CT angiography vs echocardiography for detection of cardiac thrombi in ischemic stroke: a systematic review and meta-analysis
ConclusionsCTA may be a promising alternative to echocardiography for detection of cardiac thrombi in patients with ischemic stroke, especially now that CTA is standard care for patient selection for endovascular treatment. However, studies were too heterogeneous and of insufficient methodological quality to draw firm conclusions. Large, prospective studies on this topic are warranted.

Neurological update: dizziness
AbstractThe diagnosis and management of vertigo remains a challenge for clinicians, including general neurology. In recent years there have been advances in the understanding of established vestibular  syndromes, and the development of treatments for existing vestibular diagnoses. In this ‘update’ I will review how our understanding of previously “unexplained” dizziness in the elderly is changing, explore novel insights into the pathophysiology of vestibular migraine, and its relationshi p to the newly coined term ‘persistent postural perceptual dizziness’, and finally discuss how a simple bedside oculomotor a...

Wake-up stroke: thrombolysis reduces ischemic lesion volume and neurological deficit
ConclusionsrTPA in WUS patients selected with CT and/or CTP resulted in reduced ischemic infarct volume on follow-up CT and better functional outcome without increment of intracranial hemorrhages and in-hospital mortality.

Underlying embolic and pathologic differentiation by cerebral microbleeds in cryptogenic stroke
ConclusionsCMBs were common, and age, male sex, hypertension, chronic kidney disease, and cerebral white matter diseases were related to CMBs in cryptogenic stroke. Aortic complicated lesions were associated with deep and diffuse CMBs, while prior anticoagulant therapy was related to lobar CMBs.

Elevated blood glucose is associated with aggravated brain edema in acute stroke
ConclusionElevated admission BGL were associated with increased early brain edema and poor clinical outcome mediated by collateral status. Patients with higher BGL might be targeted by adjuvant anti-edematous treatment.

Efficacy and safety of mechanical thrombectomy for M2 segment of middle cerebral artery: a systematic review and meta-analysis
ConclusionMT for M2 segment benefit patients with AIS on 3-month functional independence compared with that of M1 segment or medical care, without increasing the risk of sICH.

The outcome of acute functional neurological disorder: a meta-analysis of stroke-mimic presentations
ConclusionPatients presenting with acute motor FND have good outcome of their presenting symptoms by discharge and at follow-up, following thrombolysis for presumed stroke, in contrast to specialist cohorts. Demographic differences, in addition to the thrombolysis, are possible explanations for these more favourable results.

Single mean arterial blood pressure drops during stroke thrombectomy under general anaesthesia are associated with poor outcome
ConclusionsFor patients undergoing MT under GA, single MAP drops  <  60 mmHg are independently related to unfavourable 3-month outcome. Therefore, every effort should be made to prevent periinterventional hypotensive episodes, especially below this threshold.

Premature vascular disease in young adult stroke: a pathology-based case series
ConclusionYoung adult stroke patients with recurrent stroke, fatal stroke, or SVD on imaging have advanced atherosclerosis and arteriolosclerosis-related pathological changes in multiple organ systems. Aggressive control of atherosclerosis risk factors is warranted even in young individuals.

Optimization of risk stratification for anticoagulation-associated intracerebral hemorrhage: net risk estimation
ConclusionsIn this anticoagulation-associated ICH cohort, baseline hemorrhagic risk exceeded ischemic risk in approximately 50%, highlighting the importance of careful consideration of risk/benefit ratio prior to anticoagulation decisions. The remaining 50% suffered an ICH despite excess baseline ischemic risk, stressing the need for biomarkers to allow more precise estimation of hemorrhagic complication risk.

Long-term outcome changes after mechanical thrombectomy for anterior circulation acute ischemic stroke
ConclusionsA significant number of patients with MT experience a favorable long-term outcome. Outcomes remained stable between short- and long-term follow-up, but some individuals may still show improvement beyond short-term rehabilitation. Right hemispheric stroke and clinical stroke severity at hospital discharge may be frail predictors for delayed decline of functional status, whereas successful recanalization remains a positive outcome predictor. Death rarely occurs beyond 3  months after MT treatment.

Post-stroke medication adherence and persistence rates: a meta-analysis of observational studies
ConclusionsMedication adherence and persistence rates are low in patients after suffering a stroke. Patient medication adherence or persistence and their influencing factors should be considered for the treatment of stroke patients. More detailed disease prevention and management strategies need to be developed for stroke patients with different comorbidities.

Detailed phenotyping of posterior vs. anterior circulation ischemic stroke: a multi-center MRI study
ConclusionIschemic stroke subtypes differ between the two phenotypes. Diabetes and male sex have a stronger association with PCiS than ACiS. Definitive MRI-based PCiS diagnosis aids etiological investigation and contributes additional insights into specific risk factors and mechanisms of injury in PCiS.

White matter burden does not influence the outcome of mechanical thrombectomy
ConclusionsWMHs burden does not seem to influence clinical outcome and imaging parameters in patients treated by MT.
