A rapid evidence assessment of bleed-related healthcare resource utilization in publications reporting the use of direct oral anticoagulants for non-valvular atrial fibrillation.
CONCLUSIONS: The limited available evidence for HRU burden among patients receiving oral anticoagulation for NVAF suggests that DOACs (particularly apixaban and dabigatran) offer some degree of benefit in terms of HRU outcomes, compared with warfarin. Further work is required to understand HRU outcomes in patients receiving DOACs. PMID: 30380959 [PubMed - as supplied by publisher]

Oral anticoagulation therapy and subsequent risk of venous thromboembolism in atrial fibrillation patients.
CONCLUSIONS: In this large practice-based population of AF patients prescribed OACs for primary prevention of stroke and systemic embolization, subsequent risk of VTE was lowest among those prescribed apixaban and dabigatran, while risk was similar with prescriptions for warfarin and rivaroxaban. Among AF patients prescribed OACs, lowering risk of VTE may be an additional benefit of apixaban and dabigatran, beyond the reduced bleeding risk observed in randomized clinical trials. Modeling the impact of real-world adherence to once-daily (QD) versus twice-daily (BID) non-vitamin K antagonist oral anticoagulants on stroke and major bleeding events among non-valvular atrial fibrillation patients.
CONCLUSION: QD NOACs prevented a significant number of strokes and caused no significant increase in MBs compared to BID NOACs which leads to significant net cost savings for NVAF patients in the US. PMID: 30265159 [PubMed - as supplied by publisher]

Mechanical Thrombectomy of Large Artery Occlusion Is Beneficial in Octogenarians
Conclusion: Despite a poor recovery rate, octogenarians benefitted from EVT for AIS, with a NNT comparable to that of younger patients treated with intravenous r-tPA.

Patient-reported treatment satisfaction with rivaroxaban in Japanese non-valvular atrial fibrillation patients: an observational study.
CONCLUSIONS: Findings suggest treatment satisfaction may improve in Japanese NVAF patients after a switch from warfarin to rivaroxaban. Higher treatment satisfaction may translate into improved treatment adherence, which is critical for the long-term prevention of stroke. PMID: 30067119 [PubMed - as supplied by publisher]

Cerebral Circulation in Men and Women.
Authors: Memon A, McCullough LD Abstract Despite several advancements in stroke care, disparities continue to exist with regard to sex differences in cerebrovascular disease. These sex differences are due to a combination of several factors, many of which are unique to the female sex. Some of these unique factors, such as pregnancy and menopause, are related to hormonal changes seen throughout the female life cycle. Hormonal fluctuations, which impact the protective effects of the female sex hormones, can be induced by the use of hormonal contraception. Other risk factors, although present in both sexes, have a hig...

Influence of age on the pharmacokinetics, pharmacodynamics, efficacy, and safety of rivaroxaban.
CONCLUSION: A favorable clinical profile with rivaroxaban was observed across age subgroups, supporting the premise that dosing in older adults does not necessitate adjustment. However, it is prudent that a cautious and individualized approach is taken for treatment with any anticoagulant in older adults. PMID: 29932775 [PubMed - as supplied by publisher]

Aortic Valve Predilatation with a Small Balloon, without Rapid Pacing, prior to Transfemoral Transcatheter Aortic Valve Replacement.
Conclusion: In TAVR, AVPD with a small, nonocclusive balloon can be safely performed. By avoiding rapid pacing, this technique may be a valid alternative to traditional BAV. Whether or not the use of APVD without rapid pacing translates into less periprocedural complications needs to be assessed in future studies. PMID: 29854723 [PubMed - in process]

Safety and effectiveness of apixaban in comparison to warfarin in patients with nonvalvular atrial fibrillation: a propensity-matched analysis from Japanese administrative claims data.
CONCLUSIONS: Our observational data from clinical practice broadly confirm the safety and efficacy results of pivotal randomized controlled trials of apixaban for stroke prevention among NVAF patients. PMID: 29772946 [PubMed - as supplied by publisher]

Relationship of Troponin to Incident Atrial Fibrillation Occurrence, Recurrence after Radiofrequency Ablation and Prognosis: A Systematic Review, Meta-Analysis and Meta-Regression.
Conclusions In our systematic review, meta-analysis and meta-regression, TnT was associated with both incident AF occurrence and AF recurrence after RFA, as well as stroke/SE and major bleeding. In contrast, TnI was not associated with incident AF occurrence, AF recurrence after RFA or prognosis (stroke/SE, major bleeding). PMID: 29631448 [PubMed - as supplied by publisher]

Epicardial adipose tissue and atrial fibrillation: pathophysiological mechanisms, clinical implications and potential therapies.
Authors: Goudis CA, Vasileiadis IE, Liu T Abstract Atrial fibrillation (AF) is the most common arrhythmia in clinical practice and is associated with increased cardiovascular morbidity and mortality. Epicardial adipose tissue (EAT) serves as a biologically active organ with important endocrine and inflammatory function. An accumulating body of evidence suggests that EAT is associated with the initiation, perpetuation and recurrence of AF, but the precise role of EAT in AF pathogenesis is not completely elucidated. Pathophysiological mechanisms involve adipocyte infiltration, profibrotic and pro-inflammatory paracri...

Risk Factors, Clinical Features and Prognosis for Subtypes of Ischemic Stroke in a Chinese Population
SummaryIt is unanimously accepted that stroke is a highly heterogeneous disorder. Different subtypes of ischemic stroke may have different risk factors, clinical features, and prognoses. The aim of this study was to evaluate the risk factors, clinical characteristics, and prognoses of different subtypes of ischemic stroke defined by the Trial of ORG10172 in Acute Stroke Treatment (TOAST) criteria. We prospectively analyzed the data from 530 consecutive patients who were admitted to our hospital with acute ischemic stroke within 7 days of stroke onset during the study period. Standardized data assessment was used and the ca...
