Profile of infective endocarditis observed from 2003 - 2010 in a single center in Italy
Conclusion: S. aureus emerged as the leading causative organism of IE in a University hospital in northern Italy. Our study confirmed the high in-hospital mortality of IE, particularly if health-care associated, and the protective role of surgery.

Clinical significance of cerebrovascular complications in patients with acute infective endocarditis: a retrospective analysis of a 12-year single-center experience
Conclusions: IE can give rise to various forms of CVC, most frequently, acute ischemic brain lesions. In our study, major CVC was associated with high risk of mortality although total CVC was not significantly related to the risk of death in patients with IE.

The Association Between Surgical Indications, Operative Risk and Clinical Outcome in Infective Endocarditis: A Prospective Study From the International Collaboration on Endocarditis.
CONCLUSIONS: -Surgical decision-making in IE is largely consistent with established guidelines, although nearly one-quarter of patients with surgical indications do not undergo surgery. Operative risk assessment by STS-IE score provides prognostic information for survival beyond the operative period. S. aureus IE was significantly associated with non-surgical management. ConclusionsLong‐term survival after surgery for native mitral valve IE was independently influenced by the presence of preoperative embolic stroke, congestive heart failure or uncontrolled bacteremia alone, or in combination.

Aortic Root Replacement in Octogenarians Offers Acceptable Perioperative and Late Outcomes
Conclusions Octogenarians can safely undergo aortic root replacement with moderately worse but acceptable perioperative mortality and late survival. Further studies are necessary to determine which subset of octogenarians are at the highest operative risk and may benefit from a conservative approach.

Utility of Brain MRI in Infective Endocarditis (S46.004)
CONCLUSIONS: In hospitalized patients with IE, ICH was associated with unfavorable clinical outcomes. However, in patients undergoing valve surgery, microbleeds on MRI were not associated with development of postoperative ICH. Preoperative brain MRI may result in a delay to surgery and did not have an impact on outcome. Assessment of clinical severity in patients with infective endocarditis with cardio-embolic strokes
We sincerely thank Biteker et al. for their interest in our study to suggest a possible association of stroke severity with clinical outcomes in patients with infective endocarditis (IE) with cerebral complications regardless of whether early surgery or conventional treatment. They recommended us to use National Institutes of Health Stroke Scale (NIHSS) score to evaluate this issue as recently reported by Murai et al. [1], while a necessity of patient's answer to several inquiries for a measurement of NIHSS score had imposed major limitations on analyzing our data retrospectively.

A systematic review and meta-analysis of mechanical vs biological composite aortic root replacement, early and 1-year results

Determining Which Prosthetic to Use During Aortic Valve Replacement in Patients Aged Younger than 70 Years: A Systematic Review of the Literature.
CONCLUSIONS: Published literature does not preclude the use of bioprostheses for AVR in younger patients. As new valves are developed, the use of bioprosthetic aortic valves in younger patients will likely continue to expand. Clinical trials are needed to provide surgeons with more accurate guidelines.
