Investigating Real-World Clopidogrel Pharmacogenetics in Stroke Using a Bioresource Linked to Electronic Medical Records.
In conclusion, we have demonstrated the clinical impact of CYP2C19*2 on clopidogrel efficacy using a purely EMR approach. This suggests that the risk in the ischemic stroke population may be particularly high. This article is protected by copyright. All rights reserved. PMID: 28653333 [PubMed - as supplied by publisher]

Ambient Temperature and Stillbirth: A Multi-Center Retrospective Cohort Study
Conclusions: Extremes of local ambient temperature may have chronic and acute effects on stillbirth risk, even in temperate zones. Temperature-related effects on pregnancy outcomes merit additional investigation. https://doi.org/10.1289/EHP945 Received: 09 August 2016 Revised: 06 December 2016 Accepted: 22 December 2016 Published: 22 June 2017 Address correspondence to P. Individual and Joint Effects of Early-Life Ambient PM2.5 Exposure and Maternal Prepregnancy Obesity on Childhood Overweight or Obesity
Conclusions: In the present study, we observed that early life exposure to PM2.5 may play an important role in the early life origins of COWO and may increase the risk of COWO in children of mothers who were overweight or obese before pregnancy beyond the risk that can be attributed to MPBMI alone. Our findings emphasize the clinical and public health policy relevance of early life PM2.5 exposure. https://doi.org/10.1289/EHP261 Received: 29 March 2016 Revised: 08 August 2016 Accepted: 23 August 2016 Published: 14 June 2017 Address correspondence to X. Hypoglycaemia seriousness and weight gain as determinants of cardiovascular disease outcomes among sulfonylurea users
ConclusionsThis study provides evidence of increased CVD risk associated with hypoglycaemia, especially serious hypoglycaemia events. While associations were attenuated with non‐serious hypoglycaemia, the results were suggestive of a potential increased risk.

CHA2DS2-VASc Scores and Major Bleeding in Patients With Nonvalvular Atrial Fibrillation Who Are Receiving Rivaroxaban
Conclusion Higher CHA2DS2-VASc scores are associated with increased incidence of major bleeding in nonvalvular atrial fibrillation patients receiving rivaroxaban.

Economic evaluation of ezetimibe treatment in combination with statin therapy in the United States.
CONCLUSIONS: Compared with statin monotherapy, ezetimibe with statin therapy was cost-effective for secondary prevention of CHD and stroke and for primary prevention of these conditions in patients whose LDL-C level is ≥ 100 mg/dL and in patients with diabetes, taking into account a 90% cost reduction for ezetimibe. Impact of Neurologist In-Hospital 24/7 on stroke patients arriving to the Emergency Department activated as a Code Stroke (S8.003)
Conclusions:In-hospital 24/7 coverage for acute stroke treatment reduces door to alteplase treatment times significantly. For each minute reduction in a neurologist at the bedside for CS, a 2 minute and 18 second improvement in door to alteplase treatment time occurs. An associated reduction in mortality is also demonstrated. A limitation of this study includes a smaller number of post-implementation data. These data will need on-going monitoring to ensure enduring impact.Disclosure: Dr. Schneider has nothing to disclose. Age, Gender and Racial Disparities in Acute Stroke Therapy in a Large Non-Academic Health System (P3.261)
Conclusions:We confirmed known disparities in ATT rates between PSC and AH. No disparity in age or gender were identified in this population. Racial disparity was problematic because of a small number of non-white patients and a number of patients with no racial data captured. Better racial data collection, analysis of access to care, and care process modeling might impact these findings.Disclosure: Dr. Taylor has nothing to disclose. Abstract 150: Machine Learning Methodology Predicts Comorbidities are Associated With Increased Total Healthcare Costs Among Patients With Severe Peripheral Artery Disease Session Title: Poster Session II
Conclusion: In this study, the presence of chronic ulcers in the lower extremities and CKD were two factors most predictive of increased all-cause total HC in a geographically diverse population of severe PAD patients.

7 medtech stories we missed this week: Feb. 24, 2017
[Photo from unsplash.com]Several companies made distribution deals this week, while others received approval for their products. Here are 7 medtech stories we missed this week but were still worth mentioning. 1. Israel OKs E-Qure’s BST device for chronic wounds E-Qure Corp. announced this week that it received marketing approval for its Bio-electrical Signal Therapy Device (BST Device) from the Israeli Ministry of Health, according to a Feb. 24 news release. BST is an electrotherapeutic method for treating chronic wounds. E-Qure will begin marketing its device in Israel by activating a signed distribution agreement with...

Adult simulation and demonstration of nurse competency with neurological assessment
Nurses work in challenging environments and vast amounts of information are shared on a continual basis. Changes in health care and bedside nursing are constant which make maintaining competency in this dynamic environment challenging. In addition, as documentation has transitioned to electronic medical records, new challenges have been created. The purpose of this paper is to describe an educational intervention using a live standardized patient (actor) simulation with staff nurses that care for patients after a stroke or carotid intervention.

First Diagnosis of Atrial Fibrillation at the Time of Stroke
Conclusions: In this contemporary cohort, nearly one in 5 AF-related strokes occurred without a pre-stroke AF diagnosis. AF was readily diagnosed using standard rhythm monitoring. The vast majority of patients with newly diagnosed AF were at high enough pre-stroke risk to merit anticoagulation. In conclusion, our findings support screening for AF before stroke. Patients with past transient AF may merit more intensive screening.Cerebrovasc Dis 2017;43:192-199

Identification of unique venous thromboembolism-susceptibility variants in African-Americans.
Abstract To identify novel single nucleotide polymorphisms (SNPs) associated with venous thromboembolism (VTE) in African-Americans (AAs), we performed a genome-wide association study (GWAS) of VTE in AAs using the Electronic Medical Records and Genomics (eMERGE) Network, comprised of seven sites each with DNA biobanks (total ~39,200 unique DNA samples) with genome-wide SNP data (imputed to 1000 Genomes Project cosmopolitan reference panel) and linked to electronic health records (EHRs). Using a validated EHR-driven phenotype extraction algorithm, we identified VTE cases and controls and tested for an association ...

Safety and Effectiveness of Statins for Prevention of Recurrent Myocardial Infarction in 12 156 Typical Older Patients: A Quasi-Experimental Study
Conclusions: Estimates of statin effectiveness for the prevention of recurrent myocardial infarction in patients aged 60–79 years were similar to trial results, but more evidence is needed in the older group. There may be an excess of falls and fractures in very old patients, which deserves further investigation.
