Semi-quantitative assessment of pulmonary arterial hypertension associated with congenital heart disease through myocardial perfusion imaging.
CONCLUSION: Semi-quantitative MPI has high diagnostic value in evaluating the severity level of pulmonary arterial hypertension in patients with congenital heart disease. The diagnostic performance of MPI at pre-treatment patients was superior to that at post-treatment. More cases need to be included for further study. Left ventricular strain and dyssynchrony by CMR Feature tracking in Idiopathic Pulmonary Arterial Hypertension(IPAH)
Conclusion: Impaired LV longitudinal strain and intraventricular dyssynchrony appear in IPAH. In the setting of impaired longitudinal function, LVEF is maintained by circumferential contraction. In IPAH with impaired LVEF, circumferential strain is impaired in addition to longitudinal strain.

Effect of riociguat on pulmonary arterial compliance in patients with pulmonary arterial hypertension (PAH) in the RESPITE study
Conclusion: In RESPITE, riociguat improved PAC in patients with PAH who had transitioned from PDE5i. The increase in PAC was more pronounced in patients whose exercise and functional capacity had also improved by Week 24.

Altered synchrony of right ventricular contraction inborderline pulmonary hypertension
Conclusion: RV dyssynchrony occurs in borderline PH and may reflect early RV arterial uncoupling.

The effect of 6-mercaptopurine treatment on experimentally induced pulmonary arterial hypertension
Conclusion: 6-MP partly reversed RV systolic pressure and RV afterload in established PAH. Together with the observed improved RV remodeling and cardiac function, 6-MP may be a promising intervention for PAH.

What is the role of screening for pulmonary hypertension in adults and children with sickle cell disease?
Authors: Willen SM, Gladwin MT Abstract Patient case: An 18-year-old male patient with homozygous hemoglobin SS disease was evaluated for progressive dyspnea and elevated tricuspid regurgitant jet velocity (TRV) on echocardiography. The patient's case is described in detail in Lancet1 He had been treated with regular transfusions since childhood for stroke, had rare episodes of vaso-occlusive pain episodes, and did not take narcotic pain medications. He presented with progressive severe dyspnea on exertion and lower extremity edema. His laboratory tests were notable for a total hemoglobin level of 11.8 g/dL and hem...

Late Transient Contrast-Induced Encephalopathy after Percutaneous Coronary Intervention
Cerebrovascular events are a recognized complication of cardiac catheterization, with various neurological symptoms. The incidence is estimated to be .3%-.4%.1 A much more infrequent complication is a contrast-induced encephalopathy (CIE) described about 50 cases in the literature. The pathogenesis is believed to be a direct neurotoxic effect causing an osmotic disruption of the blood –brain barrier.2,3 Risk factors are hypertension, diabetes mellitus, renal impairment, large volumes of contrast, and previous adverse reaction to iodinated contrast.

Cardiac adaptation to hypertension in adult female Dahl salt ‐sensitive rats is dependent on ovarian function, but loss of ovarian function does not predict early maladaptation
Abstract Aim of study was to examine experimentally the adult female hypertensive heart in order to determine the role of ovary function in the response of the heart to salt‐dependent hypertension. Dahl salt‐sensitive rats, age 12 weeks, with/without ovariectomy were fed a standard (0.3% NaCl) or high‐salt diet (8%) for 16 weeks. Mean arterial blood pressure monitored noninvasively in conscious state increased significantly by high salt. Echocardiography was performed at baseline and endpoint. Heart function and molecular changes were evaluated at endpoint by left ventricle catheterization, by sirius red staining f...

Characteristics of Ventricular Function in Pulmonary Hypertension Patients
with Different Shape of Interventricular Septum:
Preliminary Study with Cardiac Magnetic Resonance Imaging
Background and objective To study the characteristics of ventricular function in Pulmonary Hypertension (PH) Patients with different shape of Interventricular Septum (IVS) by cardiac magnetic resonance (CMR). Methods 36 PH patients diagnosed by right heart catheterization accepted CMR. According to the morphology of IVS, the patients were divided into two groups: the non-deformation group (10 patients) and the deformation group (26 patients). The ventricular function parameters were as follows: RV and LV end-diastolic volume index (EDVI), end-systolic volume index (ESVI), stroke volume index (SVI), cardiac index (CI), ejec...

MRI-derived Regional Biventricular Function in Patients with Chronic Thromboembolic Pulmonary Hypertension Before and After Pulmonary Endarterectomy
Conclusion While biventricular resynchronization and recovery of global predominantly RV function was observed, regional circumferential function mainly improved in the lateral and inferior walls of both ventricles and regional radial function in the RV wall and septum 12 days after PEA, suggesting fibers primarily affected by myocardial stress in patients with CTEPH possibly need a relatively longer recovery time.

Effects of Riociguat on Right Ventricular Remodeling in Chronic Thromboembolic Pulmonary Hypertension Patients: A Prospective Study
Conclusions Riociguat treatment was associated with increased RVSVI and trends for improvement in myocardial remodeling in CTEPH patients. A larger clinical study is warranted to observe the therapeutic benefits of riociguat on RV remodeling. Teaser Riociguat is an approved medical treatment for chronic thromboembolic pulmonary hypertension (CTEPH) patients. We studied the effects of riociguat treatment on right ventricular (RV) remodeling by means of magnetic resonance imaging (MRI) and positron emission tomography (PET). After 6 months of riociguat treatment in 6 CTEPH patients, RV function increased, fibrosis decreased ...

Early Pulmonary Vascular Disease in Young Adults Born Preterm.
CONCLUSION: Young adults born preterm demonstrate early pulmonary vascular disease, characterized by elevated pulmonary pressures, a stiffer pulmonary vascular bed, and right ventricular dysfunction, consistent with an increased risk of developing pulmonary hypertension. Clinical and hemodynamic correlates of pulmonary arterial stiffness in incident, untreated patients with idiopathic pulmonary arterial hypertension.
CONCLUSIONS: In incident idiopathic PAH, PA stiffness was related to mPAP and heart rate, and this outperformed the potential influences of age and gender. Baseline PA stiffness did not independently predict outcome. The great dispersion of RC-time implied that PVR and PA stiffness were differently affected by the disease process.
