Effect of Smoking and Folate Levels on the Efficacy of Folic Acid Therapy in Prevention of Stroke in Hypertensive Men Clinical Sciences
Conclusions—Baseline folate levels and smoking status can interactively affect the risk of first stroke. Our data suggest that compared with never smokers, ever smokers may require a higher dosage of folic acid to achieve a greater beneficial effect on stroke. Rapid transitions in the epidemiology of stroke and its risk factors in China from 2002 to 2013
Conclusions: In 2002–2013, the incidence of stroke in China increased rapidly. Combined with a high prevalence, a trend toward a younger age, and stable mortality, this finding suggests that additional clinical and behavioral interventions for metabolic and lifestyle risk factors are necessary to prevent stroke, particularly in certain populations.

Association between percent decline in serum total homocysteine and risk of first stroke
Conclusions: Percent lowering in tHcy was significantly associated with a reduction in first stroke risk in Chinese adults with hypertension, and if further confirmed, may serve as a useful indicator for folic acid treatment efficacy on stroke prevention. Clinicaltrials.gov identifier: NCT00794885.

Risk factors and pathogenic microorganism characteristics for pneumonia in convalescent patients with stroke: A retrospective study of 380 patients from a rehabilitation hospital
Stroke significantly impacts public health and ranks among the leading causes of death and disabilities, resulting in enormous costs measured in both health care resources and lost productivity. It results primarily from embolus or thrombosis for ischemic stroke and hypertension for hemorrhagic stroke, respectively [1]. In 2016, there were 5.5 million deaths and 116.4 million disability-adjusted life-years (DALYs) owed to stroke [2]. As reported, the impact of stroke on the Chinese population is more severe compared to average global levels, and the prevalence of stroke continues to surpass that of ischemic heart disease [3 –4].

We read the study conducted by Huangfu et  al that was published in the Journal of Stroke& Cerebrovascular Diseases enthusiastically and meticulously.1 The authors tried to examine the combined effect of smoking and hypertension on ischemic stroke incidence over a 10-year follow-up among Inner Mongolians in China. The article provides interesting results; however, we think some methodological issues should be considered to avoid misinterpretation.

According to relevant epidemiological data, the prevalence of hypertension in the elderly in China is as high as 40 –60%.1 Hypertensive hemorrhagic stroke is one of the most serious complications of hypertension, and it usually occurs in the elderly. The main reason is that the wall of small arteries in the brain of hypertensive patients has glassy or fibrous degeneration, which reduces the elasticity of the bl ood vessel wall and causes local vasodilation.
