Renovascular hypertension: A report of 21 cases.
Abstract Renovascular hypertension (RVH) is among the most common forms of secondary hypertension. We studied 21 patients (eight male and 13 female) with RVH who were encountered during a period of 16 years. The average age of the patients was 34.75 years. Trans-renal Doppler ultrasound confirmed the diagnosis of renovascular stenosis (RAS) as the cause of RVH in 95% of the patients. Computed tomography angiography was performed in ten cases, which confirmed the diagnosis in all these cases. Magnetic resonance angiography was performed in only three patients. Renal scintigraphy was performed in 11 patients, with D...

Pheochromocytoma crisis resulting in acute heart failure and cardioembolic stroke in a 37-year-old man
A previously healthy, 37-year-old man presented to his primary care physician with new-onset hypertension, cough, and dyspnea. He was initially diagnosed with asthma and was treated with inhalation corticosteroids. Despite treatment, his symptoms worsened over a 3-month period. Although previously athletic, he developed exercise intolerance, orthopnea, and paroxysmal nocturnal dyspnea. His symptoms prompted admission to a community hospital to rule out pulmonary embolism. Contrast computed tomography of the chest instead demonstrated a 5.3 × 4.8-cm left adrenal tumor ().

Hemorheology of Clinically Silent Multifocal Vascular Cerebral Lesions (P1.114)
CONCLUSIONS: We may suggest that a better red cell elasticity in patients results from a self-regulatory compensation. IgM molecules seem to play an opposite role to fibrinogen molecules in red cells aggregation phenomenon.Study Supported by: no supportedDisclosure: Dr. Kowal has nothing to disclose. Intracranial Dural Arteriovenous Fistula Presenting with Recurrent Transient Fluent Aphasia (P2.097)
Conclusion: This case illustrates that a vascular malformation can present with recurrent transient focal deficits. A well-defined hypodensity with increased vascularity is an uncommon early finding in acute ischemic stroke. In the setting of recurrent neurological deficits, these findings should prompt a thorough neurovascular evaluation.Disclosure: Dr. Nourbakhsh has nothing to disclose. Morphological, Hemodynamic, and Clinical Independent Risk Factors for Anterior Communicating Artery Aneurysms Clinical Sciences
Conclusions— The risk of ACoA aneurysm formation is determined by several independent clinical, morphological, and hemodynamic factors. The strongest independent risk factors include smoking, asymmetry of A1 segments >40%, low blood flow pulsatility, and the angle between A1 and A2 segments ≤100°.

Unfavorable outcome of aggressive lowering of high blood pressure. Case report.
Abstract Cerebral autoregulation is essential in the maintenance of cerebral blood flow. Due to this autoregulation, cerebral perfusion is constant in healthy subjects if blood pressure values are between 50-150 mmHg. In hypertensive patients the curve is right-shifted towards higher blood pressure values (pathological autoregulation). Aggressive blood pressure reduction can lead to severe ischaemia. The authors report the history of a 73-year-old man with the background history of widespread atherosclerotic disease. The patient complained about headache and dizziness and was found to have high blood pressure (160...

Pneumonia: When Antibiotics Are Not the Answer
We present the case of an 88-year-old bedridden man, with hypertension, multiple myeloma, coronary artery disease, ischemic stroke, seizures, dysphagia, and constipation under treatment with mineral oil. He was brought to the emergency department after 3 days of cough, dysphagia, and dyspnea, following a recent hospitalization, due to a health care–associated pneumonia. A chest computed tomography scan obtained showed bilateral multifocal opacities with consolidates described as Hounsfield units of −62, consistent with fat. A diagnosis of lipoid pneumonia was done, and mineral oil was discontinued. Given the patient’...

A case of PRES in an active lupus nephritis patient after treatment of corticosteroid and cyclophosphamide
Abstract Posterior reversible encephalopathy syndrome (PRES) is primarily a radiological diagnosis. The syndrome is characterized by headache, altered mental status, seizures, and bilateral posterior white matter edema in a nonvascular distribution on neuroimaging with resolution of findings usually in 7–14 days (Casey et al. in AJNR Am J Neuroradiol 21:1199–1206, 2000). In most cases, computed tomography of the brain will show hypodense lesions in the parieto-occipital lobe. Although this syndrome is uncommon, prompt and accurate recognition allows early treatment, which has been shown to produce favorable o...

Quantitative computed tomography of pulmonary emphysema and ventricular function in chronic obstructive pulmonary disease patients with pulmonary hypertension.
CONCLUSION: The study revealed that percent emphysema is correlated with RV dysfunction among COPD patients with PH. Based on our findings, percent emphysema can be considered for use as an indicator to predict the severity of right ventricular dysfunction among COPD patients. PMID: 25469102 [PubMed - in process]

Effectiveness of Diagnostic Strategies in Suspected Delayed Cerebral Ischemia: A Decision Analysis Clinical Sciences
Conclusions— Imaging studies to test for the presence of angiographic vasospasm or perfusion deficits in patients with clinical DCI do not seem helpful in selecting which patients should undergo treatment and may not improve outcomes. Future directions include validating these results in prospective cohort studies.

Bilateral ascending weakness in a patient with Azygous Anterior Cerebral Artery Stroke. (P1.040)
CONCLUSION: Our patient’s clinical presentation was challenging because ascending pattern of weakness is usually seen in Acute Inflammatory Demyleinating Neuropathy or spinal cord injury. During the early stages deep tendon reflexes may be perserved in many conditions but may change on serial clinical examination. Hyperreflexia noted on follow-up examination and the absence of sensory involvemnt were the key feature suggesting a cerebral insult. Our study calls to attention to this rare vascular anomaly and its misleading clinical presentation.Disclosure: Dr. Melkumova has nothing to disclose. Dr. Noroozi has nothing...

Hemodynamic Significance of Internal Carotid or Middle Cerebral Artery Stenosis Detected on Magnetic Resonance Angiography.
CONCLUSION: A considerable proportion of ICA-MCA stenosis detected on MRA does not cause CVR impairment despite a fair correlation between them. Thus, hemodynamic state needs to be assessed for evaluating significance of stenosis, particularly in asymptomatic patients. PMID: 26446655 [PubMed - in process]
