When it all comes down to pressure: right ventricular ejection fraction at cardiac catheterisation
In the present issue of the European Respiratory Journal, Heerdt et al. [1] report on the validation of right ventricularejection fraction (RVEF) calculated from a pressure curve at cardiac catheterisation. This is of great interest as right ventricular (RV) function is the main determinant of symptomatology and outcome in pulmonary hypertension [2], and RVEF accordingly adds significantly to risk stratification in these patients [3]. However, ejection fraction (EF) is a ratio of stroke volume (SV) to end-diastolic volume (EDV), with no pressure in the equation. So, how is it possible to calculate a volume ratio from pressure measurements?

Pulmonary vascular and right ventricular burden during exercise in interstitial lung disease.
CONCLUSIONS: ILD with resting and exercise PH is associated with increased exercise RV work, reduced pulmonary vascular reserve and reduced peak VO2. The findings highlight the role of pulmonary vascular and RV burden to ILD exercise limit. PMID: 32173491 [PubMed - as supplied by publisher]

Risk assessment in PAH using quantitative CMR tricuspid regurgitation: relation to heart catheterization
ConclusionsA clear trend towards worse outcome with larger TRV or TR% was shown; however, the number of events was insufficient for significant outcome differences. Prognostic value of quantitative TR should be investigated in a larger multicentre cohort. Effective RV ejection fraction may be considered an improved measure of RV function in PAH.

Elevated plasma endocan and BOC in heart failure patients decrease after heart transplantation in association with improved hemodynamics
ConclusionOur results suggest that (i) plasma endocan in HF may reflect the state of pulmonary vascular congestion and PH-LHD, whereas (ii) plasma BOC may reflect the cardiac function and the hemodynamic overload  in HF. The exact role of these proteins and their clinical applicability as biomarkers in HF and PH-LHD ought to be investigated in larger cohorts.

High Right Ventricular Afterload Is Associated with Impaired Exercise Tolerance in Patients with Left Ventricular Assist Devices

Comparison of levosimendan, NO, and inhaled iloprost for pulmonary hypertension reversibility assessment in heart transplant candidates
ConclusionsLevosimendan may be a safe and effective alternative for pulmonary hypertension reversibility assessment or a valuable pre ‐test medical optimization tool in end‐stage heart failure patient assessment for heart transplantation offering extended haemodynamic benefits. Whether it increases the rate of positive responses or allows a better selection of candidates to heart transplantation remains to be established.
