, 222 patients (mean age 69; 46% female) were identified who were treated with IVT for acute ischaemic stroke associated with IE. David aortic valve-sparing reimplantation versus biological aortic root replacement: a retrospective analysis of 411 patients
ConclusionsShort-term outcomes for both AVr-D and ARr-B-bio are excellent in patients with aortic root pathology. The long-term outcomes were associated with comparable survival and freedom from reoperation. AVr-D may be preferable to ARr-B-bio in patients with suitable pathoanatomy.

Short and long-term outcomes of medically treated isolated left-sided endocarditis: a retrospective study with 5-year longitudinal follow-up
ConclusionNon-operative management of LSIE carries a poor prognosis but may have a role in select cases. Surgical management remains the mainstay for patients with clear surgical indications, including severe regurgitation and PVE. Further prospective analyses are required to better delineate appropriate patient selection for non-surgical management.

Left-sided infective endocarditis in persons who inject drugs
ConclusionsLSIE in PWID is not uncommon. Compared to non-LSIE in PWID, valve surgery is more common and mortality is higher. For reasons that are unclear, stroke is more frequent in LSIE in PWID than in non-PWID with LSIE but mortality is no different.

Mitral valve prosthesis choice in patients < 70 years: A systematic review and meta ‐analysis of 20 219 patients
ConclusionMechanical MVR in patients aged less than 70 years is associated with a lower risk of operative mortality as well as a 20% lower risk of long ‐term death and 65% lower risk of mitral valve reoperation but 60% greater risk of bleeding compared with bioMVR in matched or adjusted data.

Comparison Between Healthcare-Associated and Community-Acquired Infective Endocarditis at Tertiary Care Hospitals in Japan.
CONCLUSIONS: HAIE has different clinical characteristics and causative microorganisms, as well as worse prognosis, than CIE. Implications of Methicillin-Resistant Staphylococcus aureus Carriage on Cardiac Surgical Outcomes.
CONCLUSIONS: MRSA carriers undergoing cardiac surgery are not at higher risk for mortality or SSI and can expect outcomes similar to those of non-carriers. Higher rates of postoperative MRSA infection and septicemia among carriers, although still very low, support the need for selective preoperative screening and prophylaxis when possible. Aortic and mitral valve surgery for infective endocarditis with reconstruction of the intervalvular fibrous body: an analysis of clinical outcomes.
Conclusions: The double-valve replacement and AMC reconstruction (the Commando procedure) is an effective technique in complex heart valve disease. The short- and mid-term results with this technique are optimal, with a very low in-hospital mortality and nearly 100% of long-term survival during follow-up. PMID: 32395280 [PubMed]

The fate of active left ‐side infective endocarditis with operative indication in absence of valve surgery
ConclusionThe mortality of this cohort of patients is extremely high. A multidisciplinary evaluation is of paramount importance in the decision ‐making process with shared responsibility for denial of operative options. In a perspective of correct healthcare resources allocation an early palliative care consult may need to be considered in some of those patients.
