Non-vitamin K oral anticoagulant drugs for stroke prevention in patients with atrial fibrillation and chronic kidney disease.
Abstract Atrial fibrillation (AF) and chronic kidney disease (CKD) are disorders with increasing prevalence. The presence of CKD increases the risk of incident AF and vice versa, and the presence of AF may accelerate CKD progression. Nearly a third of patients with established CKD also have AF, whilst half of AF patients may have some degree of renal dysfunction. Both AF and CKD are associated with increased cardiovascular morbidity and mortality, including significantly increased risk of stroke or systemic embolism. Oral anticoagulant therapy (OAC), either with vitamin K antagonists or with non-vitamin K oral ant...

Carotid artery disease and perioperative stroke risk after surgical aortic valve replacement: A nationwide inpatient sample analysis
In conclusion, perioperative stroke risk has remained more or less constant despite advancements in surgical techniques with risk having gone up in patients <65years of age. CS and cerebral arterial occlusion significantly increase stroke risk following SAVR. Improved patient selection with pre-operative risk stratification and institution of preventive strategies are necessary to improve operative outcomes following SAVR.

The Relation between Plasma Gelsolin Levels and Prognosis in Patients with Acute İschemic Stroke upon Admission to Emergency Department
To compare serum gelsolin levels and Glasgow Coma Scale (GCS) scores in patients with acute ischemic stroke and to compare with healthy control subjects. Included in this study were patients with onset of symptoms in the first 3 h after acute ischemic stroke. Healthy volunteers were included as the control group. Patients with chronic obstructive pulmonary disease, diabetes mellitus, acute coronary syndromes, renal failure, heart failure, trauma, hepatic failure, infection, sepsis, and hematologic and oncologic disease were excluded from study. Venous blood samples were obtained from the patients and the control group upo...

Hypertension is Associated with Increased Mortality after Ischemic Stroke in Children (S42.003)
Conclusion: The observation that HTN was associated with a 3 times greater odds of death in the hospital for children presenting with ischemic stroke requires further study. HTN is a potentially modifiable risk factor for poor outcome.Disclosure: Dr. Caceres has nothing to disclose. Dr. Malik has nothing to disclose. Dr. Qureshi has nothing to disclose. Balancing stroke and bleeding risks in patients with atrial fibrillation and renal failure: the Swedish Atrial Fibrillation Cohort study
Conclusions Patients with both AF and renal failure will probably benefit most from having the same treatment as is recommended for other patients with AF, without setting a higher or lower threshold for treatment. Adding additional points for renal failure to the CHADS2 and CHA2DS2-VASc scores did not improve their predictive value.

Off-Pump Coronary Artery Bypass Reduces Early Stroke in Octogenarians: A Meta-Analysis of 18,000 Patients.
CONCLUSIONS: Coronary artery bypass in octogenarians can be performed safely with low early mortality. Although off-pump operations reduce the risk of early stroke, all other adverse events are comparable in on- and off-pump coronary artery bypass operations. Data regarding late mortality is at present limited; however, both on- and off-pump procedures appear to produce comparable survival. PMID: 25791924 [PubMed - as supplied by publisher]

Acute ischemic stroke and abnormal blood hemoglobin concentration
ConclusionsOur results suggest that an elevated HGB on the initial admission is associated with more severe strokes, greater disability at discharge, and higher 30‐day mortality after AIS. A low HGB on admission is associated with longer stay in the acute care hospital.

Atrial Fibrillations Effect on the Rate of Ischemic Stroke, Death and Disability in Subarachnoid Hemorrhage (P1.256)
Conclusion: There are significantly decreased rates of favorable outcomes in subarachnoid hemorrhage patients with atrial fibrillation. Variables affecting Stroke Outcome for TPA received between 3 to 4.5 hours (P2.301)
Conclusions: Age, NIHSS and renal dysfunction were significantly associated with worse outcome in patients who had received t-PA between 3 to 4.5 hours. Our findings were consistent but more robust than previous studies showing poorer outcome with renal dysfunction (Meta-analysis OR1.06 NS). After controlling for age and NIHSS, diabetes or hypertension did not additionally contribute to poor outcome. Prognostic and risk stratification scales are useful as predictors of outcome but do not imply efficacy of t-PA therapy or lack thereof.Disclosure: Dr. Yeo has nothing to disclose. Admission Hydration Status and Ischemic Stroke Outcome-Experience from a National Registry of Hospitalized Stroke Patients
J Clin Med. 2021 Jul 26;10(15):3292. doi: 10.3390/jcm10153292.ABSTRACTReduced intravascular volume upon ischemic stroke (IS) admission has been associated with in-hospital complications, disability, and reduced survival. We aimed to evaluate the association of the urea-to-creatinine ratio (UCR) with disability or death at discharge, length of stay, in-hospital complications, and mortality during the first year. Using a national registry, we identified hospitalized IS patients without renal failure. Disability or death at discharge, length of stay, in-hospital complications, and mortality during the first year were studied ...

Massive acute ischemic stroke after Bothrops spp. envenomation in southwestern Colombia: Case report and literature review
We present here the case of a 50-year-old male with no comorbidities referred from a rural area in southwest Colombia with a Bothrops spp. snakebite on the left hand. On admission, the patient presented with a deterioration of consciousness and required mechanical ventilation assistance. The MRI showed multiple ischemic areas in the bilateral frontaltemporal and occipital regions. Two months later, the patient had a favorable resolution, although central paresis in the III and VI cranial nerves and positive Babinski's sign persisted. As already mentioned, the pathophysiology of ischemic stroke due to snakebite is complex b...
