Efficacy of nafamostat mesilate as anticoagulation during cardiopulmonary bypass for early surgery in patients with active infective endocarditis complicated by stroke.
CONCLUSION: Nafamostat mesilate, administered in conjunction with low-dose heparinization, served as an effective anticoagulant for early surgery in patients with AIE complicated by stroke, and caused no further deterioration of the cerebral lesions. PMID: 25790622 [PubMed - in process]

Risk Stratification and Stroke Prevention Therapy Care Gaps in Canadian Atrial Fibrillation Patients (From the CO-ordinated National Network to Engage physicians in the Care and Treatment of patients with Atrial Fibrillation CONNECT AF Chart Audit)
The objectives of this national chart audit (January-June 2013) of 6346 AF patients (≥18 years without a significant heart valve disorder) from 647 primary care physicians were to: 1) Describe the frequency of stroke and bleed risk assessments in patients with non-valvular atrial fibrillation (AF) by primary care physicians, including the accuracy of these assessments relative to established predictive indices; 2) outline contemporary methods of anticoagulation used; and, 3) report the time in the therapeutic range amongst patients prescribed warfarin.

Excellent Outcomes for Transcatheter Aortic Valve Replacement Within 1 Year of Opening a Low-Volume Centre and Consideration of Requirements
Conclusions Excellent outcomes can be achieved in newly initiated relatively low-volume centres, which compares favorably to previously published large series. Important considerations include appropriate team training, rigorous patient screening, use of multimodality imaging techniques, a heart team approach, constant integration of lessons learned from larger published experiences, and maintaining a recommended minimum volume of 25 cases per year.

Ventricular energetics early after surgery for chronic mitral regurgitation: repair versus replacement.
CONCLUSION: Compensation of LV geometry after correction of chronic MR preserved ventricular contractility. Furthermore, the results of MV repair were not superior to those of MV replacement with partial chordal preservation in the early postoperative period. Early in-vivo hemodynamic comparison of supra-annular aortic bioprostheses: Trifecta versus Perimount Magna Ease.

Quantification of low-gradient severe aortic stenosis using a hybrid approach combining Doppler echocardiography and thermodilution.
CONCLUSION: The hybrid method is reasonably accurate in assessing AVA in patients with low-gradient severe AS. Although the continuity equation should be used in routine clinical practice in most patients, this method could serve as an alternative when the LVOT diameter and/or velocities seem questionable. Postoperative warfarin following mitral valve repair or bioprosthetic valve replacement.

Urgent surgery compared with fibrinolytic therapy for the treatment of left-sided prosthetic heart valve thrombosis: a systematic review and meta-analysis of observational studies
Conclusion Urgent surgery was not superior to FT at restoring valve function, but substantially reduced the occurrence of thrombo-embolic events, major bleeding, and recurrent PVT. In experienced centres, urgent surgery should probably be preferred over FT for treating left-sided PVT, pending the results of randomized controlled trials.
