Comorbid Psychiatric Disease Is Associated With Lower Rates of Thrombolysis in Ischemic Stroke Brief Report
Background and Purpose—Intravenous thrombolysis (IVT) improves outcomes after acute ischemic stroke but is underused in certain patient populations. Mental illness is pervasive in the United States, and patients with comorbid psychiatric disease experience inequities in treatment for a range of conditions. We aimed to determine whether comorbid psychiatric disease is associated with differences in IVT use in acute ischemic stroke. Methods—Acute ischemic stroke admissions between 2007 and 2011 were identified in the Nationwide Inpatient Sample. Psychiatric disease was defined by International Classification of Diseases, ...

Wants Talk Psychotherapy but Cannot Talk: EMDR for Post-stroke Depression with Expressive Aphasia
CONCLUSION This is the first reported case demonstrating that EMDR can be effective for depression, even in those with severe expressive aphasia. In our case, there was no reluctance to disclose information, simply a neurological inability to do so. Through preparation, patience, perseverance, and plasticity (clinician flexibility, though perhaps also neuroplasticity), the patient’s PSD gradually improved, and she was able to reinvent her life within her limitations. ACKNOWLEDGMENTS The authors gratefully acknowledge Eugene Schwartz, E.C. Hurley, and Mark Hubner for providing consultation during patient care. REFERENCES ...

A Post-hoc Study of D-Amino Acid Oxidase in Blood as an Indicator of Post-stroke Dementia
In conclusion, our data support that plasma DAO levels were increased in PSD patients and correlated with brain WMH, independent of age, gender, hypertension, and renal function. Plasma DAO levels may therefore aid in PSD diagnosis. Introduction Stroke is a risk factor for both vascular dementia and Alzheimer's disease (1, 2). Functional recovery develops over the course of 26 weeks after a stroke (3), but the survivors are often left with disabilities. In addition to the sequelae of acute neuronal damage, the 1-year post-stroke dementia (PSD) rates after first-ever and recurrent stroke are ~10 and 30...

Quantitative Clinical Proteomic Study of Autopsied Human Infarcted Brain Specimens to Elucidate the Deregulated Pathways in Ischemic Stroke Pathology.
Abstract Ischemic stroke, still lacking an effective neuroprotective therapy is the third leading cause of global mortality and morbidity. Here, we have applied an 8-plex iTRAQ-based 2D-LC-MS/MS strategy to study the commonly regulated infarct proteome from three different brain regions (putamen, thalamus and the parietal lobe) of female Japanese patients. Infarcts were compared with age-, post-mortem interval- and location-matched control specimens. The iTRAQ experiment confidently identified 1520 proteins with 0.1% false discovery rate. Bioinformatic data mining and immunochemical validation of pivotal perturbed...

Antipsychotic medications and stroke in schizophrenia: A case-crossover study
ConclusionsUse of second-generation antipsychotic drugs appeared to be associated with an increased risk of ischemic stroke in the patients studied, possibly mediated by high affinity for histamine-1 receptor blockade. Further research regarding the underlying biological mechanism and drug safety is suggested.

Heat stroke during treatment with olanzapine, trihexyphenidyl, and trazodone in a patient with schizophrenia.
CONCLUSION: Heat stroke can occur during the maintenance treatment of olanzapine, trihexyphenidyl, and trazodone for schizophrenia. Clinicians should be proactive to reduce the risk of heat stroke in psychiatric patients. PMID: 26503496 [PubMed - in process]

The prevalence and risk factors of stroke in patients with chronic schizophrenia
Conclusion: The prevalence of stroke is relatively higher in Chinese patients with chronic schizophrenia. Chronic schizophrenia patients are more likely to suffer from stroke; meanwhile, a number of risk factors were identified, including old age, female sex, smoking history, combined medication with a variety of drugs, high doses, obesity, and high blood pressure. Keywords: schizophrenia, stroke, risk, risk factors

Association of severe mental illness with stroke outcomes and process-of-care quality indicators: nationwide cohort study
CONCLUSIONS: Pre-existing SMI was associated with higher risks of mortality and further vascular events. Urgent action is needed to better understand and address the reasons for these disparities.PMID:35049490 | DOI:10.1192/bjp.2021.120

Atopic Diseases and Subsequent Ischemic Stroke Among Patients With Schizophrenia: A Nationwide Longitudinal Study
Conclusions: The combined presence of schizophrenia and atopic diseases is associated with an increased risk of ischemic stroke in later life compared with individuals without these conditions.

Incidence of ischaemic heart disease and stroke among people with psychiatric disorders: retrospective cohort study.
CONCLUSIONS: Despite declines in absolute rates of IHD and stroke, relative risks remain high in those with versus without psychiatric disorders. Cardiovascular disease monitoring and prevention approaches may need to be tailored by psychiatric disorder and cardiovascular outcome, and be targeted, for example, by age and deprivation level. Schizophrenia and risk of stroke: A meta-analysis of cohort studies
Schizophrenia is a chronic mental illness with a lifetime prevalence of 1% of the population worldwide . Researchers estimate that individuals with schizophrenia have a life expectancy 15–30years shorter than the general population . Increasing evidence shows that patients with schizophrenia have an increased prevalence of stroke risk factors including diabetes , hypertension , and hyperlipidemia . Schizophrenic patients may also have an increased risk of stroke beyond that attributable to measured cardiovascular risk factors . During the past decades, a number of cohort studies have assessed the association of schizophr...

Thought disorder in atypical schizophrenia-like psychosis after stroke: analysis of clinical cases.
We described two cases of female patients, who developed an atypical schizophrenia-like psychosis post-stroke, characterized by formal thought disorder, hallucinations, and delusion. Cerebrovascular disease affected the right temporal lobe, areas devoted to modulation of higher order cognitive processes. To date, the relation between stroke and psychosis remain unclear, maybe due to the involvement of a complex brain networks. However, the high impact of this disorder on physical, psychological and social wellbeing suggests the importance of improving specific treatments. PMID: 31138024 [PubMed - as supplied by publisher]

Suicide Following Stroke in the United States Veterans Health Administration Population
In the United States (US), suicide is a leading cause of death, and most of these suicides involve firearms, highlighting the importance of lethal means safety in suicide prevention.(1, 2) US Veterans experience a suicide rate 1.5 times higher than US civilian adults and are more likely to use firearms as the means of suicide.(3) Risk factors for suicide within this population include demographic factors such as sex, age, race, and level of education, as well as health factors such as smoking status, psychiatric conditions (e.g., posttraumatic stress disorder [PTSD], depression, bipolar and anxiety disorders, schizophrenia...
