White Matter Disease and Outcomes of Mechanical Thrombectomy for Acute Ischemic Stroke INTERVENTIONAL
CONCLUSIONS: Moderate-to-severe white matter disease is associated with worse outcomes in patients undergoing endovascular treatment without a significant increase in hemorrhagic complications. Studies comparing patients with and without endovascular treatment are necessary to determine whether the benefit of endovascular treatment is attenuated with greater white matter disease.

Is Histologic Thrombus Composition in Acute Stroke Linked to Stroke Etiology or to Interventional Parameters? INTERVENTIONAL
CONCLUSIONS: We interpreted the higher macrophage and platelet content in cardioembolic thrombi compared with large-artery atherosclerotic stenosis or embolic stroke of undetermined source thrombi as an indication that the latter type might be derived from an atherosclerotic plaque rather than from an undetermined cardiac source. The extent of thrombus fragmentation was associated with a more challenging mechanical thrombectomy and a higher lymphocyte content of the thrombi. Thus, thrombus fragmentation not only might be caused by the recanalization procedure but also might be a feature of a lymphocyte-rich, difficult-to-r...

Grade I traumatic aortic injury progression
This case highlights the importance of diagnosing Grade I aortic injuries after trauma. Although the prognosis for minimal aortic injury is typically quite good, the patient in this case suffered the unlikely complication of minimal intimal injury progression with resultant thromboembolism and massive stroke. Due to motion artifact and CT resolution limitations, the initial aortic injury was not detected. Thus, potentially life-saving measures such as serial CT monitoring of injury, blood pressure control, and endovascular repair were unable to be performed.

Computed Tomography Angiogram: Diagnosing Device Placement Failure
In patients with non-valvular atrial fibrillation (AF) and increased stroke risk, the left atrial appendage (LAA) is a primary source of thrombus [1]. Percutaneous left atrial appendage closure (LAAC), a minimally invasive procedure, is increasingly performed to reduce AF stroke risk [1]. For successful clinical outcomes, it is therefore imperative that the Watchman implant —one such LAA closure device—maintains long-term efficacy. The PROTECT-AF trial, for instance, reported peridevice leak in 32% of patients post-LAAC.

Endovascular Thrombectomy of Calcified Emboli in Acute Ischemic Stroke: A Multicenter Study INTERVENTIONAL
CONCLUSIONS: Acute ischemic stroke with large-vessel occlusion due to calcified emboli is a rare entity in patients undergoing thrombectomy, with considerably worse angiographic outcome and a higher mortality compared with patients with noncalcified thrombi. Good functional recovery at 3 months can still be achieved in about a quarter of patients.

Peri-thrombus vascular hyperintensity sign: detection of intracranial thrombus location and length in acute ischemic stroke
ConclusionThe PVHS on 3D BB contrast-enhanced MRI is a highly specific tool for evaluating the location and length of a thrombus in AIS patients.

Left atrial thrombus in patients with atrial fibrillation and under oral anticoagulant therapy; 3-D transesophageal echocardiographic study
AbstractIschemic stroke is the major complication of atrial fibrillation (AF) and only proven preventive therapy is oral anticoagulant therapy (OAC). Previous studies reported the presence of thrombus in the left atrium (LA) or left atrial appendage (LAA) despite anticoagulant therapy. We aim to investigate the predictors of LA/LAA thrombus in patients under OAC therapy and long-term clinical impact of thrombus. We prospectively enrolled consecutive patients with permanent AF under OAC therapy. Patients baseline characteristics were recorded. Transesophageal echocardiographic study performed after complete transthoracic ec...

A Pictorial Review of Intracranial Haemorrhage Revisited.
CONCLUSION: Unenhanced CT is a mainstay of acute phase imaging due to its availability and, sensitivity and specificity for detecting acute haemorrhage. Several imaging features can be identified on CT and, along with clinical information, can provide some certainty in diagnosis. For those suitable and where diagnostic uncertainty remains CT angiogram, time-resolved CT angiography and catheter angiography can help identify underlying AVMs, aneurysms, cavernomas and vasculitides. MRI is more sensitive for the detection of subacute and chronic haemorrhage and identification of underlying mass lesions. PMID: 32008532 [PubMed - in process]

Isolated superior striate vein thrombosis in adults.
CONCLUSION: Isolated superior striate vein thrombosis is a rare form of intracranial venous thrombosis and should be considered in the differential diagnosis of stroke-like episodes with headache in adults. Isolated superior striate vein thrombosis presents with characteristic imaging features on CT and MRI. PMID: 31969077 [PubMed - as supplied by publisher]

The role of diabetes mellitus on the thrombus composition in patients with acute ischemic stroke.
CONCLUSION: Clots in diabetic patients had more fibrin and fewer erythrocyte components compared with patients without diabetes mellitus, while hyperglycemia on admission did not show association with clot composition. Further studies are needed to confirm these results. PMID: 31924102 [PubMed - as supplied by publisher]

Are cerebral ischaemic complications more likely to occur in elderly patients undergoing transradial non ‐coronary intervention: A single‐centre retrospective review
ConclusionTransradial non ‐coronary interventions are safe in elderly patients, who are likely to have more atherosclerotic disease of the aorta. Of note, no complication of ischaemic strokes within 30 days of the procedure is documented in all patients.

Aortic valve calcification as a risk factor for major complications and reduced survival after transcatheter replacement
Aortic valve calcification is supposed to be a possible cause of embolic stroke or subclinical valve thrombosis after transcatheter aortic valve replacement (TAVR). We aimed to assess the role of aortic valve calcification in the occurrence of in-hospital clinical complications and survival after TAVR.
