Propensity Score Matched Analysis of Mechanical vs. Bioprosthetic Valve Replacement in Patients With Previous Stroke.
CONCLUSIONS: In patients with previous stroke, bioprosthetic valves had a lower incidence of complications connected to major bleeding than did the mechanical valves. Survival and stroke recurrence rates, however, did not differ between the 2 groups. We recommend bioprosthetic valves for patients >60 years or who have a history of GI bleeding. Efficacy of nafamostat mesilate as anticoagulation during cardiopulmonary bypass for early surgery in patients with active infective endocarditis complicated by stroke.
CONCLUSION: Nafamostat mesilate, administered in conjunction with low-dose heparinization, served as an effective anticoagulant for early surgery in patients with AIE complicated by stroke, and caused no further deterioration of the cerebral lesions. PMID: 25790622 [PubMed - in process]

Stroke is not a treatment dilemma for early valve surgery in active infective endocarditis.

Risk Stratification and Stroke Prevention Therapy Care Gaps in Canadian Atrial Fibrillation Patients (From the CO-ordinated National Network to Engage physicians in the Care and Treatment of patients with Atrial Fibrillation CONNECT AF Chart Audit)
The objectives of this national chart audit (January-June 2013) of 6346 AF patients (≥18 years without a significant heart valve disorder) from 647 primary care physicians were to: 1) Describe the frequency of stroke and bleed risk assessments in patients with non-valvular atrial fibrillation (AF) by primary care physicians, including the accuracy of these assessments relative to established predictive indices; 2) outline contemporary methods of anticoagulation used; and, 3) report the time in the therapeutic range amongst patients prescribed warfarin.

The authors present the case of a 52-year-old patient experiencing recurrent cardioembolic stroke involving the cerebral arteries secondary to a fibroelastoma originating from the free left ventricular wall and, given its symptomatic nature, indicated for surgical excision.

Apixaban Compared with Warfarin in Patients With Atrial Fibrillation and Valvular Heart Disease: Findings From the ARISTOTLE Trial.
CONCLUSIONS: -More than a quarter of the patients in ARISTOTLE with "nonvalvular" atrial fibrillation had moderate or severe valvular heart disease. There was no evidence of a differential effect of apixaban over warfarin in reducing stroke or systemic embolism, causing less bleeding, and reducing death in patients with and without valvular heart disease.
